I get to work with families, kids and babies every single day. I recently had my first baby – isn’t he cute!!!! When he was delivered at Methodist Women’s Hospital a few months ago, right after delivery, I was given the option of three important medications I knew were essential to his health. Today, I wanted to talk to you about those medications, and just why we here at Methodist recommend that you consider them for your child. The first is a Vitamin K injection. All babies are born with very low levels of Vitamin K, which is essential in blood clotting. While babies can produce some Vitamin K, it’s simply not enough. Giving newborns injectable Vitamin K at birth immediately gives them enough Vitamin K to prevent their risk of a problem called Vitamin K Deficiency Bleeding – or VKDB. VKDB is a clotting disorder that causes spontaneous bleeding. It is often internal and can cause brain damage or even death. A Vitamin K injection at birth will prevent VKDB for six months – when they are old enough to eat foods which can provide enough daily Vitamin K to prevent this spontaneous bleeding. Now, some parents ask about oral Vitamin K medication instead of an injection. There is an oral option, but your baby will need more than one dose. Talk with the hospital and your child’s health care provider about options for oral vitamin K. The next medication is also an injection. It’s the Hepatitis B Vaccine. The Hepatitis B vaccine is a series of vaccines given to prevent a virus that can cause serious liver disease. Illness from Hepatitis B can either last a few weeks, or lead to a life-long chronic illness that can lead to liver damage, liver cancer and even death. The first dose of this vaccine is given right after birth. That’s essential to prevent accidental exposure to Hepatitis B at birth. Other doses of the vaccine will be given at your baby’s follow-up appointments with their pediatrician. The final medication you will be asked about following delivery of your baby isn’t an injection. It’s an antibacterial eye ointment known as Erythromycin. Your baby could contract an eye infection as it passes through the birth canal. Eye diseases as a result of these infections can cause blindness. Erythromycin eye drops placed in the baby’s eyes can help prevent infection. While all medications have risks, you should know that the risks and side effects for all three of these medications is extremely low. Allergic reactions to Vitamin K are rare, and the biggest risk with the Hepatitis B vaccine is pain or redness at the injection site. Here at Methodist, we take the health and safety of your baby very seriously. While vaccinating your infant is a decision you should make based on your beliefs, we recommend you consider these medications be given to your newborn following delivery to prevent harmful illness. As a pediatrician, I get a lot of comfort knowing my little guy – and your new little ones – are safe. COMPLETE SCRIPT BELOW: Hello, today we will be exploring the idea behind vaccines - what they are and how they work. We will also be answering a few commonly asked questions to clear up common misconceptions. What exactly are vaccines? They are ‘preparations’ that induce an adaptive immune response without experiencing the disease itself. This allows us to be immune from otherwise lethal pathogens. Take tetanus for example, caused by a rather nasty bacteria Clostridium tetani. You can contract this via rusty nails (which is funny as they look like microscopic nails), soil, or even dog bites! C. tetani causes “Tetanus”, a disease that can result in severe muscle spasms and can induce respiratory failure and death. Numerous reports have stated the mortality rate ranges to be anywhere from 40 to 78% of non-vaccinated infections of C. tetani. If only there was some way to prevent such a nasty sounding disease… Oh wait, there is. Its called tetanus vaccine. Tetanus vaccine is a toxoid vaccine. A toxoid vaccine refers to a modified and inactivated version of the original toxin of interest. In this case we are giving you a tetanus toxoid, which will never actually harm you. During a normal infection, the toxin produced by C. tetani will migrate through nerve cells. Interestingly enough, it will keep migrating until it reaches the spinal cord, where it then interacts with the inhibitory receptors of neurons that control muscles. The toxin binds to a receptor called VAMP which blocks the “off switches” for the neuron. This results in hyper-neuronal activity and the muscle spasms characteristic of tetanus. The toxoid vaccine is thus inactivated but still recognizable. This means it cannot bind to the inhibitory receptors of nerve cells. What the toxoid actually does is stimulate B and T cells. The toxoid is taken up by scouting cells, processed, and the information about the toxin is shared with effector cells. In the resulting immune response, “memory cells” are created, which will remember the exact antigen. So, in the event that we do come into contact with tetanus, our memory cells can eradicate the pathogen quickly and effectively. (To help stimulate the immune response, adjuvants are used. These are chemicals that help “wake up” patrolling cells which release chemical signals (interleukins) to initiate an immune response. Chemicals such as aluminum hydroxide, alum, or even interleukins themselves are commonly used adjuvants) Common concerns with vaccines are the potential safety concerns of the chemicals used in a vaccine. Obviously the pathogen of interest is included, albeit in a weakened state. Commonly used chemicals include formaldehyde and some adjuvants. “But I thought formaldehyde is dangerous?” It is, but the incredibly low amounts used doesn’t even compare to what we eat and what our body naturally produces. It is also a vital part of vaccines as it is a very good agent to inactivate pathogens. And as for “do vaccines cause autism? The answer is no. Just look up the case surrounding Andrew Wakefield. Three people have died and more than 100 people have suffered severe bleeding from smoking weed believed to be laced with rat poison. The deaths all happened in the state of Illinois but four other states have reported cases. Tests have been carried out on the synthetic weed and several patients and they came back positive for a lethal ingredient often used in rat poison. The federal Centers for Disease Control and Prevention have issued a national alert patients with severe, unexplained bleeding may be additional cases. The CDC is helping Illinois authorities investigate the outbreak in that state, which began in early March. Illinois reported seven more cases on Tuesday, bringing the nationwide total to at least 116. Symptoms include coughing up blood, blood in the urine, severe bloody noses, bleeding gums and internal bleeding. CDC’s alert says outside Illinois, emergency rooms in Indiana, Missouri, Wisconsin and Maryland have treated affected patients since March 10. Illnesses have been linked to fake marijuana before but this is the first outbreak involving rat poison contamination, said the CDC’s Renee Funk. Those sickened require hospitalization and treatment with vitamin K to control bleeding, she said. ‘This is an unusual outbreak,’ Funk said Tuesday, adding that is unclear how the contamination occurred. Fake marijuana, also called synthetic cannabinoids, contains man-made chemicals that produce a high similar to marijuana. It is sold in smoke shops and other stores as liquids that can be used in e-cigarettes or in dried plant material that can be smoked. Nicknames include K2, Spice and Kush. ‘The number of cases continues to go up each day,’ Melaney Arnold, spokeswoman for the Illinois Department of Public Health, said Tuesday. ‘Synthetic cannabinoids in general are not safe and this is one example of not knowing what chemicals are in the product. We are telling people don’t use synthetic cannabinoids.’ The federal government and many states have banned some of these products or specific ingredients, but the CDC says manufacturers skirt these laws by creating new products or labeling them ‘not for human consumption.’ Tests on fake pot bought at a Chicago convenience store by undercover agents detected the rat poison ingredient, the U.S. attorney’s office said in a news release. Three store employees were arrested and face federal drug charges. Illnesses from fake marijuana have increased in recent years; a CDC report noted at least 456 cases between 2010 and 2015. The products are up to 100 times more potent than the active ingredient in marijuana and severe reactions have included seizures, coma and delirium.

Researchers in the United States reported recently that the flu virus appears to disarm the body's first line of defense against disease. When the virus attacks, the body's natural defenses produce white blood cells. These cells make antibodies that link up to the invading microorganism. They neutralize it and, in that way, prevent or lessen the severity of infection. The cells also remember the invading virus so they can fight it in the future. That is the traditional understanding of how the body fights the flu virus. Now biologists have discovered how the virus can disarm those white blood cells, known as B cells. Hidde Ploegh is with the Whitehead Institute for Biomedical Research in Cambridge, Massachusetts. He led the team of researchers. Their laboratory studies with special mice suggest the flu virus enters the B cells and interferes with the production of antibodies. This can kill the B cells and the body's first line of defense. Mr. Ploegh says that may explain why some forms of influenza can cause a stronger physical reaction than others. Normally, the frontline antibodies occupy lung cells to protect the body against future viral invasions through breathing. But the flu virus, having disabled the antibodies, may instead attack lung cells. In that way, the virus blocks the antibody's ability to remember the invader. Hidde Ploegh says that by interfering with the body's "frontline troops", the virus has more time to reproduce and establish itself in the cells. A report on how influenza overcomes the body's immune system was published in the journal Nature.

Facts: Two hours after Hannah Bruesewitz received her six-month diphtheria, tetanus and pertussis vaccine in 1992, she started developing seizures and was hospitalized for weeks. Hannah has continued to suffer from residual seizure disorder that requires her to receive constant care, according to her parents. When their daughter was three-years-old, Russell and Robalee Bruesewitz filed a petition seeking compensation for her injuries. One month prior to the petition, new regulations eliminated Hannah's seizure disorder from the list of compensable injuries. The family's petition was denied. Three years later, in 1998, the drug company Wyeth withdrew the type of vaccine used in Hannah's inoculation from the market. The Bruesewitzes filed a lawsuit against Wyeth in state court in Pennsylvania. They claimed the drug company failed to develop a safer vaccine and should be held accountable for preventable injuries caused by the vaccine's defective design. A federal judge dismissed the lawsuit, ruling that the National Childhood Vaccine Injury Act protected Wyeth from lawsuits over vaccine injury claims. The U.S. Court of Appeals for the 3rd Circuit affirmed. Question: Can a federal law shield vaccine manufacturers from certain product liability lawsuits in state court that seek damages for serious health problems suffered by children? Conclusion: Yes. Delaware County Council Encourages All Residents To Receive Flu Vaccination

Delaware County Council reminds all residents about the importance of getting an annual flu shot. Whether it's the standard flu shot, the needle-free squirt-in-the nose option or the new smaller needle version, a flu vaccination is the best way to guard against the flu. Delaware County's flu prevention campaign is conducted by County Council, ICH and the county's Senior Medical Advisor Dr. George Avetian, the Pennsylvania Department of Health (DoH), and members of the Delaware County Flu Task Force. "Receiving the flu vaccine is the first and most important step in preventing the flu and decreasing the risk of severe flu-related illnesses," said County Councilman Tom McGarrigle. "Each year, in the United States, 200,000 people are hospitalized due to complications from the flu and 36,000 people due from the flu and related complications. We want to ensure that Delaware County residents stay healthy." Dr. Avetian said everyone 6 months and older should get vaccinated against the flu, and sufficient supplies of vaccine are now available. People at the greatest risk of serious flu complications include young children, pregnant women, people with chronic health conditions such as asthma, diabetes or heart and chronic lung disease and individuals age 65 and older. The flu season usually occurs from fall through early spring. The peak of flu season has occurred anywhere from late November through March. The overall health impact of the flu, including infections, hospitalizations, and deaths, varies from year to year. The 2011-2012 vaccine will protect against three strains of influenza: A H3N2 virus, an influenza B virus and the H1N1 virus that emerged in 2009 to cause a pandemic. It is estimated that 166 million doses of flu vaccine will be produced this year. "We recommend that everyone seek a flu vaccination from their family physician or at a public flu shot clinic, perhaps at their pharmacy," Dr. Avetian said. He said efforts are being made to reach out to the entire community including senior citizens and members of the county's culturally and ethnically diverse population. In addition to immunization, there are everyday preventative measures that inhibit the spread of influenza virus, including frequent hand-washing with antibacterial soap, covering your nose and mouth when coughing or sneezing, proper disposal of tissues, and, if you are sick with flu--like illness, the CDC recommends that you stay home for at least 24 hours after your fever is gone except to get medical care or for other necessities. Dr. Avetian also reminds people to contact their physician if they develop flu symptoms. "There are effective antiviral medications that are not antibiotics which can make flu symptoms less intense and of shorter duration," he said. "These are prescription medications and are not available over-the -counter."
