Furosemide (Lasix) - Loop Diuretics Generic Name: Furosemide Brand Name: Lasix Furosemide also known as Lasix is a loop diuretic commonly use to treat fluid retention and edema. Which can be associated with Congestive Heart Failure (CHF), cirrhosis of the liver, and many kidney disorders. It is also sometimes use to treat hypertension alone or in combination with other anti-hypertensive drugs. Diuretics are medications that increases the amount of water that passes through the kidneys as urine, thus, it is often called water pill. Lasix belongs to the group known as Loop diuretics. There a few types of diuretics and loop diuretics are just one type. It works by interfering with the sodium, potassium and chloride symporter. A symporter is a protein membrane that manages the transport of molecules across a cell membrane. The symporter that lasix interacts with is found in the thick ascending limb of the loop of Henle. What it does is………. inhibits the reabsorption of salt (Na+) and Chloride (Cl-). Since the Salt and Chloride will be leaving the body system through the loop of henle this means that water will also follow…. causing more water pass through the kidney. Which will ultimately mean, less fluid remaining in the blood stream. Once Lasix is received the system will then compensate the loss by absorbing any fluid accumulated in the tissues, such as in the lungs or legs, back to the blood stream. This will cause more fluid to be in the blood stream. Which once again pass through the kidney, which will also be excreted in the urine, since the loop diuretic inhibits its reabsorption. This mechanism, is what makes loop diuretics a treatment for fluid retention also known as edema. It can therefore, ease symptoms of edema such as breathlessness caused by congestion of fluid in the lungs which patients diagnosed with CHF, cirrhosis of the liver, nephrotic syndrome and other edematous states may encounter. Lasix is available in forms that can be taken orally and intravenously. When taken intravenously, it is considered to be twice as strong. Parenteral or intravenous Furosemide is indicated when a fast acting and an intense diuresis is needed such as in acute pulmonary edema and cerebral edema. It also indicated when oral therapy is not possible because of problem with absorption in the intestine or for other reasons. Parenteral administration should be observed only in hospital or outpatient clinics. However, in cases of emergency that furosemide should be given right away outside hospital setting, recommended doses should be closely adhered to and patient must be closely monitored. Contraindication: Furosemide should not be taken if your patient cannot urinate. Severe Hypokalemia - is a major contraindication. This means low potassium. Ascites is the accumulation of fluid (usually serous fluid which is a pale yellow and clear fluid) that accumulates in the abdominal (peritoneal) cavity. The abdominal cavity is located below the chest cavity, separated from it by the diaphragm. Ascitic fluid can have many sources such as liver disease, cancers, congestive heart failure, or kidney failure. When fluid builds up inside the abdomen, It is dangerous disease What are the types of ascites? Traditionally, ascites is divided into 2 types; transudative or exudative. This classification is based on the amount of protein found in the fluid. A more useful system has been developed based on the amount of albumin in the ascitic fluid compared to the serum albumin (albumin measured in the blood). This is called the Serum Ascites Albumin Gradient or SAAG. Signs and symptoms Mild ascites is hard to notice, but severe ascites leads to abdominal distension. Patients with ascites generally will complain of progressive abdominal heaviness and pressure as well as shortness of breath due to mechanical impingement on the diaphragm. symptoms may develop gradually or suddenly, depending on the cause. When only a small amount of fluid is present in the abdomen, ascites may be undetectable. Abdominal pain and bloating increase with the amount of fluid collected in the body. Ascites is detected on physical examination of the abdomen by visible bulging of the flanks in the reclining patient ("flank bulging"), "shifting dullness" (difference in percussion note in the flanks that shifts when the patient is turned on the side) or in massive ascites with a "fluid thrill" or "fluid wave" (tapping or pushing on one side will generate a wave-like effect through the fluid that can be felt in the opposite side of the abdomen). Other signs of ascites . include: Ankle or leg swelling (edema) Changes to the navel Decreased appetite Fatigue For instance, in portal hypertension (perhaps due to cirrhosis or fibrosis of the liver) patients may also complain of leg swelling, bruising or mental changes due to encephalopathy. Those with ascites due to cancer may complain of chronic fatigue or weight loss. Those with ascites due to heart failure may also complain of shortness of breath as well as wheezing and exercise intolerance Treatments Since ascites prevents the flushing of salt through your pee, the first step is to cut down on the salt you eat, by a lot. Your doctor also may prescribe “water pills,” also called diuretics, to help flush the extra fluid from your body. Two of the most common diuretics are: • Furosemide • Spironolactone  Reduce the amount of fluids consumed you throughout the day  If change in diet and prescription diuretics aren’t effective, or your symptoms are severe, your doctor may have to use paracentesis to remove large amounts of excess fluid through a needle inserted into your abdomen. This procedure must be combined with a low-salt, low-liquid diet, otherwise the fluid will simply come back. ****Update: RIP Tinkerbell, July 12, 2005 - July 14, 2016*** In this video I discuss the news that was given to me and a mutual agreement with my Vet to not allow this disease get any worse. Over a period of time, Tinkerbell has had episodes of passing out with no heartbeat and all liquid inside her comes out. Saying goodbye is the hardest thing I have ever had to do. Type of medicine Aldosterone antagonist diuretic, potassium-sparing diuretic Used for Fluid retention Heart failure Primary hyperaldosteronism Also called Aldactone®, Lasilactone® (spironolactone with furosemide), Aldactide® (spironolactone with hydroflumethiazide) Available as Tablets Spironolactone is used to treat fluid retention (oedema) caused by liver disease, kidney problems or heart failure. Oedema occurs when fluid leaks out of your blood vessels, causing swelling in the tissues of your lungs, feet or ankles. This makes you feel breathless or your legs feel puffy. Spironolactone prevents a build-up of fluid in your body by increasing the amount of urine your kidneys produce. It is also used to treat some other conditions which cause fluid retention, such as a disorder called primary hyperaldosteronism. Spironolactone is known as a potassium-sparing diuretic. Unlike some other diuretics, it does not cause your body to lose potassium. Diuretics are sometimes referred to as 'water tablets'. Spironolactone is often used alongside other diuretics. When it is used like this, it may be prescribed as a combination product, such as in Lasilactone® (spironolactone with furosemide) and Aldactide® (spironolactone with hydroflumethiazide). Combinations like these help to reduce the number of tablets you need to take each day. Before taking spironolactone Some medicines are not suitable for people with certain conditions, and sometimes a medicine may only be used if extra care is taken. For these reasons, before you start taking spironolactone it is important that your doctor or pharmacist knows: If you are pregnant, trying for a baby or breast-feeding. If you have kidney problems. If you have a problem with your adrenal glands, called Addison's disease. If you have a rare inherited blood disorder, called porphyria. If you are taking any other medicines. This includes any medicines you are taking which are available to buy without a prescription, as well as herbal and complementary medicines. If you have ever had an allergic reaction to a medicine. How to take spironolactone Before you start this treatment, read the manufacturer's printed information leaflet from inside your pack. The leaflet will give you more information about spironolactone and a full list of side-effects which you may experience from taking it. Take spironolactone exactly as your doctor has told you. The usual dose varies from 25 mg to 200 mg. Depending upon which dose is right for you, you may be asked to take several tablets a day but these are generally taken at the same time. Take the tablets with or just after a meal. Spironolactone is commonly prescribed just once daily and you can generally take the dose at a time to suit you. However, diuretics are best taken no later than mid-afternoon. This is because you will find you may need to go to the toilet a couple of times after taking them and this could disturb your sleep if you take it late in the day. If you have been prescribed more than one tablet of spironolactone a day and told to take them at different times, make sure you take your last tablet no later than 6 pm. If you forget to take a dose, take it as soon as you remember. However, if it is after 6 pm in the evening, skip the missed dose and continue as usual the next day. Do not take two doses at the same time to make up for a missed dose. Getting the most from your treatment If you are having an operation or dental treatment, tell the person carrying out the treatment which medicines you are taking. Can spironolactone cause problems? Along with their useful effects, most medicines can cause unwanted side-effects although not everyone experiences them. These usually improve as your body adjusts to the new medicine, but speak with your doctor or pharmacist if any of the following side-effects continue or become troublesome. Spironolactone side-effects What can I do if I experience this? Feeling sick or being sick (vomiting) Stick to simple meals - avoid rich or spicy foods Feeling tired, dizzy, or sleepy If this happens, do not drive and do not use tools or machines until you feel better Sexual problems, breast discomfort and enlargement, feeling confused, irregular periods, confusion, sweating, cramps, hair loss or unwanted hair growth, and skin rash If any of these become troublesome, speak with your doctor If you experience any other symptoms which you think may be due to this medicine, speak with your doctor or pharmacist.

She has experienced the following symptoms of CONGESTIVE HEART FAILURE few months back in May 2017 as shown in the video above such as: -Breathing difficulty (0:00 to 0:23) -Excessive coughing (0:24 to 0:53) -Swollen belly caused by water retention -Panting -Bluish grey gum -Almost collapsed due to poor oxygen flow -Sleeping difficulty That's when I brought her to the vet clinic immediately to realise that she's diagnosed with congestive heart failure. The medication that my dog has taken is as follows: -Furosemide -Enlapril -Vetmedin Initially, she was prescribed with only furosemide and enlapril. However, the vet has recommended to reduce the dosage of furosemide after some time as its function is to eliminate water and salt from the body so there's a risk of kidney problem in the long term. My dog didn't respond well as her condition was quite serious so the vet introduced Vetmedin and no more furosemide to be taken. She's been taking 2.5mg of Vetmedin per day for a few months time and she's doing really fine now.
