5 Common Signs of Prostate Cancer Prostate cancer is a silent killer. Unlike many other cancers where symptoms occur early on, making the disease easier to detect, with prostate cancer there are no early warning signs. Only once a tumor causes the prostate gland to swell, or the cancer spreads beyond the prostate, will there be signs that the disease may be present. While regular screenings are essential for the identification and treatment of prostate cancer, if it goes undetected, the following are among the most common signs of the disease. 1. Urination Issues Experiencing troubles while urinating is among the most common indicators of prostate cancer. According to the Cancer Treatment Centers of America, these difficulties can include: -Burning or pain during urination -Difficulty urinating, or trouble starting and stopping while urinating -More frequent urges to urinate at night -Loss of bladder control -Decreased flow or velocity of urinee stream -Blood in urinee (hematuria) While these issues may indicate a prostate concern, they are not symptoms of the cancer. They occur as a result of the tumor in the prostate pressing on the bladder. 2. Swelling, Numbness or Pain in the Bones or Tissues It’s possible for the cancer to spread beyond the prostate and into the bones, which can cause pain and/or stiffness in the bones in areas such as the hips, back or chest. Prostate cancer can also spread to the nearby tissues and, more seriously, the lymphatic system. This can lead to symptoms such as swelling in the legs or pelvic area, as well as weakness or numbness in the legs or feet. 3. Constipation or Loss of bowell Control As mentioned previously, someone with prostate cancer may experience weakness or numbness in the legs or feet. This is caused by the cancer having spread to the tissues of the spine and pressing on the nerves. In addition to this weakness or paralysis of the lower limbs, a person may also experience digestive issues. This can either present itself as constipation, or a loss of bowell control. 4. Shortness of Breath It’s possible for someone with prostate cancer, typically in an advanced stage, to develop a conditional called anemia. Anemia is characterized as a drop in red blood cells, which results in there not being sufficient oxygen in the blood to meet the body’s needs. According to Prostate Cancer UK, symptoms of anemia can include “feeling tired or weak, being out of breath and looking pale.” Shortness of breath isn’t always due to anemia, however. It may also occur, along with chest pain, if the cancer spreads into a person’s lungs. 5. Loss of Weight, Fatigue and High Blood Pressure Although these symptoms are far more rare, it’s possible for prostate cancer to cause something called paraneoplastic syndrome. This is a group of symptoms that includes high blood pressure, fatigue and weight loss, as well as muscle cramps, illness and changes in the blood. So I’ve come to Cambridge University hospital to speak with Dr. Vincent Gnanapragasam to find out how they go about diagnosing this common cancer. Well Vincent, Prostate Cancer is notoriously difficult to diagnose isn’t it. Indeed, in fact Prostate Cancer most often has no symptoms. A man might go to his GP because he has notice a change in his urinary symptoms, getting up at night maybe or having difficulty passing water. And most often that’s due to a normal enlargement of the prostate as he gets older. In fact prostate cancer doesn’t cause any symptoms unless it is very advanced. In which case it might mimic some of those symptoms of an obstructed or large prostate. But if there are any concerns the first step is to go and see your GP who can then investigate further. What would your GP do? - Take a history - Do a physical examination - PSA blood test So what would your GP do? Most often they’ll take a history from you, do an examination of the prostate and do a PSA blood test. What is a PSA test? So PSA stand for Prostate Specific Antigene. It’s produced by the prostate gland and it’s a marker for the presence of Prostate Cancer. However it can also be raised by other conditions so it’s not a very specific marker but it’s the first test that we can do to see whether we should proceed to further investigations. What is a PSA test? A blood test that measures the amount of prostate specific antigen (PSA) in your blood. So what happens if you get sent to the hospital for an examination? The specialist will see you, again take a history and an examination and then most often what they will do is arrange a prostate biopsy. Increasingly they might recommend a scan to be done first, an MRI scan and then discuss with you what the best way to do a biopsy is. What is a Biopsy? The removal of a small sample of tissue for examination to determine the presence or extent of a disease. And if it is cancer tell us what happens then, tell us about some of the tests. Well, a discussion then starts. The first step is to make sure we know exactly what kind of Cancer it is and that might involve other types of scans, for example a CT scan or a bone scan. The kind of Cancer is determined by the size of it or the stage of a cancer. And once the specialist puts all that information together with what the biopsy has shown then they start to discuss with you what the treatments are."

Men Should Skip Common Prostate Cancer Test

Men shouldn't be screened for prostate cancer with a common blood test, a widely followed federal advisory panel recommended on Monday. But the report isn't likely to quell a dispute about whether the test's risks outweigh its potential benefits. The U.S. Preventive Services Task Force recommended a "D" grade for prostate-specific--antigen, or PSA, testing which has been widely used for almost two decades to screen men for prostate cancer. Previously the task force had recommended against PSA testing for men age 75 and older. Now the recommendation extends to all ages. I did a lot of research, using only reliable websites like the ones you see on your screen, and I'll summarize what I learned about prostate cancer stages in the next minute or so. Prostate cancer stages indicate how far the cancer has progressed. A lower stage means cancer cells are confined to the prostate. A higher stage is assigned when the cancer has grown to invade nearby tissue or spread to distant areas of the body. Prostate cancer usually grows slowly and in the beginning, confines itself to the prostate gland. Some types of prostate cancer may not even need to be treated, depending on the age of the patient. But other types are more aggressive, can spread quickly, and require treatment. There are four stages. Stage I. At this stage, the cancer is located only in a small area of the prostate. Stage II. Cancer cells may still be small but could possibly be aggressive, depending on the type. Stage II could also, however, include a larger area that ha grown to involve both sides of the prostate gland. Stage III. At this stage, the cancer has spread beyond the prostate to nearby tissues. Stage IV. The cancer is invading nearby tissues, bones or organs, which may include the bladder, lymph nodes, or even those as far away as the lungs. Your doctor assigns your cancer a stage after testing. Of course, cancer in the early stages, when it's still confined to the prostate, has a better chance of successful treatment and a longer survival rate. 5 Common Signs of Prostate Cancer Prostate cancer is a silent killer. Unlike many other cancers where symptoms occur early on, making the disease easier to detect, with prostate cancer there are no early warning signs. Only once a tumor causes the prostate gland to swell, or the cancer spreads beyond the prostate, will there be signs that the disease may be present. While regular screenings are essential for the identification and treatment of prostate cancer, if it goes undetected, the following are among the most common signs of the disease. 1. Urination Issues Experiencing troubles while urinating is among the most common indicators of prostate cancer. According to the Cancer Treatment Centers of America, these difficulties can include: -Burning or pain during urination -Difficulty urinating, or trouble starting and stopping while urinating -More frequent urges to urinate at night -Loss of bladder control -Decreased flow or velocity of urinee stream -Blood in urinee (hematuria) While these issues may indicate a prostate concern, they are not symptoms of the cancer. They occur as a result of the tumor in the prostate pressing on the bladder. 2. Swelling, Numbness or Pain in the Bones or Tissues It’s possible for the cancer to spread beyond the prostate and into the bones, which can cause pain and/or stiffness in the bones in areas such as the hips, back or chest. Prostate cancer can also spread to the nearby tissues and, more seriously, the lymphatic system. This can lead to symptoms such as swelling in the legs or pelvic area, as well as weakness or numbness in the legs or feet. 3. Constipation or Loss of bowell Control As mentioned previously, someone with prostate cancer may experience weakness or numbness in the legs or feet. This is caused by the cancer having spread to the tissues of the spine and pressing on the nerves. In addition to this weakness or paralysis of the lower limbs, a person may also experience digestive issues. This can either present itself as constipation, or a loss of bowell control. 4. Shortness of Breath It’s possible for someone with prostate cancer, typically in an advanced stage, to develop a conditional called anemia. Anemia is characterized as a drop in red blood cells, which results in there not being sufficient oxygen in the blood to meet the body’s needs. According to Prostate Cancer UK, symptoms of anemia can include “feeling tired or weak, being out of breath and looking pale.” Shortness of breath isn’t always due to anemia, however. It may also occur, along with chest pain, if the cancer spreads into a person’s lungs. 5. Loss of Weight, Fatigue and High Blood Pressure Although these symptoms are far more rare, it’s possible for prostate cancer to cause something called paraneoplastic syndrome. This is a group of symptoms that includes high blood pressure, fatigue and weight loss, as well as muscle cramps, illness and changes in the blood. This is a short video by Dr. Herb Riemenschneider of Riverside Urology in Columbus, Ohio about Benign (noncancerous) Prostatic Hypertrophy or BPH. www.RiversideUrology.com Bph104 BPH is probably the most common prostate problem for men. The prostate is an organ that sits below the bladder and the urethra runs directly through it which makes it a vital component in the urination process. When the bladder pushes hard against the resistance of an enlarged prostate the muscles in the bladder hypertrophy or get thick. Also when the prostate is enlarged the channel or urethra that runs through the prostate is narrowed. If you think of the prostate as a doughnut, the doughnut hole being the channel through which the urine passes, the benign enlargement occurs closest to the doughnut hole. The bladder has a unique characteristic called compliance. Compliance means the bladder muscles actually slide one over the other so you can go from a very small content after you've emptied your bladder to a very large content before you have to do something about it. When the bladder muscle thickens the compliance breaks down and instead of having a normal voiding pattern they get sort of fits and starts and that's where people get urgency and sometimes soil themselves. Some common symptoms include frequent and urgent needs to urinate, dribbling or leaking after urination, intermittent or weak stream, straining to urinate, pain or burning during urination and feeling that the bladder never completely empties. Peak urinary flow and voided volume often deteriorate with age. To help diagnose this we can do a physical examination by doing a digital rectal exam to evaluate the anal sphincter tone and prostate gland with regard to approximate size, consistency, shape and abnormalities. Another tool is to use a daily voiding diary to track and determine a symptom index score for BPH. If the score is less than eight you can probably live with it. If it's under 20 you should probably be on some sort of treatment. If it's over 20 or close to 30 you'll probably need to have some sort of intervention. BPH needs to be treated when your symptoms are severe enough to affect your quality of life. We can use medications to alter the bladder function and how it responds to stimulation. There are many other ways to solve the problem including Green Light Laser. With the green light laser you can vaporize tissue and it coagulates blood very efficiently so you can literally remove prostate tissue thereby opening the channel and not have any bleeding. It can be a very efficient treatment with minimal discomfort and symptom relief is typically immediate.

Benign Prostatic Hyperplasia (BPH) and Prostate Cancer epidemiology, etiliogy, causes and symptoms, investigations, treatment and management. Handwritten full lecture for USMLE. ANATOMY of PROSTATE Central zone of prostate sorrounds ejaculatory duct. The Transitional zone of prostate encompases meddle part of urethra. Peripheral zone is placed outside. Prostate cancer is most common in peripheral zone (60-70%). The Central zone is the second most common site for prostate cancer (25%). The Transition zone only has an occurence rate of 5% for prostate cancer. Benign Prostate Hyperplasia (BPH) is in the transition zone and that is why it presents mostly with urinary symptoms. BENIGN PROSTATIC HYPERPLASIA (BPH) Prostate is heavily influenced by DHT and testosterone, however, DHT is ten times more sensitive. Testosterone is converted to DHT by 5alpha reductase. This causes hyperplasia of collagen cells and the smooth muscle. The collagen will cause a physical obstruction while the smooth muscle will cause mechanical obstruction. Eventually there is bladder enlargement which may forma false diverticulum (outpouching of the bladder). Clinical Signs and Symptoms Obstructive symptoms - Consists of the patients experiencing hesitancy, difficulty voiding, straining, decrease in the force of stream and dribbling of urine. Irritation symptoms - increase frequency or urgency as well as nocturia. These symptoms occur as the bladder gets larger and begins to overflow. These symptoms can produce an International Prostate Symptoms Score (IPSS) which determines severity of Benign prostatic Hyperplasia through symptoms. Diagnosis of Benign Prostatic Hyperplasia Digital Rectum Exam is done initially, however it doesn't correlate well with symptoms and it is very difficult to palpate. Urinalysis is also done to rule out prostatitis. PSA is organ specific but can be elevated in cancer and prostatitis. Ultrasound or CT to look for complications such as bladder distention. Transrectal ultrasound is used before surgery. Treatment for Benign Prostatic Hyperplasia (BPH) If the degree of benign prostatic hyperplasia (BPH) is minimal such as 0-7 on the IPSS score than watchful waiting because most spontaneously. So for these patients continue monitoring PSA and Digital Rectums Exams. If getting worse continue to medical treatment. Alpha 1 blocker allows relaxation of the smooth muscle that may be constricting the urethra. Finasteride is a 5 alpha reductase inhibitor to prevent formation of DHT from testosterone and help debulk the prostate. Commonly used drugs are terazosin and doxazosin but this can lead to hypotension. More specific is an Alpha A1 blocker such as tamsolusin which is more specific for the prostate and therefore hypotension is not present but can lead to retrograde ejaculation. Surgery - If medical therapy doesn't work for the Benign Prostatic Hyperplasia (BPH). Two procedures transurethral and prostatectomy. PROSTATE CANCER CLINICAL SIGNS AND SYMPTOMS of PROSTATE CANCER Can range from asymptomatic to completely fatal. Screening - Digital Rectal Exam is not helpful and PSA is the only method being used. Local Disease causes symptoms similar to Benign Prostatic Hyperplasia symptoms, but this suggests involvement of the transition zone. Metastasis disease include symtpoms related to osteoblastic bone growth, primarily the spine leading to neurological symptoms and back pain. Brachytherapy is the new treatment. Radical Prostatectomy - May lead to neruovascular damage causing Erectile Dysfunction and damage to urethra my lead to incontinence. After treatment continue to monitor PSA to look for Metastasis or resurgency of cancer. TREATMENT OF METASTASIS OF PROSTATE CANCER Castration - Orchiectomy (Gold Standard), Leuprolide/Ghrelin (GnRH) but these have a temporary increase in androgens, and Anti-Androgens (Flutamide). Castration Resistant Prostate Cancer (CRPC) are tumors that are still growing despite castration. Then use chemotherapy (Doxitexal, Mitoxantrone, Dosatinib)
