What is Metastatic Cancer? Let’s learn about Metastatic cancer. When cancer cells break away from the primary spot where they were formed and spread to other parts through lymph system or blood, it is known as metastatic cancer. The cancer cells form tumours in other parts of the body, which are known as metastatic tumours. However, both the primary and metastatic cancer are same in nature. Often, a metastasis occurs in the stage four of many forms of cancer, says cancer.gov. Metastasis is a serious stage as it means that the cancer was strong enough to spread to other parts of the body. As metastatic cancer is similar to the primary form of cancer, so, for example, if breast cancer spreads to lungs, it will be called metastatic breast cancer, not lung cancer. The treatment for the metastatic cancer would be like that of stage IV breast cancer. Because the cancers may be similar in nature, doctors can often fail to locate the primary spot of cancer. The liver, lungs, lymph nodes, and bones are common areas of spread or metastasis. Even when cancer spreads to a new location, it is still named after the area of the body where it started. For example, a person with breast cancer that has spread to the bones is said to have breast cancer with bone metastases. If a cancer has spread widely throughout the body before it is discovered and it is unknown exactly where it started, it is called cancer of unknown primary origin. Treatment for metastatic cancer aims to slow the growth or spread of the cancer. Treatment depends on the type of cancer, where it started, the size and location of the metastasis, and other factors. Typically, metastatic cancer requires systemic therapy, or medications given by mouth or injected into the bloodstream to reach cancer cells throughout the body, such as chemotherapy or hormone therapy. Other treatments may include biological therapy, radiation therapy, surgery, or a combination of these. Even if the cancer has stopped responding to treatment, many therapies can ease side effects and improve quality of life. Adrenocortical carcinoma is a rare tumor, with incidence of 1–2 per million population annually. Adrenocortical carcinoma has a bimodal distribution by age, with cases clustering in children under 5, and in adults 30–40 years old. Adrenocortical carcinoma is remarkable for the many hormonal syndromes which can occur in patients with steroid hormone-producing ("functional") tumors, including Cushing's syndrome, Conn syndrome, virilization, and feminization. Adrenocortical carcinoma has often invaded nearby tissues or metastasized to distant organs at the time of diagnosis, and the overall 5-year survival rate is only 20–35%. The widely used angiotensin-II-responsive steroid-producing cell line H295R was originally isolated from a tumor diagnosed as adrenocortical carcinoma. Adrenocortical carcinoma may present differently in children and adults. Most tumors in children are functional, and virilization is by far the most common presenting symptom, followed by Cushing's syndrome and precocious puberty. Among adults presenting with hormonal syndromes, Cushing's syndrome alone is most common, followed by mixed Cushing's and virilization (glucocorticoid and androgen overproduction). Feminization and Conn syndrome (mineralocorticoid excess) occur in less than 10% of cases. Rarely, pheochromocytoma-like hypersecretion of catecholamines has been reported in adrenocortical cancers. Non-functional tumors (about 40%, authorities vary) usually present with abdominal or flank pain, varicocele and renal vein thrombosis or they may be asymptomatic and detected incidentally. All patients with suspected adrenocortical carcinoma should be carefully evaluated for signs and symptoms of hormonal syndromes. For Cushing's syndrome (glucocorticoid excess) these include weight gain, muscle wasting, purple lines on the abdomen, a fatty "buffalo hump" on the neck, a "moonlike" face, and thinning, fragile skin. Virilism (androgen excess) is most obvious in women, and may produce excess facial and body hair, acne, enlargement of the clitoris, deepening of the voice, coarsening of facial features, cessation of menstruation. Conn syndrome (mineralcorticoid excess) is marked by high blood pressure which can result in headache and hypokalemia (low serum potassium, which can in turn produce muscle weakness, confusion, and palpitations) low plasma renin activity, and high serum aldosterone. Feminization (estrogen excess) is most readily noted in men, and includes breast enlargement, decreased libido and impotence. The main etiologic factor of adrenocortical cancer is unknown. Families with Li–Fraumeni syndrome have increased risk. The p53, retinoblastoma protein (RB) tumor suppressor genes located on chromosomes 17p, 13q respectively, may be changed. The genes h19, insulin-like growth factor II (IGF-II), p57kip2 are important for fetal growth and development. They are located on chromosome 11p. Expression of the h19 gene is markedly reduced in both nonfunctioning and functioning adrenal cortical carcinomas, especially in tumors producing cortisol and aldosterone. There is also a loss of activity of the p57kip2 gene product in virilizing adenomas and adrenal cortical carcinomas. In contrast, IGF-II gene expression has been shown to be high in adrenal cortical carcinomas. Finally, c-myc gene expression is relatively high in neoplasms, and it is often linked to poor prognosis. Hormonal syndromes should be confirmed with laboratory testing. Laboratory findings in Cushing syndrome include increased serum glucose (blood sugar) and increased urine cortisol. Adrenal virilism is confirmed by the finding of an excess of serum androstenedione and dehydroepiandrosterone. Findings in Conn syndrome include low serum potassium, low plasma renin activity, and high serum aldosterone. Feminization is confirmed with the finding of excess serum estrogen.....

A brain tumor occurs when abnormal cells form within the brain.There are two main types of tumors: malignant or cancerous tumors and benign tumors. Cancerous tumors can be divided into primary tumors that start within the brain, and secondary tumors that have spread from somewhere else, known as brain metastasis tumors. All types of brain tumors may produce symptoms that vary depending on the part of the brain involved.[2] These symptoms may include headaches, seizures, problem with vision, vomiting, and mental changes.The headache is classically worse in the morning and goes away with vomiting.More specific problems may include difficulty in walking, speaking, and with sensation. As the disease progresses unconsciousness may occur. The cause of most brain tumors is unknown. Uncommon risk factors include inherited neurofibromatosis, exposure to vinyl chloride, Epstein–Barr virus, and ionizing radiation.The evidence for mobile phones is not clear. The most common types of primary tumors in adults are meningiomas (usually benign), and astrocytomas such as glioblastomas. In children, the most common type is a malignant medulloblastoma. Diagnosis is usually by medical examination along with computed tomography or magnetic resonance imaging.This is then often confirmed by a biopsy. Based on the findings, the tumors are divided into different grades of severity. Treatment may include some combination of surgery, radiation therapy, and chemotherapy. Anticonvulsant medication may be needed if seizures occur. Dexamethasone and furosemide may be used to decrease swelling around the tumor. Some tumors grow gradually, requiring only monitoring and possibly needing no further intervention. Treatments that use a person's immune system are being studied. Outcome varies considerably depending on the type of tumor and how far it has spread at diagnosis. Glioblastomas usually have poor outcomes while meningiomas usually have good outcomes. The average five-year survival rate for brain cancer in the United States is 33%. Secondary or metastatic brain tumors are more common than primary brain tumors, with about half of metastases coming from lung cancer. Primary brain tumors occur in around 250,000 people a year globally, making up less than 2% of cancers. In children younger than 15, brain tumors are second only to acute lymphoblastic leukemia as the most common form of cancer.

Head and neck cancers and cancers of the throat used to be diseases seen only in elderly patients but are on the rise in younger and healthier patients. There has been a rise in the number of these cancers, particularly those caused by particular forms of the human papilloma virus which, when found early, are highly treatable. Advances in treatment have focused on preserving function and quality of life. The treatments, however, often require a multi-disciplinary approach using a combination of modalities. Dr. Abie Mendelson will discuss the many advances in robotic surgery. Dr. Allen Chen will talk about radiation treatment and the importance of preserving function. Dr. Deborah Jean Lee Wong will explore the chemotherapeutic agents that help sensitize the tumors to radiation treatments. These treatments, even in their best form, take a toll on the physical well-being of the patient and often require considerable symptom management. And Carolyn Katzin, an Integrative Specialist with specialized training in nutrition, addresses how to manage nausea, swallowing, eating and maintenance of body weight. The information presented will assist patients and families in making decisions, understanding treatments and preserving well-being into survivorship.

In medicine, lung cancer staging is the assessment of the extent to which a lung cancer has spread from its original source. As with most cancers, staging is an important determinant of treatment and prognosis. In general, more advanced stages of cancer are less amenable to treatment and have a worse prognosis. The initial evaluation of non-small cell lung cancer staging uses the TNM classification. This is based on the size of the primary tumor, lymph node involvement, and distant metastasis. After this, using the TNM descriptors, a group is assigned, ranging from occult cancer, through stage 0, IA, IB, IIA, IIB, IIIA, IIIB to IV. Systemic lymphadenopathy, eg glandular fever, HIV or lymphoma. Non-Hodgkin's lymphoma or Hodgkin's disease commonly presents in this way and may be the only symptom. Cervical TB: scrofula is now rare in this country but may be seen in recent immigrants. Cervical nodes become enlarged and matted together. It is a leading cause of cancer death in men and women in the United States. Cigarette smoking causes most lung cancers. The more cigarettes you smoke per day and the earlier you started smoking, the greater your risk of lung cancer. High levels of pollution, radiation and asbestos exposure may also increase risk. There are many types of lung cancer. Each type of lung cancer grows and spreads in different ways and is treated differently. Treatment also depends on the stage, or how advanced it is. Treatment may include chemotherapy, radiation and surgery. Lung cancer that originates in the cells of the lungs is called primary lung cancer; however, cancer may also spread to the lung from other parts of the body. Metastatic cancers spread to the lungs most commonly from the breast, colon, prostate, kidney, thyroid gland, stomach, cervix, rectum, testis, bone, and skin (melanoma). More than 90% of primary lung cancers start in the bronchi such lung cancer is called bronchogenic carcinoma. The specific types of lung cancer are small cell carcinoma, squamous cell carcinoma, large cell carcinoma, and adenocarcinoma. The last three types of lung cancer are often referred to as nonsmall cell lung cancers. Alveolar cell carcinoma originates in the small air sacs of the lung (alveoli). Although alveolar cell carcinoma can occur at a single site, it often develops simultaneously in more than one area of the lung. Less common lung tumors are bronchial carcinoid (which may be cancerous or noncancerous), chondromatous hamartoma and sarcoma. Lymphoma is a cancer of the lymphatic system; it may start in the lungs or spread to them. Lung cancer is the rapid growth of abnormal cells in the lung. It can start anywhere in the lungs and affect any part of the respiratory system . When we breathe in, the lungs take in oxygen, which our cells need to live and carry out their normal functions. When we breathe out, the lungs get rid of carbon dioxide, which is a waste product of the body's cells. Cancers that begin in the lungs are divided into two major types, non-small cell lung cancer and small cell lung cancer, depending on how the cells look under a microscope. Causes of Lung Cancer 1. Symptoms of vaginal cancer are abnormal vaginal bleeding, lump in the vagina, pain during urination, watery discharge, constipation and pelvic pain. • Heart Cancer Heart cancer is very rare because it can either come out as primary heart tumor or secondary heart tumor. Malignant heart cancers come in different types such as leiomyosarcomas, liposarcomas and angiosarcomas to name a few. Heart cancer can lead to death in just two years of diagnosis. They are fatal cancers because it can block the blood flow in the heart and quickly infiltrates the myocardium causing death. • Appendix Cancer We may have all heard of appendicitis but rarely appendix cancer. A malignant type of appendix cancer called mucinous cystadenocarcinoma affects the wall of the appendix or the epithelial cells of the mucous in the peritoneum. • Male Breast Cancer Breast cancer could be something we may hear often but this becomes common because it happens mostly to women but breast cancer among males makes it an uncommon type of cancer. This type of cancer spreads in the lymphatic system and blood stream in males similar to what happens to female breast cancer. Because males have smaller breast size as opposed to females, detecting it makes it easier. But most cases of male breast cancer are found in their 3rd to 4th stage. Don't Ignore These Early Symptoms of Breast Cancer

Anyone can develop this breast cancer, and it is still not clear what causes it. However, certain factors put you at a higher risk. Women who get annual mammography screenings are much less likely to die from breast cancer than those who do not. Although it is more common among women, men can also get breast cancer. Here are some important warning signs of breast cancer. 01:02 Lump in the Breast or Armpit. 02:05 Red or Swollen Breasts. 03:09 Nipple Discharge. 04:07 Pain in the Breast or Chest Area. 05:06 Changes in Breast Appearance. 06:03 Nipples Become Sensitive. 07:02 Itching on the Breasts. 08:01 Pain in the Upper Back, Shoulder and Neck. Feeling a lump in your breast or armpit may be the first warning sign of breast cancer. Such lumps are often hard and painless, though some may be painful. A lump under the armpit may feel hard or thick and dense as compared with the armpit on the other side. Do not panic when you notice a lump, as it is not always cancerous. A few common causes of breast lumps are hormonal changes in the body, a breast infection and fat necrosis (damaged tissue). It is important to have your doctor check out any new or suspicious lumps on your breasts to determine the cause. When breasts hurt, most women think it is due to premenstrual syndrome (PMS), and when breasts feel hot or appear red, they usually suspect an infection. Swollen breasts with soreness or redness might even be a sign of breast cancer. A tumor located underneath a breast can push on tissues, making the breast swollen and sore. In some cases, the swelling is more prominent under the arm or under the collarbone. If you have this symptom for more than a week, be sure to see a doctor. Discharge from your nipple without any squeezing or touching of the breast may make any women wonder. Other than breast milk, any kind of nipple discharge can be a warning sign of breast cancer. When a tumor starts in the milk ducts just under or around the nipple, it may cause irritation and infection, leading to discharge. If you notice any nipple discharge, particularly clear or bloody, get it checked by your doctor. Such discharge can also be due to an infection or other condition that needs treatment. If you have an ache, sharp stab or pain in the breast or chest area, it is not a good sign. Sharp pain that comes and goes or a mild electric sensation that you may experience from one breast to another may be a sign of breast cancer. A tumor located directly behind the nipple or in one of the milk ducts often causes pain and discomfort in the breast. Though not necessarily a warning sign of cancer, pain in the breast or chest area can be due to other serious health issues that may need treatment. In medicine, the Sister Mary Joseph nodule or more commonly node, also called Sister Mary Joseph sign, refers to a palpable nodule bulging into the umbilicus as a result of metastasis of a malignant cancer in the pelvis or abdomen. Gastrointestinal malignancies account for about half of underlying sources, and men are even more likely to have an underlying cancer of the gastrointestinal tract. Gynecological cancers account for about 1 in 4 cases. Nodules will also, rarely, originate from appendix cancer Pseudomyxoma peritonei. Unknown primary tumors and rarely, urinary or respiratory tract malignancies cause umbilical metastases. How exactly the metastases reach the umbilicus remains largely unknown. Proposed mechanisms for the spread of cancer cells to the umbilicus include direct transperitoneal spread, via the lymphatics which run alongside the obliterated umbilical vein, hematogenous spread, or via remnant structures such as the falciform ligament, median umbilical ligament, or a remnant of the vitelline duct. Sister Mary Joseph nodule is associated with multiple peritoneal metastases and a poor prognosis. This video is targeted to blind users. Veterinary doctors may stage the tumor by performing a needle aspirate biopsy/cytology, complete blood count, serum chemistry profile, lymph node aspirate, and an abdominal ultrasound to check the liver and spleen. Further testing may include urine analysis, bone marrow aspirate, ultrasound, and X-rays. If mast cell tumors are detected, the best treatment is complete surgical removal. There are many specialty and emergency animal care hospitals that offer 24-hour emergency care and a full array of board-certified specialists for serious or critical conditions. These facilities are generally high-end surgery centers as well. It is considered safe to remove the mast cell tumor, as well as the tissue that surrounds it within a range of 2-3 cm. In some areas, where this 2-3 cm rule cannot be applied, a repeated biopsy and radiation treatment are done to ensure no further growth. Low-grade tumors can easily be cured through local surgery, whereas high grade tumors may require systematic procedures or chemotherapy. For this reason, it becomes crucial to continuously check your pet health to prevent cancer and seek treatment in early stages. While the cause remains unknown, seeking immediate treatment is an effective, preventative measure to ensure pet health. If large bumps are detected in your pet, it is important to remember to visit the nearest veterinary hospital that offers specialty and emergency animal care. Although these masses might grow somewhat quickly, they often go unnoticed at onset because of their asymptomatic nature. A persistent asymptomatic neck mass in an adult should be considered malignant until proven otherwise.6 Because delayed diagnosis contributes to decreased survival in conditions such as laryngeal cancer, it is paramount for family physicians to recognize common presentations of head and neck cancers.7,8 Squamous cell carcinomas of the upper aerodigestive tract are the most common primary neoplasms of the head and neck, and their metastases are often the source of cervical lymphadenopathy of unknown origin.8 Common presenting symptoms include nonhealing ulcers, dysarthria, dysphagia, odynophagia, loose or misaligned teeth, globus, hoarseness, hemoptysis, and oropharyngeal paresthesias.8 Lymph nodes associated with malignancy are usually firm, fixed, and matted. However, any persistent cervical lymphadenopathy or symptoms in the setting of risk factors, nonresponse to antibiotics, or unclear etiology warrants further investigation.7 Risk factors for upper aerodigestive tract cancers include male sex and use of alcohol, tobacco, or betel nut (commonly chewed by Southeastern Asians).8 Additional risk factors for oropharyngeal cancer include a family history of squamous cell carcinoma of the head and neck and poor oral hygiene.9 A subset of squamous cell carcinomas with increasing prevalence includes those related to human papillomavirus infection (especially high-risk human papillomavirus 16).9,10 These lesions present with rapidly enlarging, lateral, cystic lymph nodes; persistent cervical nodal hypertrophy; palatine or lingual tonsillar asymmetry; dysphagia; voice changes; or pharyngeal bleeding.10 The population most at risk is 35- to 55-year-old white men with a history of smoking, heavy alcohol use, and multiple sex partners (especially involving orogenital contact).9 Nearly 80% of salivary gland tumors are benign and arise in the parotid gland.1 These tumors are unilateral, asymptomatic, slow-growing, mobile masses, in contrast to malignant tumors, which may have rapid growth, skin fixation, pain, or cranial nerve involvement (especially cranial nerve VII [facial]).1,11 An intraparotid or isolated pre-auricular lymph node is another diagnostic possibility. Excisional biopsy is the preferred diagnostic approach for these tumors following review of contrast-enhanced CT or FNAB results.11 Persistent glandular enlargement with symptoms of xerophthalmia and xerostomia may point to Sjögren syndrome, whereas a hypertrophied, fibrotic gland may suggest chronic sialadenitis caused by a duct stone (sialolith) or stenosis.12 Skin cancers, especially melanoma, also metastasize to local lymph nodes. When a primary head and neck cancer is not evident to explain regional lymphadenopathy, physicians should search mucosal areas (nose, paranasal sinuses, oral cavity, and nasopharynx) for melanoma.7 Rarely, metastases from basal cell and squamous cell carcinoma lead to lymphadenopathy.7 Constitutional symptoms of fever, chills, night sweats, and weight loss may point to distant metastases. When metastases manifest as supraclavicular lymphadenopathy, FNAB reveals a malignancy in more than one-half of cases, with age older than 40 years being the major predictor of malignancy.13 The most common primary malignancy sites that produce cervical lymphadenopathy include the lungs, breasts, lymphomas, uterine cervix, gastroesophageal area, ovaries, and pancreas.13 The neck is a common area for lymphoma to present as a painless lymph node that may grow rapidly, and subsequently become painful. Early constitutional symptoms often precede development of diffuse lymphadenopathy and splenomegaly. In comparison to lymph nodes associated with metastatic disease characterized above, those associated with lymphoma are usually rubbery, soft, and mobile.11 Hodgkin lymphoma has a bimodal age distribution (15 to 34 years and older than 55 years)14 and rarely presents extranodally, whereas non-Hodgkin lymphoma is more common in older adults and is likely to present extranodally in the tonsillar ring located in the pharynx.1 Contrast-enhanced CT of the neck, chest, abdomen, and pelvis helps stage the lymphoma and provides possible biopsy sites.1 Rheumatologic diseases account for 3% of disorders presenting with enlarged salivary glands and 4% of those presenting with cervical lymphadenopathy.15 The rheumatic entities that most commonly cause salivary gland or cervical node enlargement include Sjögren syndrome and sarcoidosis.15 - video upload powered by https://www.TunesToTube.com

In this, the final Maps of Meaning lecture for 2017, I review the year and its offerings: What is a belief system? Why are people so inclined to engage in conflict to protect their belief systems? It's partly because our belief systems are not only systems of belief, but structures that serve to render everyone who participates in that belief and its dramatization and acting out in the world predictable, trustworthy and cooperative (even when competing). Is there a hierarchy of rank or value among belief systems, or are they merely arbitrary? What is the relationship between descriptions of the objective world and moral guidelines? How do you determine how to conduct yourself in the world? What should you do (and is that question even genuine -- or answerable?) What inbuilt structures do you bring into the world, as a consequence of biological evolution, that help you orient yourself in life, in the face of its overwhelming complexity? What is the relationship between the games that children learn to play when becoming socialized and the cultural structures that guide us in broader society? How is all this related to the underlying symbolic structures (religious structures) that sit at the base of our societies and belief structures? Just like we do screening for breast cancer, screening for cervical cancer, colon cancer, all of those cancers combined kill fewer people than lung cancer. Doctor Claudia Henschke talking to patient LEAD IN : A new study offers the strongest evidence yet that screening smokers for lung cancer with computerised chest scans can save lives, much as mammograms do for women with breast cancer. STORYLINE: The new study suggests that those at risk of lung cancer can greatly increase their chance of catching the cancer early if they undergo a CT scan. The scan which takes about 20 seconds and produces around 600 pictures of the lung area allows doctors to better identify small abnormalities in the lung. CT scans are sophisticated X-rays which produce images of the lungs from many angles and can reveal pea-size growths long before they produce symptoms. Interest in the scans rose in 1999, when Dr. Claudia Henschke of New York-Presbyterian Hospital/Weill Cornell Medical Centre published a landmark study showing that they found far more tumours than conventional X-rays did. Her new study, reported recently in New England Journal of Medicine, extends these results to a larger group of people and reports on survival.
