Absence of menses is normal in pregnant, breastfeeding and menopausal women, but pathological otherwise. Amenorrhea is not a disease on its own, but rather a symptom of a variety of underlying conditions. PRIMARY amenorrhea is when a woman has NEVER had her periods, while SECONDARY amenorrhea is when a woman has STOPPED having them. Menstruation is part of the menstrual cycle, the monthly events that occur within a woman’s body in preparation for the possibility of pregnancy. Each month, an egg is released from an ovary in a process called ovulation. At the same time, the lining of the uterus THICKENS, ready for pregnancy. If fertilization does NOT take place, the lining of the uterus is shed in menstrual bleeding and the cycle starts over. The menstrual cycle is under control of multiple hormones secreted by the hypothalamus, pituitary gland, and ovaries. Basically, the hypothalamus produces gonadotropin-releasing hormone, GnRH; the anterior pituitary secretes follicle-stimulating hormone, FSH, and luteinizing hormone, LH; while the ovaries produce estrogen and progesterone. These hormones are involved in a REGULATORY network that results in monthly cyclic changes responsible for follicular maturation and ovulation. Amenorrhea can be caused by ANATOMICAL or ENDOCRINE problems. Anatomical causes refer to abnormalities in the female reproductive system and include: - absent or underdeveloped female organs in some genetic disorders, such as MRKH syndrome - congenital defects that OBSTRUCT blood outflow - and destruction of the uterine cavity by previous infections or surgeries. Endocrine problems refer to structural or functional defects of the hypothalamus, pituitary gland and ovaries. A common cause in this category is the impaired function of the hypothalamus which occurs when the hypothalamic-pituitary-ovarian axis is SUPPRESSED due to an ENERGY DEFICIT. This can result from weight loss, eating disorders, excessive exercise, malabsorption syndromes, or emotional stress. The common feature is a REDUCED production of GnRH by the hypothalamus, which results in corresponding LOW levels of FSH and LH and subsequent impairment of follicular maturation and absence of ovulation. Other endocrine causes include: - Kallmann’s syndrome, a genetic disorder associated with congenital defects of the hypothalamus, causing GnRH deficiency. - Sheehan’s syndrome, a condition in which excessive blood loss during childbirth or chronic hypotension during pregnancy IMPAIRS PITUITARY functions. - Tumors, infections, trauma or autoimmune destruction of the pituitary gland. - Polycystic ovary syndrome, an endocrine disorder in which FSH deficiency disrupts follicle maturation. - Loss of normal ovarian function in conditions such as Turner's syndrome - Thyroid disorders Treatment is by addressing the underlying cause and can range from nutrition plans, hormonal therapy to surgical interventions.

16 Reasons for Missed or Irregular Periods in Hindi | By Ishan While most women aren’t exactly excited about menstruation, most realize the importance of this bleeding when, suddenly, one month it either doesn’t appear or it shows up when least expected. What’s going on when you have irregular periods? Or why do they suddenly stop? Let’s start by understanding the mechanics of the menstrual cycle. During a woman's fertile life, the eggs mature within the ovaries in a cycle that typically lasts 28 days (this varies from woman to woman and even from period to period). On day five of the cycle, about twenty eggs start to mature in the follicles of the ovaries. Each of these follicles is like a tiny fluid-filled sac. As day fourteen approaches, one follicle has generally matured earlier than the others and releases its mature egg to be fertilized. What happens to the other follicles? They shrivel up and are reabsorbed by the body. But the mature follicle transforms into what is known as a "corpus luteum," which is responsible for producing the hormone progesterone to prepare the uterus to receive the fertilized egg. If the egg is not fertilized, it disintegrates and dies. Likewise, the corpus luteum withers. This causes the uterus to shed its lining, the endometrium, causing the bleeding of menstruation. What is a regular menstrual cycle? Having a regular cycle means that the interval between periods is consistent. As mentioned earlier, it’s usually 28 days, but depending on the individual woman, can range from every 20 to every 35 days. What is an irregular menstrual cycle? Having an irregular cycle means that the interval between periods varies each month. That is, sometimes they come every 28 days, sometimes every 20, sometimes every 30. Having irregular periods is very common.The causes can range from something insignificant to something that requires treatment. For example, if you're a teen, your body's hormones can fluctuate and take some time until they find a balance. Therefore, it’s normal to have an irregular period during adolescence or, sometimes, for your period not to arrive in a given month. But, before we get down to business, remember that the main cause for your period not to arrive is a pregnancy. So, if your period doesn’t arrive, check to make sure it’s not actually a baby on the way. Now, let’s look at the most common causes of irregular periods: 1.Eating disorders, like anorexia and bulimia 2.Excessive weight gain or weight loss; extreme thinness and obesity both cause your menstrual cycle to become irregular or even to disappear 3.Stress or emotional problems 4.Hormonal problems, for example, when the thyroid malfunctions Travel 5.Over-exercising: If you're an athlete and train very hard, you may stop menstruating because your body wants to survive and save energy and menstruation requires energy 6.Problems with the pelvic organs (such as polycystic ovarian syndrome, for example) 7.Drugs like birth-control pills can affect the frequency and / or intensity of menstruation 8.Breastfeeding Irregular periods or a stop of menstruation can be due to a condition called "premature ovarian failure" which causes a woman to stop having her period before age 40. This can be caused by radiation, surgery or chemotherapy, in the case of a woman with cancer. Don’t forget that if you are sexually active and don’t want to get pregnant, you should use a birth control method you can trust so that irregular periods don’t bring you an unexpected surprise. Also, remember that even if your period goes away for a while, this does not mean you cannot become pregnant. You are still at risk! If you're not planning to have a baby, be careful! And, as always, I recommend that you consult your healthcare provider if you have questions about what’s causing your irregular periods. What are the signs you are pregnant while on birth control? You hit that row of do nothing pills in the birth control pack and your period does not come. It could come late for other reasons like hormones out of whack. And it was concern that my hormones would be messed up that I did not use them. Then I cannot say that you got pregnant on the pill because you used antibiotics. Antibiotics can make you poop a lot, passing the pills through faster. If you were taking thyroid medication, it would metabolize them faster. However, if you get a stomach bug and throw them up, you could get pregnant. If I’m throwing up a lot, that’s a sign I am pregnant on birth control. If you are nauseous or throwing up in the morning but not in the afternoon or evening, then it is likely due to pregnancy. If it is all day, then you may need antibiotics or another medication. I have not been taking antibiotics. If you were on the pill, you’d be able to guess you were pregnant if you had a light period the next cycle and none the one after that. With a birth control implant, though, a woman would not have a period at all. Well, most of the time. You could develop breast tenderness in early pregnancy. I get that with my normal cycle. Peeing a lot is a sign of pregnancy. Or a urinary tract infection. If you used an injected contraceptive, your odds of pregnancy are really low barring medication that interferes with it. I do not use the injectables. What are the other signs I could be pregnant while on birth control? If you used an intrauterine, it causes heavier than normal periods. If you’re pregnant, the period stops. That’s usually normal regardless of the contraception method. You could be certain you did a pregnancy test and it came back positive. Don’t birth control methods interfere with that? If you used a condom, cervical cap or even an intrauterine, you’d have the normal hormonal cycles for ovulation and pregnancy. It is only the hormone based pills and shots that throw off your hormones. And it may be that my contraceptive method failed. If you were taking birth control pills, the estrogen or progesterone won’t throw off the pregnancy test. In fact, no medicine you take for anything would mess it up. So a pregnancy test is the best way to see if I’m pregnant. And it would be a sure sign to talk to your doctor about the best way to stop your birth control, since you clearly won’t need it for a while.

Learn how to calculate safe period to avoid pregnancy using a free, simple, natural and effective method. HOW TO CALCULATE SAFE PERIOD TO AVOID PREGNANCY: FREE, SIMPLE, AND EFFECTIVE METHOD! Calculating your safe period and using it in preventing unwanted pregnancy is free. It has no side effects; it is always available and quite effective if properly calculated and applied. But many people find this very confusing and difficult to calculate. And so, today, we will be looking at a simple method of calculating one's safe period and applying it in preventing pregnancy. 1) Okay, the first thing to note is that: you should start counting from the date you started bleeding (not the day you finished the bleeding). For example if you started bleeding on the 4th, you start counting from the 4th (not the day you finished but the day you started). The 4th will be day 1 then 2, 3, 4, 5, 6, 7 and so on the day you started bleeding will be your day 1. 2) The second thing to note is: when it comes to calculating your safe period you must avoid Day-14 and days around Day-14: These days are the "dangerous days". So if you started bleeding on the 4th, that would be your day 1 then 2, 3, 4, 5, 6, 7, 8, 9, 10, 11, 12, 13, 14...then the 14th  day  -- and days around Day-14 should be avoided: these are the dangerous days because, these are the days the egg is likely to be released and the probability of getting pregnant is very high! 3) The third thing to note here is that: every other day apart from this danger period constitutes your safe period; so all these days aside Day-14 (and days around it) constitutes your safe period. As you get closer to Day-14, the likelihood of getting pregnant increases and away from Day-14, the likelihood decreases. So, the day you started bleeding is part of your safe period but as you get closer to Day-14, the likelihood of getting pregnant increases. Days around day 14...avoid, but other days are safe and down to your next period next month are safe. The danger period are the days to avoid sexual intercourse which are the days around 14th day. I will repeat it one more time: The first thing is start counting from the first day you started bleeding that would be Day 1. The next thing is avoid Day 14 and days around day 14. I hope that helps but before we leave, 2 very important points to note: 1) First this method is best for ladies with a fairly regular menstrual circle where the difference in months is just few days. You might see your period 17th this month and 18th or 19th next month or 15th or 16th next month. The difference is just few days plus or minus. We can say you have a fairly regular menstrual circle then you can apply this method of contraception. 2) The second point is this method is not advisable for ladies with very short menstrual circle say for ladies that see their menses every 3 weeks and it is also not advisable for ladies with very long menstrual circle say for ladies that see their period once in every 2 months. They contain hormones that suppress ovulation. The most effective type is the combination pill which contains both estrogen and progestin - a synthetic form of progesterone. These 2 hormones interfere with the normal menstrual cycle to prevent ovulation. The menstrual cycle refers to the monthly events that occur within a woman’s body in preparation for the possibility of pregnancy. Each month, an egg is released from an ovary in a process called ovulation. At the same time, the lining of the uterus thickens, ready for pregnancy. If fertilization does not take place, the lining of the uterus is shed in menstrual bleeding and the cycle starts over. The menstrual cycle is under control of multiple hormones secreted by the hypothalamus, pituitary gland, and ovaries. The 2 hormones that are required for ovulation are: FSH, which starts the cycle by stimulating immature follicles to grow and produce a mature egg; and LH, which is responsible for the release of the egg from the ovary - the ovulation event itself. Two other hormones, estrogen and progesterone, are at high levels after ovulation, in the second half of the cycle. They suppress FSH and LH during this time, preventing the ovaries from releasing more eggs. If fertilization occurs, estrogen and progesterone levels REMAIN HIGH throughout pregnancy, providing a continuous suppression of ovulation. On the other hand, in the absence of pregnancy, their levels FALL, causing menstrual bleeding. The levels of estrogen and progesterone in the combination pills mimic the hormonal state after ovulation, tricking the ovaries into thinking that ovulation has already occurred; FSH and LH are constantly suppressed, no egg is matured or released. The pills are taken every day for three weeks, followed by one week of placebo pills containing no hormones. During the week of placebos, estrogen and progesterone levels fall, triggering a so-called withdrawal bleeding, or fake periods. The bleeding serves as a convenient indication that fertilization did not happen, but it is not required for birth control. In fact, there exist continuous-use contraceptive pills with less or no placebos, resulting in less or no menstrual periods. These pills are particularly beneficial for women who suffer from menstrual disorders such as excessive menstrual bleeding, painful menstruation and endometriosis. Also, a woman might notice changes in her own schedule: one month her period might come earlier, the next month, it’s late. -Spotting refers to very light brown discharge that occurs between menstrual periods. It happens because of a drop in estrogen, the female sex hormone. -You might miss your period if you’re really sick, on the pill or abruptly stopped taking it, stressed, or you’ve over-exercised. It can also be because of a hormonal problem or from being over- or underweight. -Irregular bleeding can be a sign that something’s wrong hormonally. It’s definitely not normal, especially if you’re not on the pill and you’re bleeding in between periods. -Doctors recommend changing your tampon every 4-8 hours depending on your flow. -There’s always sanitary pads. But what’s even better is reusable menstrual products, like reusable cloth pads, menstrual cups, or period panties. -“Fake periods” are shorter than a regular period, less painful, and bleeding isn’t that heavy. A regular period can occur only after a woman quits taking birth control. It takes from 6 to 12 months to restore a normal menstrual cycle. -If you’re pregnant, the amount can be really small, just a couple of drops, which is why it often goes unnoticed. -It’s perfectly fine to continue playing sports or swimming during your period. -Contact bleeding occurs during contact, like sexual intercourse or a gynecological exam. If the damage isn’t severe, contact bleeding doesn’t last long. -Menstrual cramping is super common, although the degree of pain can differ from one woman to another. Cramping happens due to hormone-like substances called prostaglandins. -If you’re trying to conceive, better not do it on your period. To pill or not to pill? That is the question. Hormonal Birth Control Pills Hormonal or chemical birth control pills will be explained in this video, with the pros and cons of each type. But before I demonstrate the different types, I would like to explain to you how hormonal birth control pills work. When a lady is pregnant, ovulation (or egg release) does not occur. That’s how our body’s system normally works. The hormones in a woman’s body are balanced in a certain way that makes the hormones that stimulate the release of the eggs – called the ovulation hormones – to become inactive during pregnancy. So we thought: If we give the woman pregnancy hormones in pills, we will be able to inhibit ovulation, in other words: to stop it from happening, which, in turn, would prevent the pregnancy to occur. So the scientists decided to put the pregnancy hormone and the female hormone in a pill. This gives us an idea about the first type of birth control pills. This type consists of synthet-ic forms of hormones – the pregnancy hormone progesterone (called progestin) and the female hormone oestrogen. On the other hand there’s another type of birth control pills that consists of the pregnancy hormone only, and in a very small amount. It is called the mini Pill or the Progesterone-Only Pill (P.O.P.). The pills I will talk about are not the only hormonal birth control methods. We can take other forms of chemical or birth control at the doctor’s office, which would have the same mechanism of action. So, back to the pills. Let me explain them now. Let’s start with a combined pill. “Combined” because it has a combination of hormones. Let’s know more about it. The combined pill consists of progestin – a synthetic progester-one (the pregnancy hormone) – and oestrogen (the female hormone). You will need to take a pill daily for 21 days, starting from the 7th day of your menstruation. Even if you’re still bleeding, you must start using the pill. Take a pill daily for 21 days and stop the pill for 7 days, taking into consideration that your period cycle is 28 days. The combined pill is a good birth control choice in cases of: • You don’t mind to take hormones for birth control • You usually suffer from pains [I’m not sure about what comes here → 2:40] during menstruating, since it will ease the dysmenorrhea. • If your period is not regular, this will regulate it. • You bleed heavily during the period: it will decrease the amount of bleeding, which in turn might prevent you from becoming anaemic. • If you have acne: this might improve your skin condition. • You’re not a smoker The combined pills are not good if you … don’t wish to take any hormones … are Breastfeeding … are a heart patient … have hypertension or high blood pressure … have risk of thrombosis or atherosclerosis … have had diabetes for more than 20 years … have a liver or gall bladder disease … have a history of breast cancer in the past 5 years … are suffering from recurrent migraines or have the symptoms that proceed mi-graine, called the aura … are overweight … have been taking medications such as antibiotics, epilepsy medications or anti-depressants All these may weaken the effect of the combined pill. Always read the patient in-formation leaflets. This birth control method is 99% effective, on condition that the pill is taken on the same time every day. The other type is the mini Pill or the pill that... All rights reserved. Understanding the menstrual cycle. The menstrual cycle is a term used to describe monthly events that occur within a woman's body in preparation for the possibility of pregnancy. Each month, an egg is released from an ovary in a process called ovulation. At the same time, the lining of the uterus thickens, ready for pregnancy. If fertilization does not take place, the lining of the uterus is shed in menstrual bleeding and the cycle starts over. An ovary contains hundreds of thousands of primary oocytes - immature eggs, or ova. Each of these is enclosed in a structure called a follicle, and at this stage -- a primordial follicle. The menstrual cycle is under control of hormones secreted by the pituitary gland and the ovaries. The pituitary itself is under control of the hypothalamus. The hypothalamus produces a hormone called the gonadotropin-releasing hormone or GnRH. GnRH stimulates the anterior lobe of the pituitary to secrete follicle-stimulating hormone (FSH). FSH travels in the bloodstream to the ovaries and stimulates a group of follicles to grow. These primordial follicles develop into primary follicles and then secondary follicles. These produce a hormone named estrogen which acts to stimulate the growth of the endometrium -- the inner lining of the uterus. The secondary follicles compete with each other and only one of them will survive and become a mature follicle, the rest atrophy and die. The increasing level of estrogen also acts on the hypothalamus and the anterior pituitary to increase the level of GnRH and induce the production of another hormone -- luteinizing hormone (LH). A surge in LH secretion triggers ovulation - the release of the egg from the follicle and the ovary. The egg is then swept up by the fimbriae (FIM-bree-ee) and taken into the uterine tube. Fertilization by a spermatozoon, when it occurs, usually takes place in the ampulla, the widest section of the fallopian tube. The fertilized egg immediately begins the process of development while travelling toward the uterus. After 6 days it becomes a blastocyst and is implanted into the endometrium of the uterus. Meanwhile, the left-over of the ruptured follicle has become a corpus luteum which secretes progesterone. Progesterone further stimulates uterine development making it a nutritious bed for the embryo in the event of pregnancy. In the absence of pregnancy, the corpus luteum atrophies and progesterone level falls. This leads to the breakdown of the endometrium, menstruation begins and the cycle starts over. If I don't take birth control pills at the same time each day, will it affect my ovulation cycle?

A lot of women who are taking birth control for contraception want to now if the pill should be taken at the same time each day. And ideally, they should. Do what you can to take them around the same time each day on every day of the month. For one, it develops a habit, even on the days when you don't need to be taking a pill, it keeps you in the habit of taking it when you're taking the placebo pills. Because if you miss a pill or take it at different times of the day, there's a possibility that your body might ovulate, or release an egg, and that means that it's possible for you to get pregnant if you time intercourse with ovulation. If you skip a pill or take it at a much different time of the day than what you're used to taking it at, and you're about mid-cycle, then I do recommend using a back-up form of contraception just to be safe. Also watch for your next period to come, and if the day comes when it should start, and it doesn't, then you should take a pregnancy test just to either rule out or confirm that as the cause of your missed period. And if you are indeed pregnant, then congratulations! Don't start taking more pills, and instead, call your doctor and schedule your first prenatal appointment. If the first result is negative, but you haven't started bleeding for another 5 to 7 days, then re-test and see what that result is. It is possible to get false negatives if the pregnancy test is taken a little bit too early, and for some women, that might be even beyond the day of their period or the day after, depending on when they actually ovulated. Home pregnancy tests can pick up pregnancy about 2 weeks after conception, and for women who ovulate a little bit later, that will mean that they detect pregnancy a little bit later. Estrogen vs Progesterone Hormonal Dominance: Control Low Hormones Symptoms

Steroids are a group of hormones that regulate body functions – and they are all derived from cholesterol. Or fat. That’s right, fat. Estrogen, which is usually the focus of HRT, is actually a generic term for three different forms of hormone called Estriol, Estrodiol and Estrone - Estrodiol being the most potent. Estrogen is called the female sex hormone because it regulates the menstrual cycle and causes thing to grow during puberty – Dr Raj likes estrogen because it creates curves and it also stimulates cell growth in preparation for pregnancy. Estrogen is a growth hormone. It has received a lot of bad press because people have been taking too much estrogen in the form of synthetic hormone for a long period of time. And any hormone you take in excess is going to cause problems. And a growth hormone like estrogen if you take too much of it. You are going to get excessive cell growth which is another term for what we call cancer. So its understandable, estrogen is not good or bad, but if you take too much estrogen its going to cause problems. Progesterone is the pro gestational hormone. It’s the hormone that keeps the uterine lining intact if you get pregnant. And its depleted when you are under stress. So the typical reaction is high stress, high cortisol levels when you are under stress and the depletion of the progesterone. Its pretty amazing what progestronine helps with. It helps protect from many of the negative effects of extra estrogen. It helps women sleep. It gives them better sex drive, better energy levels, makes your skin look better. It’s a wonderful hormone. If we compare them side-by-side, you’ll see that they counterbalance each other in many ways. There’s an ideal ratio between these two hormones. Estrogen given in too large of a dose can cause depression, headaches, it increases the risk of blood clotting and stroke. It causes the loss of many nutrients such as zink and copper. It interferes with oxygen levels. Again that in a person that is getting excessive estrogen which has basically been the history of menopausal treatment. Dr Raj wants to leave you with an important point to free yourself from chronic fatigue and pain and depression and overweight. Ovarian hormones do not spontaneously malfunction by themselves. The female hormones do not all of a sudden malfunction by themselves. They are responding to stress as a stress hormone called cortisol. Every molecule of cortisol that you produce comes from progesterone. When you are under stress guess what happens? Your cortisol goes up and your progesterone drops. Dr Raj wants to test your cortisol and fix it. And he wants to test your female hormone and fix your whole hormonal system at the same time and make all your symptoms go away. Hormones Involved in Birth Control & Fertility Treatment | Biology for All | FuseSchool

In this video we are going to look at how these hormones can be used in contraception and compare them to non-hormonal methods. We are also going to look at the use of hormones in treating infertility. Female fertility is controlled by hormones, this means that knowledge of these hormones can be used to increase (fertility treatment) or decrease (contraception) the chances of fertilisation and pregnancy. FSH causes the egg to mature in the ovary, so if we can stop the production of FSH happening then we can prevent pregnancy happening. And we already know that oestrogen prevents the production of FSH, so contraceptive pills just need to contain oestrogen. Progesterone also can act as a contraceptive; it makes the mucus in the uterus very thick and so sperm cannot get through to the egg to fertilise it. Pills are either oestrogen only, combined oestrogen and progesterone or nowadays there is a mini-pill which is progesterone only. Early contraceptive pills contained much higher amounts of oestrogen. This caused quite significant side effects, such as weight gain, mood and blood pressure. More modern pills contain much less oestrogen, and some are even just progesterone. Progesterone is a lot more gentle on the body; oestrogen pills make the body think it is in a permanent state of pregnancy. Oral contraceptives are great in many ways. They give people the freedom to choose when to have a child, and when to not have children. But as mentioned above, even the more modern versions can cause side effects in people. There is also an increased risk of developing blood clots. So they are not perfect, and so people do seek out other methods of contraception instead. Inter-uterine devices (IUDs) or coils are another contraceptive option. They must be fitted by a doctor, and have spermicidal properties and prevent implantation. Or there are also contraceptive implants which work by releasing progesterone into the body and last for 3 years. Whereas the contraceptive injections also make use of progesterone, but they only last for between 8 and 13 weeks. Non-hormonal methods of contraception include condoms and other barrier methods which stop the sperm from reaching the ovum. How effective these methods of contraception are depends on whether you use it correctly. The IUDs and implants are great because they do not have human error; you cannot forget to take it or put it on. If taken correctly, the contraceptive pill is over 99% effective. But you need to take it at a similar time each day, and remember to take it daily. Other than condoms and barrier methods, all forms of contraceptives come with differing side effects and girls should consider the different options when deciding which one suits them best. So that is how we use our knowledge of hormones to prevent pregnancy. But what about helping women conceive? Some women don’t produce enough FSH to allow their eggs to mature, and so have difficulty becoming pregnant. Fertility drugs contain FSH and also LH, which stimulate the eggs to mature in the ovary. Remember that LH causes the mature egg to be released from the ovary. Because fertility drugs contain FSH and LH, this can cause multiple eggs to be released and so increases the chances of multiple conceptions; twins or even triplets. Fertility treatments do increase a woman’s chances of becoming pregnant, but it is not 100% assured and there can be other factors to consider.
