Thyroid exam: How to check your thyroid with Dr. Christianson http://drchristianson.com/blog Today we will examining our thyroid. Thyroid cancer is the fastest increase of cancer in women. THere are many symptoms of possible thyroid issues such as fatigue, weight loss, and weight gain. In this video Dr. Christianson demos how to check your thyroid at home. This can be done with just some water and a mirror it is neither invasive or time consuming. A great idea to check your thyroid consistently is to perform once a month with a self breast exam. Is you suspect an issue schedule an appointment with your physician. If you have hyperthyroidism or hypothyroidism make sure you are getting an ultrasound at least on an annual basis to monitor your thyroid effectively. When visiting your physician here are a few test they can perform to give you a better idea of your thyroid heath. 1. TSH test You want to check the hormones released by the thyroid, as well. The TSH checks the pituitary, which tells the thyroid how to work. The hormones released include the T3 and T4. The free versions of those hormones are more meaningful than the total version. If it does not say anything besides just T3 or T4, this is the total version and not the free. 2. It is also good to check thyroid antibodies. If your body is attacking your thyroid, the gland typically gets diseased. So, you want to see if your body is attacking it. There is the thyroglobulin antibody (TG) and the thyroid peroxidase antibody (TPO). 3. There is also thyroglobulin which is not the antibody, just thyroglobulin. Thyroglobulin is a good measurement on how irritated or inflamed your thyroid is. 4. You also want to check reverse T3. Reverse T3 is a by-product of your body getting rid of T4, and this can be abnormal, as well. The last thing I encourage for those who suspect thyroid disease is to get an ultrasound. There are many times when patients’ labs are normal, or they don’t have measurable antibodies, yet an ultrasound shows they have nodules, goiters, calcifications, clear signs of Hashimoto's. In terms of the labs, the one test that has the biggest difference in ranges from healthy populations to populations typically tested is the TSH test. Some argue that the free hormones (like the free T3) should be on the higher end of the range. I do not see data to support that. The healthy populations show a large range of T3 and a large range of T4. They are not all in the upper end of the range. The TSH scores in the healthiest populations are at the lower end of the range. So, being on the lower end of the range means you’re further away from being hypothyroid. Specifically, numbers above 1.9 are pretty suspicious. If your TSH is above 1.9, and you have some strong symptoms and some structural abnormalities, that's thyroid disease. New drug shows improvement in survival of advanced thyroid cancer

In a pivotal Phase III study led by researchers at The University of Texas MD Anderson Cancer Center, the oral anti-angiogenic therapy lenvatinib has shown dramatic improvement in progression-free survival in patients with advanced radioiodine-refractory thyroid cancer. The global study, led by Steven Sherman, M.D., associate vice-provost for Clinical Research, and professor and chair, Endocrine Neoplasia and Hormonal Disorders, is published in the New England Journal of Medicine, and could offer a new treatment paradigm for a group of patients for whom, until recently, there has been no new effective treatment since the 1940s. According to the American Cancer Society, 62,450 people will be diagnosed with thyroid cancer in 2015 and 1,950 will die from the disease. It’s the fastest growing cancer type, says Sherman, with rates of refractory disease also on the rise. Until recent therapeutic advances, historically, radioactive iodine has been the only treatment available to patients with metastatic thyroid disease. For patients with advanced and metastatic disease, lenvatinib and this class of drugs is the first potential for improved overall survival. A number of follow up studies with lenvatinib are in development, including in other types of thyroid cancers, and in combination with other novel therapies for radioiodine refractory patients. The thyroid gland controls how quickly the body uses energy, makes proteins, and controls how sensitive the body is to other hormones. It participates in these processes by producing thyroid hormones, the principal ones being triiodothyronine (T3) and thyroxine (sometimes referred to as tetraiodothyronine (T4)). These hormones regulate the growth and rate of function of many other systems in the body. T3 and T4 are synthesized from iodine and tyrosine. The thyroid also produces calcitonin, which plays a role in calcium homeostasis. People can suffer from hyperthyroidism. An overactive thyroid, is defined as an overproduction of the thyroid hormones T3and T4. This condition is most commonly caused by the development of Graves' disease, an autoimmune disease in which anomalous antibodies stimulate the thyroid to secrete excessive quantities of thyroid hormones.The disease can progress to the formation of a toxic goiter as a result of thyroid growth in response to a lack of negative feedback mechanisms. It presents with symptoms such as a thyroid goiter, protruding eyes (exophthalmo), palpitations, excessive sweating, diarrhea, weight loss, muscle weakness and unusual sensitivity to heat. The appetite is often increased. People can also suffer hypothyroidism. Hypothyroidism is the underproduction of the thyroid hormones T3 and T4. Hypothyroid disorders may occur as a result of # Congenital thyroid abnormalities # Autoimmune disorders such as Hashimoto's thyroiditis, # Iodine deficiency # The removal of the thyroid following surgery to treat severe hyperthyroidism and/or thyroid cancer. Typical symptoms are abnormal weight gain, tiredness, baldness, cold intolerance, and bradycardia. Hypothyroidism is treated with hormone replacement therapy, such as levothyroxine, which is typically required for the rest of the patient's life. Hormones released by the gland travel through your bloodstream and reach nearly every part of your body, which include the heart, brain, muscles and skin.  Hypothyroidism may be due to a number of factors which are- Autoimmune disease. People who are affected by a particular inflammatory disorder is known as Hashimoto's thyroiditis, the most common cause of hypothyroidism. Autoimmune disorders take place when your immune system produces antibodies that destroy your own tissues. These antibodies affect the thyroid and therefore it is unable to produce hormones.  Let us get a clearer picture on the causes of hypothyroidism: 1. Medication: There are certain medicines that cure heart deceases or even cancer. These medicines affect the thyroid gland making it underactive. 2. Hashimoto decease: In this autoimmune decease, the immune system destroys the thyroid cells. When this happens, there is malfunctioning of the gland which in turn decreases the secretion of hormones. 3. Iodine deficiency: This is another reason for hypothyroidism since iodine is responsible for producing thyroid hormones. Lack of iodine in the body triggers this problem. 4. Pituitary disorder. A fairly rare cause of hypothyroidism is because the pituitary gland fails to produce enough thyroid-stimulating hormone (TSH). This normally is because of a benign tumor of the pituitary gland.  5. Pregnancy. Some women develop hypothyroidism during or after pregnancy. This is because they produce antibodies to their own thyroid gland. If this condition is left untreated, hypothyroidism may cause a miscarriage, premature delivery and preeclampsia — a condition that causes a woman's blood pressure to rise during the last three months of pregnancy. It can also seriously affect the developing fetus.  6. Iodine deficiency. The mineral iodine, which is found mainly in seafood, seaweed, plants grown in iodine-rich soil and iodized salt — is vital to produce thyroid hormones. At the same time, consuming too much iodine can also cause hypothyroidism.  Symptoms:  Exhaustion and a slower metabolism are common symptoms associated with hypothyroidism. You may also experience several other symptoms that are:  1. Depression. Hypothyroidism affects the brain .This can make it difficult to concentrate which could result in clinical depression.  2. Memory problems. In some cases, hypothyroidism can cause memory loss.  3. Frequent constipation. Hypothyroidism also weakens the digestive system, which can lead to chronic constipation.   4. Dry skin. The Thyroid hormone is vital for a healthy skin. Therefore dry skin is a common symptom among people with hypothyroidism.  5. Hair loss. People have been diagnosed with hypothyroidism at a later stage, may suffer from hair loss of their scalp.  6. Changes in the menstrual cycle.  Women, who suffer from hypothyroidism, may have a longer or a shorter menstrual period.  Home remedies for hypothyroidism are mentioned below: 1. Coconut oil: Add two tablespoons of coconut oil to a glass of milk and have it in the morning. Regularly doing this can help you get rid of hypothyroidism. This is one way to deal with thyroid. 2. In this video, Dr. Daniel Kelly, Director of the Saint John's Brain Tumor Center & Pituitary Disorders Program discusses the essential hormonal functions of the pituitary gland, its anatomy in relation to the surrounding skull base and brain, and the wide-ranging effect of pituitary adenomas and related tumors which may include hormonal abnormalities, visual loss and headaches. The pituitary gland is known as the master gland, because it regulates the function of the great majority of endocrine glands throughout the body. These functions include regulating the body's stress response (through adrenocorticotropic hormone -- ACTH), metabolism (through thyroid stimulating hormone -- TSH), tissue and bone growth (through growth hormone -- GH), sexual function and reproduction (through luteinizing hormone -- LH and follicular stimulating hormone -- FSH), breast feeding and milk production (through prolactin & oxytocin), and water and electrolyte balance (through vasopressin also known as antidiuretic hormone -- ADH). Pituitary tumors and cysts can cause pituitary gland dysfunction by compressing the gland affect its various functions. Symptoms of pituitary hormonal dysfunction (under-production) include fatigue, decreased energy, weight gain, light-headedness, growth delay (in children), infertility, sexual dysfunction, increased urination and increased thirst, to name a few. The most common pituitary tumors are pituitary adenomas of which there are two major categories. Endocrine-active pituitary adenomas produce excess hormones and include those causing acromegaly from excess GH, Cushing's disease from excess ACTH, prolactinomas from excess prolactin and the rare TSH-secreting adenomas. In contrast, endocrine-inactive adenomas do not produce excess hormones and typically cause symptoms of "mass effect" which may include hormonal loss (hypopituitarism), visual loss, headaches and tumor bleeding (pituitary apoplexy). The other common tumors that may arise in the region of the pituitary gland include craniopharyngiomas, Rathke's cleft cysts, meningiomas and clival chordomas. This video provides a brief description of all these tumor types. Given the complexity of diagnosing and optimally treating pituitary tumors and related hormonal disorders this video also stresses the importance of a multidisciplinary approach to evaluating and treating pituitary patients that includes neurosurgeons, otolaryngologists (ENT surgeons), endocrinologists, neuro-ophthalmologists, radiation oncologists, neuro-pathologists and neuro-radiologists. The great majority of symptomatic pituitary tumors will warrant treatment with endonasal endoscopic surgery although prolactinomas can typically be treated with dopamine agonist medical therapy. Coffee is beneficial because it contains massive amounts of antioxidants, which help slow the aging process and protect you from free radical damage. Coffee also helps stimulate the liver and increases circulation, helping blood flow to the brain. Coffee can be harmful in some ways because of the amount of caffeine it contains. The caffeine in coffee can overstimulate the body, burn out your adrenal glands, and overtax your body. About 80% of people in the world will struggle with thyroid problems or fatigue issues. For this group, I wouldn’t recommend coffee because it will burn out your adrenal glands. However, if you’re generally healthy, then drinking coffee in moderate amounts can be fine, and even good for your health. I wouldn’t recommend drinking coffee past lunch time because it can affect your sleep cycle that night. Depending on the condition of your body, if you’re suffering from an autoimmune disease, coffee can make your disease worse. If you’re pretty healthy, looking for a pick-me-up, or looking for more antioxidants, then coffee can provide you these benefits. When picking out your coffee beans, I would recommend choosing all natural, organic coffee because it’s one of the most highly sprayed plants in the world.
