A ketogenic diet is not a high protein diet, it's only consuming 3-6 oz of protein with each meal. There is a condition called ketoacidosis, which is different that nutritional ketosis, and your pH does not raise to any danger levels. Keto can also increase the uric acid, but if you consume enough vegetables, again, you will be fine. The potassium in vegetables is very kidney protective. But high sugar, and high insulin is damaging to the kidney more than anything else. Take a look at a diabetic - they usually end up with long term kidney damage. GMO foods can also effect the kidney, so eat organic. I had another question relating to does ketogenic diet cause kidney disease. Number 1, ketogenic diet is a very natural thing for the body to do. In fact I believe that is was the very first diet that we all were on and our bodies are very used to it. It is a much better fuel running your body on ketones. And the confusion is that a ketogenic diet is a high protein diet and of course high proteins can damage the kidney and the liver as well but it is not but it is not a high protein diet and you are only doing 3 to 6 ounces of protein with each meal. The ketogenic diet is a low carb diet and low carbs are very beneficial to the kidney. High carbohydrates and high sugars are very damaging to the kidney. Let’s take a look at the diabetic, they usually have vision problems, nerve problems and eventually kidney problems and they usually end up kidney dialysis because that amount of sugar is very damaging to the kidneys. High carbohydrates destroy the kidney and low carbohydrates are very safe for the kidney. There is a condition when you have diabetes called ketoacidosis and that means that the pH coming from those ketones and this is from the very unhealthy ketosis by the way. It is very different on the type of ketosis I’m talking about. It is more of a side effect because the body is burning up protein and is turning on ketones and the pH is coming very acid. But you don’t have to worry about that because when you are actively doing just by cutting out the carbs and you are not a diabetic type 1. I’m gonna recommend and I am recommending a lot of vegetables to get your potassium and potassium is very protective on the kidneys. Until you have complete kidney failure and you have kidney dialysis, in that situation you don’t want to consume a lot of protein because you can’t really utilize that protein that’s a completely different situation. But in general, potassium is very protective on kidney and that is why we recommend 7 to 10 cups of vegetables with the ketogenic diet plan to protect the kidney and give it the nutrients that it needs. Another problem with keto is that it increases the uric acid. It is not gonna do that if you keep your vegetables high, but some people can get uric acid stones, gout, simply because they are not consuming enough vegetables. Intravenous Light Therapy Provides Relief for People With Lyme Disease - NBC 22-05-2017

Lyme disease is already a tough disease to diagnose. But a new treatment that we've never seen before might make it easier to cope with the symptoms. A new treatment is giving some people with Lyme disease relief. Lyme disease can cause neck pain, joint pain, muscle aches, muscle fatigue and a lot of fatigue in general. Kathy Fowler Silverstein has struggled with some of Lyme's most serious symptoms, for years. She said she's tried all the treatments. While some have helped, some have been a waste of time. Lately, she's been getting relief from light injected directly into her veins. “After a few treatments, I thought this really seems to be working,” she said. A German doctor developed intravenous laser therapy 10 years ago. Andrew Bloch, a local physical therapist who also does acupuncture, went to Germany last December to learn more about ILT. “They wanted to come up with other ways to utilize different types of light into the body, mostly blue, yellow and green lights, but those don't penetrate through the skin,” he said. A German doctor came up with a system that introduces light underneath the skin. Bloch started offering the new treatment at his pain and rehab clinic in Rockville a few months ago. Four different lights each have a different application: Red helps with cellular regeneration and stimulating the immune system. Yellow light is vitamin D and serotonin effect. Green light is more oxygen uptake like a plant. Blue has a cooling effect and is anti-inflammatory. The lights are administered individually for about 10 minutes each. The light therapy has been approved by the FDA but only on the skin for things like muscle pain and arthritis. Bloch typically uses all four lasers with his patients who have Lyme and other hard to treat diseases. Heidi Foster has an autoimmune illness, and many of her symptoms are similar to those of Lyme disease. “I have pain all over and each day it could radiate to somewhere else,” she said. “One day in my wrist, another day my knees, my neck, my lower back.” There are no known side effects of the treatment, Bloch said, and it's making a difference for some of his patients, who have seen their pain levels go way down. Other uses for ILT include multiple sclerosis, fibromyalgia and depression. A lot of you who were doing a ketosis, I want to show you what to do to prevent kidney stones because when you are in a ketosis diet you do get rid of calcium in the urine more than usual. When you are consuming cruciferous vegetables which I am always recommending spinach all that have some high amount of oxalates. The oxalates combine with the calcium to make stones. Kale is very low in oxalates compared to spinach, spinach is off the charts. All you have to do is to not necessarily avoid calcium, take some food based calcium whether it is from cheese or something like that because if you consume dietary food calcium, the high levels of oxalates will bind in the digestive system. Instead of going into the blood and out to the urine, to the kidney. In other words, if you are not consuming enough calcium in foods, that calcium in the digestion would protect the kidneys by taking more calcium and never ends up in the kidney. The other mechanism for stones is that you are low in citrates. There is a certain thing in the body called the citrates and when you don’t have enough of that, you can be more at risk of kidney stones, so it is recommended to take the lemon juice on the regular basis. Maybe 2 or 3 ounces of lemon juice in a kale shake, or you can start putting lemon in the water you can do it just the liquid or the actual lemon but at least one lemon a day that will also decreases the risk of kidney stones. The other thing is to make sure that you have enough magnesium, that’s going to come from the vegetables and can also be taken as a supplement too. Kidney stone facts A kidney stone is a hard, crystalline mineral material formed within the kidney or urinary tract. Nephrolithiasis is the medical term for kidney stones. Symptoms of a kidney stone include flank pain (the pain can be quite severe) and blood in the urine (hematuria). Kidney stones form when there is a decrease in urine volume and/or an excess of stone-forming substances in the urine. Dehydration is a major risk factor for kidney stone formation. People with certain medical conditions, such as gout, and those who take certain medications or supplements are at risk for kidney stones. Diet and hereditary factors are also related to stone formation. Diagnosis of kidney stones is best accomplished using an ultrasound, IVP, or a CT scan. Most kidney stones will pass through the ureter to the bladder on their own with time. Treatment includes pain-control medications and, in some cases, medications to facilitate the passage of urine. If needed, lithotripsy or surgical techniques may be used for stones which do not pass through the ureter to the bladder on their own. A kidney stone, also known as a renal calculus or nephrolith, is a calculus formed in the kidneys from minerals in the urine. Kidney stones typically leave the body by passage in the urine stream, and many stones are formed and passed without causing symptoms. If stones grow to sufficient size (usually at least 3 millimeters (0.12 in)) they can cause blockage of the ureter. This leads to pain, most commonly beginning in the flank or lower back and often radiating to the groin or genitals. This pain is often known as renal colic and typically comes in waves lasting 20 to 60 minutes. Other associated symptoms include: nausea, vomiting, fever, blood in the urine, pus in the urine, and painful urination. The diagnosis of kidney stones is made on the basis of information obtained from the history, physical examination, urinalysis, and radiographic studies. Ultrasound examination and blood tests may also aid in the diagnosis. Urinary stones are typically classified by their location in the kidney (nephrolithiasis), ureter (ureterolithiasis), or bladder (cystolithiasis), or by their chemical composition (calcium-containing, struvite, uric acid, or other compounds). Slightly more men are affected than women. Blockage of the ureter causes decreased kidney function and dilation of the kidney. In those who have previously had stones, prevention is recommended by drinking fluids such that more than two liters of urine is produced per day. If this is not effective enough, thiazide diuretic, citrate or allopurinal may be taken. It is recommended that soft drinks containing phosphoric acid (typically colas) be avoided. When a stone causes no symptoms, watchful waiting is a valid option. For stones which are causing symptoms, pain control is usually the first measure, using medications such as nonsteroidal anti-inflammatory drugs or opioids. More severe cases may require procedures. For example, some stones can be shattered into smaller fragments using extracorporeal shock wave lithotripsy. Some cases require more invasive procedures. Examples of these are cystoscopic procedures such as laser lithotripsy or percutaneous techniques such as percutaneous nephrolithotomy. Sometimes, a tube (ureteral stent) may be placed in the ureter to bypass the obstruction and alleviate the symptoms, as well as to prevent ureteral stricture after ureteroscopic stone removal. Pain and swelling or tenderness will occur in the cancerous area. Leukemia is a form of bone cancer because it can affect the marrow. Bone pain can also result when blood supply has been restricted or disrupted for some reason. A bone that has become infected will cause pain. Bones may begin to hurt when they become brittle because they're not getting enough of the minerals they need. Common Types of Bone Pain Back Pain and Back Injuries A wide range of conditions can affect the spine and cause pain. Are there any risks associated with too much vitamin C? When obtained from food sources and supplements in the recommended dosages, vitamin C is generally regarded as safe. Side effects are rarely reported, but include diarrhea, nausea, abdominal cramps, and other gastrointestinal symptoms. For most healthy individuals, the body can only hold and use about 200-250 mg of vitamin C a day, and any excess is lost though urine. At times of illness, during recovery from injury, or under conditions of increased oxidative stress (including smoking), the body can use greater amounts. High doses of vitamin C (greater than 2,000 mg/day) may contribute to the formation of kidney stones, as well as cause severe diarrhea, nausea, and gastritis. Please be considerate...our patients are NOT pin cushions. They are already in pain so there is NO need to cause even more pain by multiple injections. So what is the issue and what is the purpose of this educational video clip? I have noticed an increasing number of our patients getting medications for their painful condition (I.e. Kidney stones, fractures, migraines, sickle cell disease, chronic pain, etc.) via the intramuscular route (I.e. SHOTS) There is nothing wrong with that, if we consider the implications of what you are ordering. For example...a patient with migraine headache gets an order for 10mg of IM Decadron 4mg of IM Zofran And 4mg of Morphine Pretty good cocktail to treat a headache right? Not for the patient... None of these medications can be mixed together so this patient will end up getting 3 different shots. Which is an OK price to pay if you were guaranteed success at treating the painful condition to the point of relief and discharge from the ER. But what frustrates me most is the under dosing of the narcotic medication. I am not saying we should treat all migraines with narcotic shots, BUT... If you ARE going to treat with narcotics, then you need to give an adequate dose. The dosing of morphine is 1mg/kg so for most of our patients that is 6-8 mg dose. But this is to treat narcotic naive patients, patients with chronic painful conditions (like the migraine in this clinical scenario) will probably needs twice as much medication. Did you ask the patient what works for them? Often they will tell you that it usually takes X mg of Z medication to 'abort' the headache. If that is so, then why give him half the dose of a different medication? Then we wonder why isn't their HA any better? Why does the patient keep begging for more medications? Why are they such a 'drug seeker'? Maybe because we have cornered them into that position by giving sub-therapeutic doses of pain medications. Let me paint you a picture of a patient we had in the ER not too long ago. She was in her late 30's with chief complaint of severe headache, fevers, malaise, and confusion. Diagnostics were ordered, and patient was ordered IV fluids, and a dose of pain medication. 1 mg Dilaudid. HA was not improved, she needed altogether a total of 4 mg of Dilaudid before her HA was controlled to the point where she was acting normally. You heard me right, she was acting normally...not sedated, not drowsy. It would be easy to think that this patient is obviously a narcotic user. Obviously she has tolerance to narcotics. "I've given enough medications to put an elephant down', anyone would think to themselves. Except, this patient is NOT a drug seeker...how do I know this? She is my wife, And she was not just having a HA...she had encephalitis.She was very sick indeed. This experience in treating her headache taught me that patients indeed have different thresholds to pain and to pain medication dosing. And we should be sensitive to this, and treat adequately. So let's go back to the example of the pain shots ordered for our 'migraine' headache. Zofran, Decadron, and Morphine cannot be mixed and given as single shot. So...if this is the cocktail you are to try on this patient you should order it through the IV route instead. This way, if the medication fails you can then use the already established IV to give additional doses of pain medication, fluids, etc. If you are sure that you are only going to give the patient 'shots' and then discharge then consider this instead. You can give the Zofran via oral dissolving tablet at 8mg. Give a good dose of pain medication...either 8-10 mg of morphine or 1-2 mg of Dilaudid. And consider giving the Decadron PO (unless they are actively vomiting) there is really no clinical advantage to give Decadron IM vs PO (there are plenty of literature proving this fact) and IF the patient is vomiting...then you should have started an IV and fluids to begin with.

Measuring Iron Levels To measure the iron level of your body you need to know your hemoglobin level, which is made up of iron and oxygen. For women, the normal hemoglobin level is 12.0 and for men, it is at 13.5. A person's iron level can also be measured through the transferrin saturation (TSAT) and ferritin tests. Normal TSAT level is at least 20 percent, while normal ferritin level is at 100 ng/ml. Doctors recommend the monthly testing of iron levels until the normal hemoglobin level is reached. When the target hemoglobin level is reached testing every three months is recommended. Recommended Treatments for Low Iron Your doctor will make a treatment plan if you have a low iron level. He will more likely recommend increasing your intake of iron-rich foods such as eggs, leafy green vegetables and red meat such as steak and tofu. A dietician can help develop meal plans suitable for your CKD condition. Another option is for your doctor to prescribe iron supplements either in tablet or liquid form, or prescribe a multivitamin and mineral supplement that has folic acid, iron and vitamin B. A third option is for your family physician to inject iron medicine into your vein. First Signs of Pregnancy - Early Symptoms of your First Trimester

Before my first pregnancy, I enjoyed a glass of wine with dinner and an occasional big, juicy chili dog. But even in those very first days after I conceived, the wine tasted flat and the hot dog repulsed me. Fast-forward a few weeks. With a positive pregnancy test in hand, I realized that my body knew I was pregnant before my mind did. Of course, the earliest symptoms of pregnancy wax and wane and are different for each woman; in fact, some women may experience (or notice) none of them. But several can crop up well before you even miss a period. If you’re the impatient type, here are the most common early-pregnancy clues to look for. Clue 1} Breast tenderness Your breasts may be extra tender as early as a week or two after conception. “You’re making so much estrogen and progesterone in early pregnancy that the glands in the breasts start growing,” explains Jasbir Singh, M.D., an OB-GYN at Baylor Medical Center at Waxahachie in Texas. This hormone surge causes breasts to retain more fluids and feel heavy, sore or more sensitive than normal PMS tenderness. What to do Buy a very supportive bra. Clue 2} Implantation bleeding When the fertilized egg implants into the plush lining of the uterus about six to 12 days after conception, spotting—light vaginal bleeding—may occur. This is harmless, but if you suspect you’re pregnant, let your doctor know, just in case it’s something else. (If the bleeding occurs as late as 12 days after you conceive, you may think it’s your period.) “In the first trimester, bleeding should be evaluated for three things in particular—miscarriage, an ectopic pregnancy or certain types of infections,” Singh says. “Implantation bleeding is sort of a diagnosis of exclusion. Clue 4} Frequent urination You might think this comes later, when the baby presses on your bladder, but frequent peeing sometimes starts early. Not only can the swelling uterus put pressure on your bladder, but the extra blood flow to the kidneys (which begins right away) also causes them to produce more urine. What to do Nothing, unless the frequent urination is accompanied by burning, urgency or other signs of infection. (If so, notify your doctor.) Do not cut back on your fluid intake. Clue 5} Food aversions and nausea This can start as early as two weeks after conception. “Progesterone causes a lot of things to slow down,” Singh says. That includes your digestive processes, sometimes resulting in constipation or indigestion. Since your stomach doesn’t empty as quickly as it normally does, it thinks there’s too much going on in there and wants to purge in some way—either into the gut or out through the mouth. Nausea also is related to human chorionic gonadotropin (HCG), a hormone that can be detected in the mother’s blood or urine even before a missed period. The higher the HCG level (as with twins), the sicker you may feel. “Another theory is that nausea and food aversions are a protective effect the body has toward the fetus,” Perkins says. If things like free radicals or nitrates in some foods could hurt a growing baby, you may naturally develop a distaste to steer clear of them. What to do Many women find ginger and lemons soothing. Ask your doctor about special vitamin B6 supplements that may alleviate nausea. Acupressure wristbands also can help. If you’re vomiting frequently or can’t hold down any liquids, see your OB for possible IV hydration and medication. ERYTHEMA MULTIFORME Erythema multiforme is the mildest form of the epidermal necrolysis condition. Erythema Multiforme has a very characteristic target lesion that is very similar to urticarial. It is differentiated from erythema multiforme by the fact that urticarial disappears after 24 hours whereas erythema multiforme doesn’t. Causes and etiology – Herpes, mycoplasma, drugs allergies may imitate erythema multiforme History – Generally it is not preceded by Prodrome such as cough or sore throat, but they may have fever with a history of prior attacks. Examination – normally found on face and extremities. Also exhibits the Koebner phenomenon. Sometimes the central area can blister. Mucocutaneous lesion may also be seen. Course – usually last 2 weeka nd then there is resolution without sequelae, however recurrence is common. Management – hospitalize if there is impaired feeding, may develop to SJS, and if there are constitutional symptoms. EPIDERMAL NECROLYSIS STEVEN JOHNSON SYNDROME (SJS) Defined as involvement of less than 10percent of body surface area. TOXIC EPIDERMYLOSIS NECROLYSIS (TEN) Defined as involvement of more than 30 percent of body surface area. CAUSES – High risk drugs – Sulfonamide, anti-convulsants, allopurinol, NSAIDs, NSAIDs usually taken 8 weeks prior. Also viral infections, immunizations, mycoplasma, GVHD. Clinical – skin peels off for 5-7 days and plateaus for days to week and this is when there is sepsis and failure. Starts with prodromal symptoms such as fever, headaches, rhinitis. Cutaneous begins with face to upper trunk and then to upper limbs. Lesions starts as erythematous macules that then become confluent. These patients are Nikolsky’s sign positive meaning skin will rub off with minor pressure. Mucocuteneous lesions consists of pain on swallowing, eye pain. Extracutaneous symptoms related to pulmonary system (ARDS, Bronchiectasis), Esophageal to colon necrosis causing diarrhea, melena, malabsorption. Kidneys, nails, vulva, vagina involvement may also be present. Prognosis scoring system is known as the SCORTEN. Age greater than 40, heart rate greater than 120, presence of cancer, BSA greater than 10 percent, urea greater than 20mmol, HCO3 greater than 20mmol and glucose greater than 14mmol. If more than 5 than 90 percent mortality. Management – Oxygen and ABG. Check glucose, urea, and HCO3. First begin by stopping the drug which is causing the allergy. Admit to ICU or Burns wards and give fluids and electrolytes. If infection than give anti-biotics. Mouth rinse with anti-septic wash every day.
