Prostate Cancer - What Symptoms to be Aware of

Dr Chris Steele MBE hopes to raise awareness of prostate cancer. It is sometimes said that men take more care of their cars than their bodies! Men often find it hard to deal with their health and are reluctant to visit their GP. They often ignore the fact that the sooner a problem is identified, the greater the chance of a full recovery. According to the most recent statistics from Cancer Research, 41,736 men in the UK were diagnosed with prostate cancer in 2011. In 2012, 10,837 men died from the condition. Whilst these are daunting figures, it’s also important to note that 84% of men diagnosed with prostate cancer go on to survive 10 or more years after. This is largely due to early detection, which is partly down to the PSA test which we will examine in more detail later. Prostate cancer usually develops slowly so you may not be aware that you have the condition for many years. It is often only when your prostate is large enough to affect your urethra that you become more aware of a possible problem. When this happens you may notice some of the following symptoms: •you need to urinate more often, especially at night •there is an urgency to rush to the toilet • you may have difficulty starting to urinate •the flow of urine is weak •you are also left with a feeling that your bladder hasn't fully emptied Symptoms that the cancer may have spread include: •bone and back pain •a loss of appetite •unexplained weight loss •pain in the testicles The cause of the problems is often a blockage as a result of an enlarged prostate. Many men's prostates get larger as they get older. This is due to a condition known as benign prostatic hyperplasia(BPH). This is a non-cancerous condition and is also known as benign prostatic enlargement(BPE). It is particularly common in those of us aged over 50 and isn't usually a serious threat to our health. The risk of prostate cancer is no greater for men with an enlarged prostate as it is for men without one. If your GP suspects that you may have an enlarged prostate, he may ask you to take a blood test that measures PSA. What is the PSA Test? The PSA (prostate-specific antigen) test is a blood test, which is used to detect the early signs of an enlarged prostate. It’s currently used as an initial test for those who are concerned about prostate cancer. The test works by identifying a protein that is only produced within the prostate gland. Some of this protein naturally leaks into the blood, and the amount present can determine the health of your prostate. Raised levels of PSA can indicate that you may have prostate cancer. The amount varies, depending on your age. •50 – 59 year olds. Your PSA level is considered high if it’s 3ng/ml or above •60 – 69 year olds. Your PSA level is considered high if it’s 4ng/ml or above •70 years or over. Your PSA level is considered high if it’s 5ng/ml or above It’s important to note that whilst elevated levels of PSA may indicate cancer, it can also be a symptom of other conditions, such as urinary infection, prostatitis or enlarged prostate. The test is not mandatory for men in the UK. Instead, the NHS offers an ‘informed choice programme’, where men can choose to have the test or not, based on understanding of the pro’s and con’s involved. Controversy Surrounding the Test The introduction of the PSA test has not been without its share of controversy. Whilst it is effective at identifying prostate cancer in its early stages, and thus considerably improving survival rates, it can be unreliable. A recent report featured in the Daily Mail indicates that as many as 7 out of 10 patients receive a false result from their PSA test, which then leads to unnecessary medical procedures such as biopsies, which can be painful. Likewise, treatment for prostate cancer, which includes options such as chemotherapy and surgery, not only cause considerable discomfort and emotional stress, but can lead to permanent health problems, such as erectile dysfunction and incontinence. Experts speculate that for every life saved by the PSA test, 45 further men undergo treatments that they didn’t actually need. Should You Have the Test? If you’re concerned about your prostate, the first thing you should do is arrange an appointment with your GP. Remember, an enlarged prostate is a very common problem in men over the age of 50, and is generally not a serious threat to your health. Your GP will discuss your symptoms with you, and then provide you with information about the testing available. You’ll then be able to make an informed decision, with help from your doctor. Whatever course of action you decide to take, remember that it is important to seek advice, rather than ignoring the problem. Although it’s likely to be nothing serious, it makes good sense to get things checked out as early as possible.

Men shouldn't be screened for prostate cancer with a common blood test, a widely followed federal advisory panel recommended on Monday. But the report isn't likely to quell a dispute about whether the test's risks outweigh its potential benefits. The U.S. Preventive Services Task Force recommended a "D" grade for prostate-specific--antigen, or PSA, testing which has been widely used for almost two decades to screen men for prostate cancer. So I’ve come to Cambridge University hospital to speak with Dr. Vincent Gnanapragasam to find out how they go about diagnosing this common cancer. Well Vincent, Prostate Cancer is notoriously difficult to diagnose isn’t it. Indeed, in fact Prostate Cancer most often has no symptoms. A man might go to his GP because he has notice a change in his urinary symptoms, getting up at night maybe or having difficulty passing water. And most often that’s due to a normal enlargement of the prostate as he gets older. In fact prostate cancer doesn’t cause any symptoms unless it is very advanced. In which case it might mimic some of those symptoms of an obstructed or large prostate. But if there are any concerns the first step is to go and see your GP who can then investigate further. What would your GP do? - Take a history - Do a physical examination - PSA blood test So what would your GP do? Most often they’ll take a history from you, do an examination of the prostate and do a PSA blood test. What is a PSA test? So PSA stand for Prostate Specific Antigene. It’s produced by the prostate gland and it’s a marker for the presence of Prostate Cancer. However it can also be raised by other conditions so it’s not a very specific marker but it’s the first test that we can do to see whether we should proceed to further investigations. What is a PSA test? A blood test that measures the amount of prostate specific antigen (PSA) in your blood. So what happens if you get sent to the hospital for an examination? The specialist will see you, again take a history and an examination and then most often what they will do is arrange a prostate biopsy. Increasingly they might recommend a scan to be done first, an MRI scan and then discuss with you what the best way to do a biopsy is. What is a Biopsy? The removal of a small sample of tissue for examination to determine the presence or extent of a disease. And if it is cancer tell us what happens then, tell us about some of the tests. Well, a discussion then starts. The first step is to make sure we know exactly what kind of Cancer it is and that might involve other types of scans, for example a CT scan or a bone scan. The kind of Cancer is determined by the size of it or the stage of a cancer. And once the specialist puts all that information together with what the biopsy has shown then they start to discuss with you what the treatments are."

In this short video, Consultant Urologist Mr Christopher Eden shares his expertise and describes the statistics for how common BPH is for men of varying ages. Knowing your risk factors may put your mind at ease. Step 1: Know role of age Know that the risk of developing prostate cancer increases with age. The disease is most common in men over 65. Tip Prostate cancer occurs when cells in the prostate gland become abnormal, mutate, and accumulate to form a tumor that grows and invades surrounding tissue. Step 2: Understand the role of race Understand that black men have a greater risk of developing the disease than men of other races. The reason for this is not known. Step 3: Be aware of heredity Be aware that prostate cancer runs in families. If men in your family are prone to the disease, there is an increased likelihood that you may get it as well. Step 4: Recognize the role of obesity Recognize that obesity increases your risk of developing advanced prostate cancer -- the most difficult form of the disease to treat. So take note -- this is one risk factor that you have some control over. Did You Know? In 2006, 28,373 men in the United States died of prostate cancer.

A man's prostate gland usually starts to enlarge after he reaches 40 years of age. This condition is called benign prostatic hyperplaisia (BPH) and is considered a natural process of aging. More than 70% of men in their 60's experience BPH symptoms. It is not cancer, and it does not raise your risk for prostate cancer. 5 Common Signs of Prostate Cancer Prostate cancer is a silent killer. Unlike many other cancers where symptoms occur early on, making the disease easier to detect, with prostate cancer there are no early warning signs. Only once a tumor causes the prostate gland to swell, or the cancer spreads beyond the prostate, will there be signs that the disease may be present. While regular screenings are essential for the identification and treatment of prostate cancer, if it goes undetected, the following are among the most common signs of the disease. 1. Urination Issues Experiencing troubles while urinating is among the most common indicators of prostate cancer. According to the Cancer Treatment Centers of America, these difficulties can include: -Burning or pain during urination -Difficulty urinating, or trouble starting and stopping while urinating -More frequent urges to urinate at night -Loss of bladder control -Decreased flow or velocity of urinee stream -Blood in urinee (hematuria) While these issues may indicate a prostate concern, they are not symptoms of the cancer. They occur as a result of the tumor in the prostate pressing on the bladder. 2. Swelling, Numbness or Pain in the Bones or Tissues It’s possible for the cancer to spread beyond the prostate and into the bones, which can cause pain and/or stiffness in the bones in areas such as the hips, back or chest. Prostate cancer can also spread to the nearby tissues and, more seriously, the lymphatic system. This can lead to symptoms such as swelling in the legs or pelvic area, as well as weakness or numbness in the legs or feet. 3. Constipation or Loss of bowell Control As mentioned previously, someone with prostate cancer may experience weakness or numbness in the legs or feet. This is caused by the cancer having spread to the tissues of the spine and pressing on the nerves. In addition to this weakness or paralysis of the lower limbs, a person may also experience digestive issues. This can either present itself as constipation, or a loss of bowell control. 4. Shortness of Breath It’s possible for someone with prostate cancer, typically in an advanced stage, to develop a conditional called anemia. Anemia is characterized as a drop in red blood cells, which results in there not being sufficient oxygen in the blood to meet the body’s needs. According to Prostate Cancer UK, symptoms of anemia can include “feeling tired or weak, being out of breath and looking pale.” Shortness of breath isn’t always due to anemia, however. It may also occur, along with chest pain, if the cancer spreads into a person’s lungs. 5. Loss of Weight, Fatigue and High Blood Pressure Although these symptoms are far more rare, it’s possible for prostate cancer to cause something called paraneoplastic syndrome. This is a group of symptoms that includes high blood pressure, fatigue and weight loss, as well as muscle cramps, illness and changes in the blood. 5 Common Signs of Prostate Cancer Prostate cancer is a silent killer. Unlike many other cancers where symptoms occur early on, making the disease easier to detect, with prostate cancer there are no early warning signs. Only once a tumor causes the prostate gland to swell, or the cancer spreads beyond the prostate, will there be signs that the disease may be present. While regular screenings are essential for the identification and treatment of prostate cancer, if it goes undetected, the following are among the most common signs of the disease. 1. Urination Issues Experiencing troubles while urinating is among the most common indicators of prostate cancer. According to the Cancer Treatment Centers of America, these difficulties can include: -Burning or pain during urination -Difficulty urinating, or trouble starting and stopping while urinating -More frequent urges to urinate at night -Loss of bladder control -Decreased flow or velocity of urinee stream -Blood in urinee (hematuria) While these issues may indicate a prostate concern, they are not symptoms of the cancer. They occur as a result of the tumor in the prostate pressing on the bladder. 2. Swelling, Numbness or Pain in the Bones or Tissues It’s possible for the cancer to spread beyond the prostate and into the bones, which can cause pain and/or stiffness in the bones in areas such as the hips, back or chest. Prostate cancer can also spread to the nearby tissues and, more seriously, the lymphatic system. This can lead to symptoms such as swelling in the legs or pelvic area, as well as weakness or numbness in the legs or feet. 3. Constipation or Loss of bowell Control As mentioned previously, someone with prostate cancer may experience weakness or numbness in the legs or feet. This is caused by the cancer having spread to the tissues of the spine and pressing on the nerves. In addition to this weakness or paralysis of the lower limbs, a person may also experience digestive issues. This can either present itself as constipation, or a loss of bowell control. 4. Shortness of Breath It’s possible for someone with prostate cancer, typically in an advanced stage, to develop a conditional called anemia. Anemia is characterized as a drop in red blood cells, which results in there not being sufficient oxygen in the blood to meet the body’s needs. According to Prostate Cancer UK, symptoms of anemia can include “feeling tired or weak, being out of breath and looking pale.” Shortness of breath isn’t always due to anemia, however. It may also occur, along with chest pain, if the cancer spreads into a person’s lungs. 5. Loss of Weight, Fatigue and High Blood Pressure Although these symptoms are far more rare, it’s possible for prostate cancer to cause something called paraneoplastic syndrome. This is a group of symptoms that includes high blood pressure, fatigue and weight loss, as well as muscle cramps, illness and changes in the blood. I did a lot of research, using only reliable websites like the ones you see on your screen, and I'll summarize what I learned about prostate cancer stages in the next minute or so. Prostate cancer stages indicate how far the cancer has progressed. A lower stage means cancer cells are confined to the prostate. A higher stage is assigned when the cancer has grown to invade nearby tissue or spread to distant areas of the body. Prostate cancer usually grows slowly and in the beginning, confines itself to the prostate gland. Some types of prostate cancer may not even need to be treated, depending on the age of the patient. But other types are more aggressive, can spread quickly, and require treatment. There are four stages. Stage I. At this stage, the cancer is located only in a small area of the prostate. Stage II. Cancer cells may still be small but could possibly be aggressive, depending on the type. Stage II could also, however, include a larger area that ha grown to involve both sides of the prostate gland. Stage III. At this stage, the cancer has spread beyond the prostate to nearby tissues. Stage IV. The cancer is invading nearby tissues, bones or organs, which may include the bladder, lymph nodes, or even those as far away as the lungs. Your doctor assigns your cancer a stage after testing. Of course, cancer in the early stages, when it's still confined to the prostate, has a better chance of successful treatment and a longer survival rate. ELIMINATED Prostate Cancer by Combination of Powerful Nutrient in Apple Peels, Turmeric Root & Grape

— When you dine on curry and baked apples, enjoy the fact that you are eating something that could play a role starving — or even preventing — cancer. New research from The University of Texas at Austin identifies several natural compounds found in food, including turmeric, apple peels and red grapes, as key ingredients that could thwart the growth of prostate cancer, the most common cancer afflicting U.S. men and a key area of focus during Men’s Health Month, which public health advocates celebrate in June. Published online this week in Precision Oncology, the new paper uses a novel analytical approach to screen numerous plant-based chemicals instead of testing a single agent as many studies do, discovering specific combinations that shrink prostate cancer tumors. “After screening a natural compound library, we developed an unbiased look at combinations of nutrients that have a better effect on prostate cancer than existing drugs,” says corresponding author Stefano Tiziani, assistant professor in the Department of Nutritional Sciences and Dell Pediatric Research Institute at UT Austin. “The beauty of this study is that we were able to inhibit tumor growth in mice without toxicity.” During the past decade, some cancer research has highlighted the potential therapies found in plants, including chemicals found in foods such as turmeric, apple peels and green tea. These compounds minimize one of the risk factors for cancer, inflammation within the body. People who have chronic inflammation because of chronic infection, autoimmune disease or conditions such as obesity have a higher cancer risk because of damage to normal cells. The researchers first tested 142 natural compounds on mouse and human cell lines to see which inhibited prostate cancer cell growth when administered alone or in combination with another nutrient. The most promising active ingredients were then tested on model animals: ursolic acid, a waxy natural chemical found in apple peels and rosemary; curcumin, the bright yellow plant compound in turmeric; and resveratrol, a natural compound common to red grapes or berries. “These nutrients have potential anti-cancer properties and are readily available,” says Tiziani. “We only need to increase concentration beyond levels found in a healthy diet for an effect on prostate cancer cells.” The new research paper also demonstrates how the plant-based chemicals work together. Combining ursolic acid with either curcumin or resveratrol prevents cancer cells from gobbling something that they need to grow, glutamine. This is a neat solution: blocking the uptake of a nutrient needed by prostate cancer cells with nutrients that are commonly in the human diet. Funders of this research include that National Institutes of Health and the University of Texas System. The experiment was designed, analyzed and written up with coauthors Alessia Lodi, John DiGiovanni and Achinto Saha, all of UT Austin. Additional authors include Xiyuan Lu, Bo Wang, Enrique Sentandreu, Meghan Collins, all of UT Austin; and Mikhail Kolonin of The Brown Foundation Institute of Molecular Medicine at the University of Texas Health Science Center in Houston.

New prostate cancer screening guidelines released

The U.S. Preventive Services Task Force has issued new guidelines for prostate cancer screening. It says men between the ages of 55 and 69 should talk to their doctors about getting the blood test that looks for markers of the disease. The panel had previously advised against routine screening.
