However, Rarely do I come across a patient who truly over produces HCL. Most of the time their Acid reflux or Gastroesophageal reflux is caused by something else: Diet, weight, overeating, or LOW production of hydrochloric acid. Yes, Acid reflux can be caused by LOW PRODUCTION of HCl, you didn't read that wrong. Definitely pantoprazole is one of the drug of choice in management of heart burn or oesophagitis. But how long will you take that drug? It is advices to take the drug for a week, 10 days, 15 days or a month’s time. Taking pantoprazole beyond one month or two months will definitely have its own adverse effects. Any drug if it is taken for a long time it has its own disadvantages along with the advantages the drug can give. I can advice the patient not to drink and smoke when you have oesophagitis and parallely have a decent meal so that the heart burn is significantly reduced even without medications. Pantoprazole is a proton pump inhibitor. The gastric mucosa is designed in such a way that it will secrete hydrochloric acid. It is normally secreted in every other person. But in such patients we induce the proton pump to be inhibited so that the hydrochloric acid secreted should reduce in quantity so that the acidity in stomach comes down. Even if there is some amount of reflux the acids which is there will not burn the oesophagus. This is the concept of usage of pantoprazole. But if it is used for a longer time the proton pumps will gradually understand that this person may not require this much amount of hydrochloric acid and may be atrophic gastritis may set in. It should be taken under the supervision or advised as per the doctor or the surgeon.

Hyoscyamine or Anaspaz, Levsin, Symax, Levbid Information (dosing, side effects, patient counseling)

Hyoscyamine comes in an oral elixir, oral solution, tablet, dispersible tablet, extended-release tablet, and sublingual tablet. All forms are available in multiple strengths. Hyoscyamine is an anti-cholinergic most commonly used to slow the GI tract, treat muscle spasms in the GI tract, gallbladder or urinary system, and infant belly pain. No matter what your doctor has you taking it for, hyoscyamine is taken by mouth 1 to 4 times a day 30 to 60 minutes before a meal. Do not crush, break or chew extended release tablets, swallow whole. Place dispersible tablets on the tongue, let met, and then swallow. Place sublingual tablets under the tongue, let melt, then swallow. Do not take antacids at the same time you take hyoscyamine. If you miss a dose, take the missed dose as soon as you think about it. If it is close to time for your next dose, skip the missed dose and resume your normal dosing schedule. Do not take 2 doses at the same time or extra doses. Some common side effects with hyoscyamine include flushing, dizziness, drowsiness, confusion and excitement most commonly in elderly. Some less common side effects include headache, diarrhea, nausea, vomiting and increased heart rate. (Flash on screen this is not a complete list of side effects) While taking hyoscyamine, remember it is important to tell your doctor or pharmacist if you have: • Signs of an allergic reaction (rash, itching, swelling, tightness of the chest, etc.) • An allergy to hyoscyamine, nitroimidazole derivatives, or any component of the formulation. • Any of these health problems: Bowel block, bleeding problems, enlarged colon, glaucoma, heart problems, myasthenia gravis, slow-moving GI tract, a swallowing tube (esophagus) that is not normal, ulcerative colitis, or trouble passing urine. All forms of hyoscyamine need to be stored at room temperature in a dry place. Use the measuring device received from the pharmacist or doctor for the oral liquid. Proton pump inhibitors (PPIs) such as Prilosec, Nexium, Protonix, and Aciphex have been available for about 20 years and each year about 15 million people use them and bring in $10 billion every year. PPIs are vastly overprescribed by MDs and because they are available over the counter, are overused by patients. The major known side effects of PPIs include poor digestion, poor absorption of B12, iron, calcium, and magnesium which lead to an increased risk for osteoporosis, senile dementia, C difficile infections, pneumonia and now according to articles in the January 11, 2016 issue of the journal Internal Medicine an increased risk for having chronic kidney disease and, as documented in the June of 2015 issue of the journal PLoS One, heart attacks. GERD, gastroesophageal reflux disease or heartburn could develop due to drinking water/juice or soft drink during, right before or after the meal! there are 2 very important physiology rules you need to know in order to understand why you can not drink with your meal! 1. pylorus-lower valve, opens when pH of 2 is reached in the stomach and duodenum has pH of 7! when you drink water you dilute HCL (hydrochloric acid) of the stomach! 2. both valves: pylorus and LES (Lower Esophageal Sphincter) will become "tired" after about 1 hr 30 min! Meaning: they will relax and let food and stomach content out. if you drink more then 2 oz of any liquid with your meal, then you are diluting HCL of the stomach , as a result food will stay in the stomach longer then 1hr 30 min, as a result LES will get open and food and HCL will go up into esophagus! This is your GERD!!!
