Government programs, like Medicare and Medicaid, can ask for a lower price from health service providers because they have the numbers: the hospital has to comply or else risk losing the business of millions of Americans. There are dozens of private health insurance providers in the United States and they each need to bargain for prices with hospitals and doctors. The numbers of people private insurances represent are much less than the government programs. That means a higher price when you go to the doctor or fill a prescription. Uninsured individuals have the least bargaining power. Without any insurance, you will pay the highest price. Top 7 Type 2 Diabetes Medications

Some people who have type 2 diabetes can achieve their target blood sugar levels with diet and exercise alone, but many also need diabetes medications or insulin therapy. The decision about which medications are best depends on many factors, including your blood sugar level and any other health problems you have. Your doctor might even combine drugs from different classes to help you control your blood sugar in several different ways. 1. Metformin Generally, metformin is the first medication prescribed for type 2 diabetes. It works by improving the sensitivity of your body tissues to insulin so that your body uses insulin more effectively. Metformin also lowers glucose production in the liver. Metformin may not lower blood sugar enough on its own. Your doctor will also recommend lifestyle changes, such as losing weight and becoming more active. 2. Meglitinides These medications work like sulfonylureas by stimulating the pancreas to secrete more insulin, but they're faster acting, and the duration of their effect in the body is shorter. They also have a risk of causing low blood sugar, but this risk is lower than with sulfonylureas. Weight gain is a possibility with this class of medications as well. Examples include repaglinide (Prandin) and nateglinide (Starlix). 3. Thiazolidinediones Like metformin, these medications make the body's tissues more sensitive to insulin. This class of medications has been linked to weight gain and other more-serious side effects, such as an increased risk of heart failure and fractures. Because of these risks, these medications generally aren't a first-choice treatment. Rosiglitazone (Avandia) and pioglitazone (Actos) are examples of thiazolidinediones. 4. Sulfonylureas These medications help your body secrete more insulin. Examples of medications in this class include glyburide (DiaBeta, Glynase), glipizide (Glucotrol) and glimepiride (Amaryl). Possible side effects include low blood sugar and weight gain. 5. GLP-1 receptor agonists These medications slow digestion and help lower blood sugar levels, though not as much as sulfonylureas. Their use is often associated with some weight loss. This class of medications isn't recommended for use by itself. Exenatide (Byetta) and liraglutide (Victoza) are examples of GLP-1 receptor agonists. Possible side effects include nausea and an increased risk of pancreatitis. 6. SGLT2 inhibitors These are the newest diabetes drugs on the market. They work by preventing the kidneys from reabsorbing sugar into the blood. Instead, the sugar is excreted in the urine. Examples include canagliflozin (Invokana) and dapagliflozin (Farxiga). Side effects may include yeast infections and urinary tract infections, increased urination and hypotension. 7. DPP-4 inhibitors These medications help reduce blood sugar levels, but tend to have a modest effect. They don't cause weight gain. Examples of these medications are sitagliptin (Januvia), saxagliptin (Onglyza) and linagliptin (Tradjenta).

Chris Pope, of the Manhattan Institute’s City Journal, tells John Stossel that the results aren’t as great as progressives claim. “ In England there is rarely a week that goes by without a crisis or another in the healthcare system being part of the news. This year there was a crisis in emergency room care. People left in the hallways for hours and hours. “ When Stossel asks if single-payer isn’t the solution, what is? Pope suggest more competition between hospitals, and adds "We currently have a system based on employers picking which healthcare plan is good for employees. If we move towards a healthcare system where individuals were more responsible for shopping around. People would choose a better system.” Under our current system, government and insurance spend 7 out of 8 health care dollars for us. Pope says "the question is, do we improve on the healthcare system by empowering consumers, or do we basically just say, 'This is the government plan. Deal with it.'”

In 1982, Dr. Lipton began examining the principles of quantum physics and how they might be integrated into his understanding of the cell’s information processing systems. He produced breakthrough studies on the cell membrane, which revealed that this outer layer of the cell was an organic homologue of a computer chip, the cell’s equivalent of a brain. His research at Stanford University’s School of Medicine, between 1987 and 1992, revealed that the environment, operating though the membrane, controlled the behavior and physiology of the cell, turning genes on and off. His discoveries, which ran counter to the established scientific view that life is controlled by the genes, presaged one of today’s most important fields of study, the science of epigenetics. Two major scientific publications derived from these studies defined the molecular pathways connecting the mind and body. Many subsequent papers by other researchers have since validated his concepts and ideas. Dr. Lipton’s novel scientific approach transformed his personal life as well. His deepened understanding of cell biology highlighted the mechanisms by which the mind controls bodily functions, and implied the existence of an immortal spirit. He applied this science to his personal biology, and discovered that his physical well-being improved, and the quality and character of his daily life was greatly enhanced. Dr. Lipton has taken his award-winning medical school lectures to the public and is currently a sought after keynote speaker and workshop presenter. He lectures to conventional and complementary medical professionals and lay audiences about leading-edge science and how it dovetails with mind-body medicine and spiritual principles. He has been heartened by anecdotal reports from hundreds of former audience members who have improved their spiritual, physical and mental well being by applying the principles he discusses in his lectures. He is regarded as one of the leading voices of the new biology. Especially if you are travelling, you will often have to talk about money and the price of items and services. That’s why you need this lesson on ten words for talking about money and prices. I will teach you words like “rate”, “charge”, “fee”, “commission”, “cashback”, “coupon”, and more. For each word, I will give you an example of a sentence you can use it in. In today's lesson we are going to have ten words that come from the world of banking and business, but have an everyday function; that have a function if you are a holidaymaker and are looking at exchanging money, for example, or just going to another place and using money. Let's face it - we've all got to use it. So, first word today: "rate". There's a couple of different meanings of this word. It can mean the cost of something; or used as a verb: "What do you think of something?" Let's have some practical examples. "What is your rate for...?" Or: "Give me your best rate." What we're... We're thinking here is... Is sort of price; cost. "What is your best rate? I want your best rate." Particularly if you go to a country where haggling is more of a norm, then you're going to be trying to negotiate. Here in the UK things tend to be... there tends to be a set price for certainly things in a shop. "How would you rate...?" So: "How would you rate the service provided? What do you think of it?" Moving on to our second word: "commission". Okay? This word, if we're thinking of a slice... You know, a whole pizza, and there is a middle man... So, for example, I'm booking a hotel in India and I have someone there who is doing the booking for me, then he's going to take a small slice of the pizza and eat it for himself, and the rest of the pizza will go to the hotel. Metaphor; obviously we're talking about money. A practical example of "commission": "We charge 0% commission." Let's put this into another context. Let's say I'm going to Spain and I want some euros so that I can have some money there, I go along to the sort of money exchange, and they say: "We charge 0% commission." That's great. Just be warned that their rates may be not so friendly; they're going to make their money from the rate, not the commission. Next word: "charge". Now, this can be both a noun and a verb. "Are there any extra charges?" Okay? Say I'm paying for a hotel in Spain: "Are there any extra charges?" Taxes, for example. Used as a verb: "What do you charge? What do you charge?" Or: "My charge is..." Okay? If you're saying what you'd like to be paid for a day's work, for example. "Fee". This is a noun. Again, it relates to payment. "My fee is..." Or: "The fee for this is..." Or: "We do charge a fee of..." Okay? Saying we're still at the money exchange: "We do charge a fee of..." All right. "Cost" and "price", this, relating to amount of money. "So the overall cost for changing 100 pounds into euros is..." Or... "So the overall cost for 100 euros is 80 pounds." Okay? "Cost". Next words: "transfer". So, a word that may pop up in your science textbook; it's really relating to this idea of change, and it can be used both as a noun and as a verb. We're going off on this holiday to Spain. "I just received a transfer from my friend." That means they changed money from their account into my account. Well, hey, off to the beach. Pina colada for me. Or: "I made my transfer..." Let's say, for example, that I was flying from London to Paris; from Paris to Barcelona - I don't know why I would because you could fly straight, but let's just pretend: "transfer" is getting the changing flight. So, it's making sure that when you're in Paris, you get onto that change; that next flight - the change of flight. So, there I am in Spain. Now there's been a new development - a plot development because I have just had a "cut" in my salary. Now, "cut" means something taken off. So I now have less money that I am being paid each month. Because of this, I am going to need to make a "cutback". "Cutback" is a noun; means a reduction. I'm going to need to make some cutbacks in my spending. Now, you can also use this as a phrasal verb. For example: "I will need to cut back..." […]
