To pill or not to pill? That is the question. Hormonal Birth Control Pills Hormonal or chemical birth control pills will be explained in this video, with the pros and cons of each type. But before I demonstrate the different types, I would like to explain to you how hormonal birth control pills work. When a lady is pregnant, ovulation (or egg release) does not occur. That’s how our body’s system normally works. The hormones in a woman’s body are balanced in a certain way that makes the hormones that stimulate the release of the eggs – called the ovulation hormones – to become inactive during pregnancy. So we thought: If we give the woman pregnancy hormones in pills, we will be able to inhibit ovulation, in other words: to stop it from happening, which, in turn, would prevent the pregnancy to occur. So the scientists decided to put the pregnancy hormone and the female hormone in a pill. This gives us an idea about the first type of birth control pills. This type consists of synthet-ic forms of hormones – the pregnancy hormone progesterone (called progestin) and the female hormone oestrogen. On the other hand there’s another type of birth control pills that consists of the pregnancy hormone only, and in a very small amount. It is called the mini Pill or the Progesterone-Only Pill (P.O.P.). The pills I will talk about are not the only hormonal birth control methods. We can take other forms of chemical or birth control at the doctor’s office, which would have the same mechanism of action. So, back to the pills. Let me explain them now. Let’s start with a combined pill. “Combined” because it has a combination of hormones. Let’s know more about it. The combined pill consists of progestin – a synthetic progester-one (the pregnancy hormone) – and oestrogen (the female hormone). You will need to take a pill daily for 21 days, starting from the 7th day of your menstruation. Even if you’re still bleeding, you must start using the pill. Take a pill daily for 21 days and stop the pill for 7 days, taking into consideration that your period cycle is 28 days. The combined pill is a good birth control choice in cases of: • You don’t mind to take hormones for birth control • You usually suffer from pains [I’m not sure about what comes here → 2:40] during menstruating, since it will ease the dysmenorrhea. • If your period is not regular, this will regulate it. • You bleed heavily during the period: it will decrease the amount of bleeding, which in turn might prevent you from becoming anaemic. • If you have acne: this might improve your skin condition. • You’re not a smoker The combined pills are not good if you … don’t wish to take any hormones … are Breastfeeding … are a heart patient … have hypertension or high blood pressure … have risk of thrombosis or atherosclerosis … have had diabetes for more than 20 years … have a liver or gall bladder disease … have a history of breast cancer in the past 5 years … are suffering from recurrent migraines or have the symptoms that proceed mi-graine, called the aura … are overweight … have been taking medications such as antibiotics, epilepsy medications or anti-depressants All these may weaken the effect of the combined pill. Always read the patient in-formation leaflets. This birth control method is 99% effective, on condition that the pill is taken on the same time every day. Signs You Are Pregnant While on Birth Control
What are the signs you are pregnant while on birth control? You hit that row of do nothing pills in the birth control pack and your period does not come. It could come late for other reasons like hormones out of whack. And it was concern that my hormones would be messed up that I did not use them. Then I cannot say that you got pregnant on the pill because you used antibiotics. Antibiotics can make you poop a lot, passing the pills through faster. If you were taking thyroid medication, it would metabolize them faster. However, if you get a stomach bug and throw them up, you could get pregnant. If I’m throwing up a lot, that’s a sign I am pregnant on birth control. If you are nauseous or throwing up in the morning but not in the afternoon or evening, then it is likely due to pregnancy. If it is all day, then you may need antibiotics or another medication. I have not been taking antibiotics. If you were on the pill, you’d be able to guess you were pregnant if you had a light period the next cycle and none the one after that. With a birth control implant, though, a woman would not have a period at all. Well, most of the time. You could develop breast tenderness in early pregnancy. I get that with my normal cycle. Peeing a lot is a sign of pregnancy. Or a urinary tract infection. If you used an injected contraceptive, your odds of pregnancy are really low barring medication that interferes with it. I do not use the injectables. What are the other signs I could be pregnant while on birth control? If you used an intrauterine, it causes heavier than normal periods. If you’re pregnant, the period stops. That’s usually normal regardless of the contraception method. You could be certain you did a pregnancy test and it came back positive. Don’t birth control methods interfere with that? If you used a condom, cervical cap or even an intrauterine, you’d have the normal hormonal cycles for ovulation and pregnancy. It is only the hormone based pills and shots that throw off your hormones. And it may be that my contraceptive method failed. If you were taking birth control pills, the estrogen or progesterone won’t throw off the pregnancy test. In fact, no medicine you take for anything would mess it up. So a pregnancy test is the best way to see if I’m pregnant. And it would be a sure sign to talk to your doctor about the best way to stop your birth control, since you clearly won’t need it for a while.

How to Prevent Nausea from Birth Control Pills Nausea and birth control pills Since the introduction of the first birth control pill in 1960, women have come to rely on the pill as an effective way to prevent pregnancy. More than 25 percent of women who use birth control today are on the pill. The birth control pill is more than 99 percent effective at preventing pregnancy when it’s taken correctly. Like any drug, it can cause side effects. Nausea is one of the most commonly reported side effects of birth control pills. Why does the pill cause nausea? The queasiness is the result of estrogen, which can irritate the stomach. Pills that contain a high dose of estrogen, especially emergency contraceptive pills, are more likely to cause stomach upset than pills that have a lower dose of this hormone. Nausea is more common when you first start taking the pill. How to treat nausea when you’re on the pill There is no specific treatment for nausea caused by the pill. However, you may find relief from mild bouts of nausea with these home remedies: Consume only light, plain foods, such as bread and crackers. Avoid any foods that have strong flavors, are very sweet, or are greasy or fried. Drink cold liquids. Avoid any activity after eating. Drink a cup of ginger tea. Eat smaller, more frequent meals. Take a series of deep, controlled breaths. Applying pressure to certain points on the wrist has also been found to relieve mild nausea. This traditional Chinese remedy is called acupressure. Nausea caused by the pill should resolve within a few days. If the nausea persists, make an appointment to see your doctor. Nausea that doesn’t let up can have an effect on your appetite and weight. You may need to switch to another type of pill or a different form of birth control. How to prevent nausea when you’re on the pill To prevent nausea, don’t take your birth control pill on an empty stomach. Instead, take it after dinner or with a snack before bed. You can also take an antacid medicine about 30 minutes before taking the pill. This may help keep your stomach calm. Before using the emergency birth control pill, speak with your doctor to see if an anti-nausea medication can also be used. They may give you a prescription for an anti-nausea medicine, especially if this pill has made you feel sick in the past. Progestin-only emergency pills are less likely to cause nausea and vomiting than pills containing both estrogen and progestin. Don’t stop taking the birth control pill just because you have nausea. You could get pregnant if you aren’t using another birth control method as a backup. How do birth control pills work? Birth control pills contain man-made forms of the female hormones estrogen and progestin or progestin only. These hormones prevent pregnancy by stopping the release of a mature egg from a woman’s ovaries (ovulation). Birth control pills also thicken mucus around the cervix. This makes it harder for the sperm to swim to the egg and fertilize it. The pill also changes the lining of the uterus. If an egg is fertilized, the altered uterine lining will make it more difficult for the egg to implant and grow. Emergency contraceptive pills such as Plan B contain a higher dose of the hormones found in the regular pill. This high dose of hormones can be hard on your body. Therefore, you should only take emergency contraceptives if you didn’t use contraception during sex or you experienced birth control failure. Examples of birth control failure are a condom that broke or an intrauterine device (IUD) that fell out during sex. Emergency contraceptives can stop ovulation and prevent an egg from leaving the ovary. These pills can also prevent sperm from fertilizing the egg. Other side effects of the birth control pill In addition to nausea, the most common side effects caused by the pill include: breast soreness, tenderness, or enlargement headaches moodiness reduced sex drive spotting in between periods, or irregular periods weight gain or loss Most of these side effects are mild. They usually go away within a few months after you start taking the pill. One rare but serious side effect of birth control use is a blood clot in the leg (deep vein thrombosis), which if untreated can lead to a blood clot in your lungs (pulmonary embolism) and possibly death. This risk is rare. However, your risk is increased if you have used the pill for a long time, you smoke, or you are older 35 years. Choosing a birth control pill that’s right for you When choosing a birth control pill, you need to strike a balance. You want enough estrogen to prevent pregnancy but not so much that it makes you sick to your stomach. Your doctor can help you find a birth control pill that suits your needs. While you’re taking the pill, follow the directions carefully. Take your pill every day. If you skip a dose, you’ll need to take the missed dose as soon as possible.

How Intrauterine Device Prevent Pregnancy? - Manipal Hospital
This video is an informative animated presentation that explains in detail about Intra Uterine Device (IUD), a method of long lasting pregnancy prevention. An Intra Uterine Device or IUD is a birth control method consisting of a soft flexible T-shaped device with a thin string on the end. There are two types of Intrauterine Devices, Hormonal IUDs and Copper IUDs. To prevent pregnancy, a doctor places an IUD inside the uterus, with the string hanging down inside her vagina. Both types of IUD cause mild inflammation of the uterine lining, which releases immune cells and chemicals that kill sperm and thereby prevent fertilisation. Hormonal IUDS contain a synthetic type of progesterone called the Levonorgestrel. The Levonorgestrel mainly helps in preventing pregnancy by thickening the mucus inside the cevix, which prevents sperm from entering the uterus. It also prevents the uterine lining from thickening so it is less likely to receive a fertilized egg. About a month after inserting an IUD, the doctor may schedule an exam to make sure it is still in place. The doctor may also teach the woman, how to check her IUD between visits, by feeling the string inside the vagina. Depending on the type, an IUD can be left in place for three to 10 years. Hormones Involved in Birth Control & Fertility Treatment | Biology for All | FuseSchool
In this video we are going to look at how these hormones can be used in contraception and compare them to non-hormonal methods. We are also going to look at the use of hormones in treating infertility. Female fertility is controlled by hormones, this means that knowledge of these hormones can be used to increase (fertility treatment) or decrease (contraception) the chances of fertilisation and pregnancy. FSH causes the egg to mature in the ovary, so if we can stop the production of FSH happening then we can prevent pregnancy happening. And we already know that oestrogen prevents the production of FSH, so contraceptive pills just need to contain oestrogen. Progesterone also can act as a contraceptive; it makes the mucus in the uterus very thick and so sperm cannot get through to the egg to fertilise it. Pills are either oestrogen only, combined oestrogen and progesterone or nowadays there is a mini-pill which is progesterone only. Early contraceptive pills contained much higher amounts of oestrogen. This caused quite significant side effects, such as weight gain, mood and blood pressure. More modern pills contain much less oestrogen, and some are even just progesterone. Progesterone is a lot more gentle on the body; oestrogen pills make the body think it is in a permanent state of pregnancy. Oral contraceptives are great in many ways. They give people the freedom to choose when to have a child, and when to not have children. But as mentioned above, even the more modern versions can cause side effects in people. There is also an increased risk of developing blood clots. So they are not perfect, and so people do seek out other methods of contraception instead. Inter-uterine devices (IUDs) or coils are another contraceptive option. They must be fitted by a doctor, and have spermicidal properties and prevent implantation. Or there are also contraceptive implants which work by releasing progesterone into the body and last for 3 years. Whereas the contraceptive injections also make use of progesterone, but they only last for between 8 and 13 weeks. Non-hormonal methods of contraception include condoms and other barrier methods which stop the sperm from reaching the ovum. How effective these methods of contraception are depends on whether you use it correctly. The IUDs and implants are great because they do not have human error; you cannot forget to take it or put it on. If taken correctly, the contraceptive pill is over 99% effective. But you need to take it at a similar time each day, and remember to take it daily. Other than condoms and barrier methods, all forms of contraceptives come with differing side effects and girls should consider the different options when deciding which one suits them best. So that is how we use our knowledge of hormones to prevent pregnancy. But what about helping women conceive? Some women don’t produce enough FSH to allow their eggs to mature, and so have difficulty becoming pregnant. Fertility drugs contain FSH and also LH, which stimulate the eggs to mature in the ovary. Remember that LH causes the mature egg to be released from the ovary. Because fertility drugs contain FSH and LH, this can cause multiple eggs to be released and so increases the chances of multiple conceptions; twins or even triplets. Fertility treatments do increase a woman’s chances of becoming pregnant, but it is not 100% assured and there can be other factors to consider. BIRTH CONTROL (CONTRACEPTION) OVERVIEW Birth control is a way for men and women to prevent pregnancy. There are many different methods of birth control, including hormonal contraception such as "the pill." Women take the pill by mouth to prevent pregnancy, and, when taken correctly, it is up to 99.9% effective. However, the pill does not protect against sexually transmitted diseases, including HIV (the virus that causes AIDS). The latex male condom provides the best protection from most STDs. Other types of combined estrogen and progestin hormonal contraception include the patch and the vaginal ring. HOW DOES HORMONAL CONTRACEPTION WORK? A woman becomes pregnant when an egg released from her ovary (the organ that holds her eggs) is fertilized by a man's sperm. The fertilized egg attaches to the inside of a woman's womb (uterus), where it receives nourishment and develops into a baby. Hormones in the woman's body control the release of the egg from the ovary -- called ovulation -- and prepare the body to accept the fertilized egg. Hormonal contraceptives (the pill, the patch, and the vaginal ring) all contain a small amount of man-made estrogen and progestin hormones. These hormones work to inhibit the body's natural cyclical hormones to prevent pregnancy. Pregnancy is prevented by a combination of factors. The hormonal contraceptive usually stops the body from ovulating. Hormonal contraceptives also change the cervical mucus to make it difficult for the sperm to go through the cervix and find an egg. Hormonal contraceptives can also prevent pregnancy by changing the lining of the womb so it's unlikely the fertilized egg will be implanted. Another option for hormonal contraceptives is the extended-cycle pill, such as Seasonale, which was the first one to be approved. Seasonale contains the same hormones as other birth control pills, but the hormones are taken in a longer cycle. Contraception and Birth Control for USMLE Step 2
Simple handwritten Contraceptive (Birth Control) lecture for medical students taking the USMLE. Will be looking at how contraceptives work and the pharmacology. Also the different contraception methods, such as Oral contraceptive, Intrauterine Devices, Implants. Contraception is a method of females to prevent pregnancies. There are several contraceptive methods, however, there is no 100% full proof method. Tier 1 failure rate is less than 1%, Tier 2 have a contraceptive failure rate 2-3% and finally tier 3 is the barrier method and they are about 80% effective. Tier 1 is made up of Intrauterine devices (mirena, Copper), DMPA, Implanon (subcutaenous) and sterilization (tubal ligation or vesectomy). Tier 2 are the Oral Contraceptive Pills, Patches and Rings, and they are made up of comboniation or just progesterone. Tier 3 is the barrier methods include condoms, diaphragm and the spermicidies which prevents implantation. Can also prevent STDs and HPV. Intrauterine devices are implantedin the uterus. They are made up of mirena and copper. Mirena secretes levonogestrel which is a type of progesterone that has three effects. Thickens the cervical mucosa which prevents sperm form ascending into the uterus. The endometrial wall is also thickened that decreases the chance of implantation. Increase tubal motility so it is not in blastocyst and it can't implant into endometrium. Copper Intrauterine device leads to inflammation response which causes the same effects. Mirena can work up to five years, whereas copper can work for 10 years. In Mirena there will be decrease bleeding and cramping. However, Copper can increase bleeding and cramping. Contra-indications to Intrauterine Devices is pregnancy, history of breast cancer, leiomyomas, any uterine infection such as PID and endometritis, especially chlamydia. Any bleeding where the cause is unknown. Relative complications are anatomical uterine anomalies (size and shape), Heart Disease because sometimes perforates and then bleeding can affect the heart. Any history of ectopic pregnancy. Finally there may be complications such as expulsion of the device within 1 month. Perforation, ectopic pregnancy, infection (PID) which usually happens within first 2 months. Sterilization and generally irreversible. Tubal ligation and vesectomy are the two methods. Vesectomies are more effective. TIER 2 are the hormonal methods there are two types which are known as combined and progesterone only. In combined there is pill patch and ring. In Progesterone only there is pill injection and implant. No LH surge means there is no ovulation and there is atrophy of the endometrium and therefore there is no implantation possible. Cervix becomes thick and edematous and prevents sperm from coming in. Combined method is when there is estrogen and progesterone. The pill can begin any day however effects won't begin for 7 days. 21 days and 7 days off. Off days are for the withdrawal bleed. Also helps regulate and normalize the menstrual cycle. The nuvaring enters the vagina and slowly releases estrogen and progesterone. Every three weeks it needs to be removed for 1 week. Patch also slowly release estrogen and progesterone under the skin which gives 60% greater than pills. Progestin only method in pill form is a daily pill that is continuous which causes irregular breakthrough bleeding. Injection form Depo-Medroxyprogesterone Acetate (DMPA) is injected into the skin and slowly released and it lasts for three months. Implanon is implanted subcutaneously and slowly releases etanogestrol and lasts for 3 years.

Emergency contraception commonly known as the morning after pill i'm sure is known to many. What is this emergency contraception? It is a method of contraception that is used by a woman before she misses her periods or thinks that they might definitely be or more probability of having a failure having used to particular method of contraception. For example if there is a rupture in the barrier method of contraception like the condom. This emergency contraception works either by stopping the ovulation or postponing the ovulation or interfering with the normal post-ovulatory changes that are naturally occurs on the lining of the uterus following an ovulation to prepare a bit to receive if the egg is fertilized. This emergency contraceptive pill is best taken 72 hours within 72 hours of unprotected contact or exposure. But the efficacy is maximum if it is taken within 24 hours of the protected contact. But remember an emergency contraceptive pill does not give you a hundred percent protection against pregnancy. The efficacy is only between 90 and 95 percent. So after taking an emergency contraception if you miss your cycle, then please rule out a pregnancy first, it is a always preferable that you take an emergency contraceptive pill after consulting the gynaecologist especially if you are taking any medications on a long term basis for any chronic medical issues or if you have any history of any drug allergy. Also remember that this emergency contraceptive pill cannot be used in a regular basis as a regular contraception. It is not a substitute for a regular contraception and likewise it cannot be taken multiple times within one menstrual cycle as they can cause severe hormonal imbalance resulting in a normal uterine bleeding.

Ovulation is the process when during the menstrual cycle an egg is released from the ovary. When the egg passes through the fallopian tubes is when fertilization may take place. Eggs that are unfertilized may live for up to twelve hours. If the egg is not fertilized it is shed from the body in the menstrual period. You are more likely to become pregnant if intercourse takes place just before or after ovulation. There are a number of changes your body goes through during the menstrual cycle. The rhythm method is based on calendar calculations from previous menstrual cycles. This method is used for women who do not have variations in the length of the cycles. All methods require charting and if you receive training from your physician or an instructor thesis methods have proven to be between 90% and 98% effective when performed correctly. Tracking these calculations can be tricky and difficult with charts and calendars. Wouldnt it be nice if there was a way to calculate the safe period without having to keep up with a calendar or chart of your periods? There are many tools available from apps for your iPhone, downloadable programs, manual diaries and menstrual logs. There is a web site, named Safe period calculator also available that does all the calculations for you. If you are a women with a regular 26 to 30 day cycle this site will calculate for you the safe date. All you have to do is key in the date of when your last period began and the number of days in your cycle. The program gives you the results of the two-week ranges that it is safe to have intercourse and not become pregnant. Conversely you can use this to determine your fertile days if you wish to become pregnant. This is an easy to use tool that quickly will give you the safe days to have intercourse. Nothing is really simpler and if you use the information provided from this calculator along with the other physical changes your protection from this natural family planning method is as effective as most other forms of contraception.

I couldn't wait for it to arrive. It sounds silly, but I really felt like a kid on Christmas Eve. On the first night I woke up many times, so excited I was to take my temperature for the first time. And the excitement continued as I saw the first rise in the temperature when I ovulated and the first drop on the day I got my period. Basically, the app tells you whether you're fertile or not based on your basal temperature. You have to measure it ideally every day, at around the same time, as soon as you wake up. Our hormones fluctuate during the cycle and this makes our basal temperature change. When you insert our daily temperature in the app, it calculates whether you're fertile and tells you whether you need to use protection on that day. It's so brilliant! I've been using it for 3 months and couldn't be happier! However, there are other ways to check on which stage of your cycle you're in, such as the cervical fluid, the cervical position and LH tests. Checking those 3 extra things will virtually guarantee a very precise information about your cycle. This is why I highly recommend reading the book Taking Charge of Your Fertility by Toni Weschler. This book is important for any woman who wants to understand how her body, her cycles and her fertility works. Even though it can be highly scientific at times, the author just makes it as funny as it can be. She turns the process of getting to know your body into something fun and engaging. I would love to know how you control your fertility and prevent pregnancy. Pills, rhythm, condom, spermicide, app, iud... each woman should decide what's best for her. But one thing is true: knowledge about your body is invaluable! When you get to know your cycles, you'll start to listen to your body and profit from its knowledge. You're most fertile when your mucus is clear and slippery, like raw egg white, which lasts about four days. If you don't want to get pregnant, be aware that you can conceive in the three or so days before and after, too, for a total of about 10 potentially fertile days. Tip Assessing mucus viscosity can be tricky; a medical professional can help teach you. Step 3: Take your temperature Try the basal body temperature method: maintain a graph of your daily temperature, taking it every morning before you get out of bed. So, the day you started bleeding is part of your safe period but as you get closer to Day-14, the likelihood of getting pregnant increases. Days around day 14...avoid, but other days are safe and down to your next period next month are safe. The danger period are the days to avoid sexual intercourse which are the days around 14th day. I will repeat it one more time: The first thing is start counting from the first day you started bleeding that would be Day 1. The next thing is avoid Day 14 and days around day 14.
