The leaky or hyperpermeable intestinal lining places a major burden on the body's ability to absorb amino acids, essential fatty acids, minerals and vitamins. It would appear that nutrients could simply slip right on into the gut. Sadly the opposite is true. For in order for the body to absorb a mineral, a carrier protein must be attached. This protein must hook onto the mineral that actively carries it across the gut wall into the bloodstream. But when the bowel lining is damaged through inflammation the nutrient carrier proteins get damaged. In addition, the finger-like projections that line the gut and allow us to absorb food gets damaged. When these get destroyed, the result is malabsorption. So in addition to new food and chemical allergies and auto-immune diseases, the leaky gut victim may develop mineral and vitamin deficiencies, even in spite of taking adequate levels of them. Rheumatoid Arthritis involves the synovium of the joints. The condition of rheumatoid arthritis will result in deformities. Rheumatoid Arthritis occurs in females more than males. There may be a hereditary component with Rheumatoid Arthritis. Rheumatoid Arthritis has spontaneous remissions and exacerbations. The disease can have a systemic nature. Pain and stiffness of joints especially in the morning (morning stiffness). Rheumatoid Arthritis is typically polyarticular, bilateral and symmetrical and most commonly affects the hands and feet. x-rays show periarticular erosions at the time of diagnosis. Osteopenia and minimal osteophyte formation favor the diagnosis of rheumatoid arthritis. Pathogenesis Rheumatoid Arthritis is an autoimmune disease. The disease has two components: immunological reaction. Increased degradative enzymes. The IgM (Rheumatoid factor) is produced by the plasma cell as ana antibody to the native IgG, which is altered in RA. 70% of patient with RA have rheumatoid factor positive. Leucocytes are attracted to the immune complex forming deposits over the inflammatory surface of the synovium. These leucocytes ingest fibrin and immune complex and is called the Rheumatoid cells. The leucocytes release lysosomal enzymes that cause acute inflammatory response and tissue necrosis as well as inflammatory mediators ( IL-1, IL-6, TNF alpha). The chondrocytes respond to stimulation by TNF- alpha, IL-1 and other inflammatory mediators causing cells to become activated and secrete more metalloproteinases which lead to cartilage damage. The synovium becomes hypertrophied (pannus), showing intimal hyperplasia and infiltration by plasma cells and lymphocytes. Stages of Rheumatoid Arthritis Early: acute: hot swollen tender joints (synovitis) •MCP swelling •Wrist swelling •Flexor sheath synovitis Complicated: digital vasculitis, ecchymosis, skin atrophy, nodules. Advanced •Swelling of MCP joints •Lateral slippage of extensor tendon ruptures •Ulnar deviation of fingers •Xrays show destruction of MCP with subluxation, ulnar deviation and wrist destruction. •Finger deformities: Mallet, boutonniere, Swan neck The thumb is also involved. These changes occur due to proliferation, inflammation and hypertrophy of the synovium. Involvement of the distal radioulnar joint is usually associated with rupture of the extensor digiti minimi. Rheumatoid nodules 25% of patients with RA will have subcutaneous nodules on extensor surfaces of elbow and forearm. Nodules are often multiple and seen along the ulnar margin of the forearm or pulp of the digits. Vasculitis is more common in patients with SC nodules, it is strongly seropositive disease (aggressive) with less favorable prognosis. Treatment •Synovitis: splint and medical treatment •Joint space narrowing, bone erosions and osteopenia: synovectomy. •Joint destruction/fixed deformity/ loss of hand function: surgery based on condition Before operating on RA patients, x-ray of the cervical spine is needed because the patient may have subluxation of C1-C2. Metacarpophalangeal joint arthroplasty of the fingers usually results in decreased extensor lag and improvement of the ulnar drift. Scientists have linked nine genes to osteoarthritis, opening the door to new treatment possibilities.Osteoarthritis is the most common form of arthritis, which cripples more than one in four adults worldwide, and, while medications can help to alleviate symptoms, there is no cure for the degenerative joint disease.Five of the genes identified by the researchers from the University of Sheffield and the Wellcome Sanger Institute had distinctly different expressions in people who had arthritis and those who did not.The discovery of these genes could offer new, more precise targets for arthritis treatments, the study suggests.Arthritis of some form causes disability in more Americans than any other disease or illness.The disease occurs when cartilage that provides cushion between bones wears down. Eventually, this wearing down can even reach the point that bones grind together, making movement excruciating.Osteoarthritis is more commonly diagnosed among those over 65, but nearly two-thirds of people with arthritis are working-age, meaning that the days when they are in too much pain to work cost the economy as well, according to the Arthritis Foundation.There is not much that can be done for these patients.Doctors can treat the inflammation, pain and swelling caused by the action between their joints with drugs like Tylenol, ibuprofen or the antidepressant Cymbalta, which has been approved for treating arthritis pain as well.Strengthening the muscles connecting the affected bones with physical therapy or light exercise, like yoga, may also help alleviate symptoms, but that won't stop osteoarthritis from striking the knees of half of all adults.Often, this persistent pain results in a knee replacement surgery (or hip, for those who develop the disease in these joints), which is costly and puts patients – who are often older and already at elevated risks for certain conditions – in danger of surgery-related heart attacks strokes and blood clots.'Treatments target the main symptom [of joint pain] and culminate in joint replacement, so it is very important that identify targets for development of new therapy,' said Dr Eleftheria Zeggini, one of the new study's lead authors.Finding genetic targets for treatments could offer hope for an alternative treatment, but the 18 previously identified genetic associations for osteoarthritis were not expressed in ways that would make them viable targets.After analyzing genetic data on nearly 328,000 people in the UK, Dr Zeggini and her team identified nine previously unknown locations in DNA whose expressions differed in people with and without osteoarthritis.Then, to determine whether or not these genes might actually cause osteoarthritis, they tested two tissue samples from people who had had knee replacements: one of relatively healthy tissue, and one of the arthritic joint tissue.'By looking at the differences in genes in the primary tissue, we were able to pinpoint what those coded genes are most likely to be,' AutoNews- Source: http://www.dailymail.co.uk/health/article-5518983/Major-study-uncovers-9-new-genes-responsible-osteoarthritis.html?ITO=1490&ns_mchannel=rss&ns_campaign=1490

The study, led by Dr. Qing-Jun Meng, who is a Senior Research Fellow for Arthritis Research UK, could in the years to come, pave the way for drug treatment of the joint. His research findings were published in the Journal of Clinical Investigation. Dr. Meng discovered that body clocks within cartilage cells - or chondrocytes- control thousands repair function insufficient, which could contribute to osteoarthritis. Dr. Meng's of genes which segregate different biological activities at different times of the day. The body clock, he realized, controls the equilibrium between when chondrocyte cells are repaired during rest and when they are worn down through activity. But his research revealed that as we age, our cartilage cell body clocks deteriorate, making the team examined three types of human cartilage under the microscope: normal, mildly affected by osteoarthritis and severely affected. As the osteoarthritis became more severe, the number of cells that express BMAL1 - a protein which controls the body clock - became less and less. Osteoarthritis Corrective Exercise Strategy for the Knee

http://www.fitnesseducationseminars.com/icei/ http://www.iihfe.com If you work with the older population like we do in our clinic, than you recognize that one of the most common issues our clients present with is osteoarthritis of the knees and hips. Many of these individuals will have had and/or are going to require surgery. Did you know that there are currently over 7 million Americans living with knee and hip replacements? And this is not counting the millions upon millions of Americans having orthopedic surgeries of the knees and hips each year. Even more disturbing is that the average age of individuals having hip and/or knee replacement surgery is around 51 years of age. I am quickly approaching the BIG 5-0 and that number nearly made me choke when I first heard it. The unspoken issue with these statistics is that early in my practice, it used to be that the individuals having knee or hip surgery and joint replacements were generally older (60+ years of age), overweight, and inactive. However, the landscape of individuals having knee and hip orthopedic surgeries and/or replacements has completely changed. Now I’m treating young individuals that are already demonstrating degenerative changes in their knees and hips. For example, I am currently treating a 35-year old active male who will likely need a hip replacement secondary to osteoarthritic changes. I have another active, healthy male runner (50 years of age) who has already had one knee replacement and will likely require a second knee replacement secondary to degenerative changes. I have been treating increasing numbers of active teen girls (some as young as 13 and 14) requiring knee and hip surgeries for non-contact sports-related injuries. These girls are already demonstrating osteoarthritic changes (bone spurs and decreased cartilage) in their knees and hips. Please note, I am not suggesting all surgery can be avoided because some individuals will absolutely require surgery to give them back their quality of life. However, what I am illustrating is that as individuals become more active and ‘fit’, there is an increasingly alarming tendency towards excessive and early wear and tear upon their joints. Despite what MD’s and orthopedic surgeons may tell us, being active and fit should not correspond to advanced wear and tear upon our joints. When the activities an individual is participating in are performed with proper form – in other words optimal alignment and control – and are progressed in a safe and appropriate manner, there should be normal amounts of wear and tear upon the individual’s joints. This means that barring significant trauma or hereditary joint conditions such as rheumatoid or psoriatic arthritis and/or hip dysplasia, an individual’s knee and hip joints should remain healthy easily into their 70’s and 80’s. So why do so many of our clients and even my young patients experience osteoarthritis or degenerative joint disease of the knees and hips? It really comes down to one thing… It is their posture and movement habits. You see, when an individual performs activities with non-optimal alignment and control and then performs them at increasingly higher levels and/or increased frequency, they will advance the degenerative changes of their joints. We are seeing more young individuals competing at earlier ages, all year round in the same sport or activity, and participating in organized strength training and conditioning programs, not resting enough, and oftentimes without the proper nutritional support to build and repair cartilage. And many of these individuals are performing these activities with non-optimal alignment and control which when taken in combination with the above listed factors, is a recipe for disaster for their joints. So, is there anything we can do about this issue because after all this condition is frequently associated with heredity or genetics? I love the following statement attributed to Dr. Francis Collins: “All illnesses have some hereditary contribution. It’s been said that genetics loads the gun and environment pulls the trigger.” The great news is that as a health and fitness professional, you are in the best position to help individuals experiencing knee and hip osteoarthritis change their environment!!! Multiple approaches are often use in combination. Do not use human medications due to the risk of toxicity or poisoning associated with products such as Tylenol.
