Addiction is a neurological disorder that affects the reward system in the brain. In a healthy person, the reward system reinforces important behaviors that are essential for survival such as eating, drinking, sex, and social interaction. For example, the reward system ensures that you reach for food when you are hungry, because you know that after eating you will feel good. In other words, it makes the activity of eating pleasurable and memorable, so you would want to do it again and again whenever you feel hungry. Drugs of abuse hijack this system, turning the person’s natural needs into drug needs. The brain consists of billions of neurons, or nerve cells, which communicate via chemical messages, or neurotransmitters. When a neuron is sufficiently stimulated, an electrical impulse called an action potential is generated and travels down the axon to the nerve terminal. Here, it triggers the release of a neurotransmitter into the synaptic cleft - a space between neurons. The neurotransmitter then binds to a receptor on a neighboring neuron, generating a signal in it, thereby transmitting the information to that neuron. The major reward pathways involve transmission of the neurotransmitter dopamine from the ventral tegmental area – the VTA - of the midbrain to the limbic system and the frontal cortex. Engaging in enjoyable activities generates action potentials in dopamine-producing neurons of the VTA. This causes dopamine release from the neurons into the synaptic space. Dopamine then binds to and stimulates dopamine-receptor on the receiving neuron. This stimulation by dopamine is believed to produce the pleasurable feelings or rewarding effect. Dopamine molecules are then removed from the synaptic space and transported back in to the transmitting neuron by a special protein called dopamine-transporter. Most drugs of abuse increase the level of dopamine in the reward pathway. Some drugs such as alcohol, heroin, and nicotine indirectly excite the dopamine-producing neurons in the VTA so that they generate more action potentials. Cocaine acts at the nerve terminal. It binds to dopamine-transporter and blocks the re-uptake of dopamine. Methamphetamine – a psychostimulant – acts similarly to cocaine in blocking dopamine removal. In addition, it can enter the neuron, into the dopamine-containing vesicles where it triggers dopamine release even in the absence of action potentials. Different drugs act different way but the common outcome is that dopamine builds-up in the synapse to a much greater amount than normal. This causes a continuous stimulation, maybe over-stimulation of receiving neurons and is responsible for prolonged and intense euphoria experienced by drug users. Repeated exposure to dopamine surges caused by drugs eventually de-sensitizes the reward system. The system is no longer responsive to everyday stimuli; the only thing that is rewarding is the drug. That is how drugs change the person’s life priority. After some time, even the drug loses its ability to reward and higher doses are required to achieve the rewarding effect. Drugs are bad. They are very bad! According to numerous studies, about 1 in 20 people in the world abuse drugs to some degree. That’s about 230 million people, and that is a lot of druggies walking around the planet. With new drugs being created and tested each day, it seems that there is no end to the infamous “War on Drugs” and it seems that we’re losing, considering the amount of people who abuse drugs. There have been countless arguments and debates over what exactly is a drug, since marijuana is illegal while nicotine and alcohol, which have shown to be just as addicted as illegal narcotics with similar withdrawal effects, are perfectly legal. When a drug is abused over time, such as methadone, or amphetamines, they create an altered euphoric state of mind that messes with the brain. These drugs can actually force the brain to become more dependent on elevated levels of various brain chemicals that it cannot produce on its own without drugs. See how these drugs can become addictive? What tragically happens often is that a drug abuser will go get treatment and get clean, then they go back to the original amounts of drugs they were abusing before and end up dying from an overdose. Many of these drugs can be created from household products like cough syrup and bleach. Or, they can be prescriptions from a doctor that end up getting overused and abused. The drug market has become one of the most dangerous businesses to get into, as numerous gangs and mafias use drugs to make money and fund their other illegal activities. The rise in the use of drugs inspired the United States to create the D.A.R.E. program, as well as create ads such as the iconic commercial of a girl happily singing a song while displaying clear effects of using meth. If you haven’t done a search on what people look like before and after drugs, then you will be astounded as to how much drugs can change a person’s appearance, let alone their mental state and brain function. For example, cocaine creates an extreme high that lasts for about 15-20 minutes and gives the brain an overload of dopamine. This overload causes the brains natural dopamine receptors to shut down. What ends up happening is that once the high is over, the body will start to crave more and more, and thus, beginning a cocaine addiction that is hard to shake. The same effect happens in crack cocaine as well, which is a free form version of regular cocaine. However, if there is one drug that is the absolute worse to get addicted to, then it is definitely heroin. It is considered to be one of the most addictive drugs ever created. It causes the brain to release 100 times the amount of dopamine than an orgasm, and creates a euphoric and numb high that can last longer than other drugs. Parents publicly share daughter's losing battle with heroin addiction. Heroin use is on a dramatic rise and so has the face of addiction. The number of people being treated for addiction has more than doubled in the past decade and potentially deadly habit seems to currently affect young people ages 18-25 the most. Jessica Williams is one such person. She battled her addiction with heroin and lost. Her parents are sharing her story in a very public way in hopes of making a difference. “I’m thinking there's no way this is my baby. This is more than anyone should have to take,” said Wayne Williams, Jessica’s father. The grieving father says this as he sits on a bed in one of the family’s bedrooms. It’s the same bedroom Jessica Williams, a Blue Springs high school graduate, lost her battle to heroin in. “As strong and as stubborn and as smart as Jessica was, heroin was stronger,” Wayne Williams said. Jessica’s mother was notified of her death at work. “I knew. They all had tears in their eyes. I just slid down the wall and I just sat there and I just kept saying ‘no,’” Debbie Williams said. Most families keep addictions and grief behind closed doors, but the Williams family took it outside. Anyone who drives down West Broadway Boulevard, or U.S. Highway 50, in Sedalia sees a sign. “I was proud of it,” Wayne Williams said. “It makes a point. She died here.” It has a picture of Jessica on it, looking very beautiful with her makeup and hair perfectly in place. Next to the photo is text that says. “Jessica Williams, 22 died HERE Feb. 10th, 2016 of a HEROIN OVERDOSE. Heroin is here in Sedalia, and your silence may kill others. TALK ABOUT IT!” “Heroin is bad and everyone needs to know how bad it is,” Debbie Williams said. “You can buy a hit of heroin cheaper than you can buy a six-pack of beer,” Wayne Williams said. Jessica Williams was an excellent student, she went to college and planned on law school. Her parents never dreamed heroin would destroy their youngest daughter. “Is she going to be in jail? Is she going to be dead? I never knew when I’d get a phone call. Every time my phone rang I didn't even want to answer it,” Debbie Williams said. The family tried everything - three rounds of rehab, constant counseling. Matching tattoos of a compass were meant Jessica Williams to keep going in the right direction. But in the end heroin destroyed everything. “I don’t want any parent to go through this. I don’t want any parent to ever, ever feel this hurt. It’s an abyss you can't climb out of,” Wayne Williams said. He plans to keep the sign up in front of the house though the summer. He said thousands of people pass his house on the way to the Lake of the Ozarks. How they deliberately covered up and said it wasn't addictive. How they persuaded Doctors to continue prescribing OxyContin by buying their way into their pockets with money, gifts, expensive dinner and trips. It was touted as a miracle pill: a narcotic pain reliever that could change the lives of people suffering from chronic pain, but with little -- so its maker claimed, and thousands of doctors believed -- risk of addiction. Since OxyContin was introduced in 1996, Canada has recorded the second-highest number of prescription opioid painkiller addictions -- and the world's second-highest death rate from overdoses. "It's helping your pain, but then you get immune to it, so then you go to the family doctor and he says 'Well, you're gonna need more,'" a woman who became addicted to OxyContin tells the fifth estate's Linden MacIntyre. "So he puts you on the 40 milligram (dose) and you're on that for a month, and then you get used to that dose and he puts you on the 80s." But how did a little pill that only appeared in 1996 become so big, so fast? In 1998, Canadian sales were just a few million dollars. Twelve years later they had soared to $243 million. In the U.S., sales were $3.5 billion in 2010. Though there were differences in corporate style and legal structure between Purdue in the U.S and in Canada, a similar marketing approach proved wildly successful. the fifth estate examines why medical schools, GPs and specialists in pain clinics readily embraced the drug at first, and why some have now changed their minds. OxyContin was dropped from provincial health plans in Ontario, Saskatchewan and Atlantic Canada. The manufacturer has now stopped making it altogether, replacing it with a new formulation known as OxyNeo. But is it too little, too late? Did the drug's maker low-ball the risks? Did they know their time-release miracle pill was really a time bomb of addiction, waiting to go off?
