It is best to discuss exactly what your adjustments should be with your health care provider. But there are general guidelines. For insulin users, the "rule of thumb is to continue taking basal and bolus insulin," says David Dugdale, MD, FACP, professor of medicine at the University of Washington. But recognize that if you don't eat, you wouldn't take a bolus. Basal or long-acting insulin is needed throughout illness, and you may need more than usual. For ill people with type 2 diabetes, the insulin recommendations are the same as for type 1, but figuring out what to do about oral medications may actually be harder, says Dugdale. "They may take pills that are designed to offset what they eat. There is no formula for that," he says. "Some of the pills should be adjusted if [people] are sick enough that they can't eat." For example, sulfonylurea use may need to be reduced because the medication can increase the risk of hypoglycemia. People with type 2 diabetes using metformin may need to switch to insulin during a serious illness, such as gastrointestinal flu, because dehydration and the decreased kidney function that may result increase the risk for a rare complication, lactic acidosis, associated with metformin use. Dugdale suggests asking your doctor if you should take a break from amylin (Symlin) while ill. "It just adds another set of variables that might make it harder to do the right thing," he says. Mind the Ketones One of the most serious risks of sickness in people with diabetes is diabetic ketoacidosis (DKA), a life-threatening condition caused by too little insulin in the body in combination with the stress hormones associated with illness. A cup of fluid every hour should be enough, but keep in mind that vomiting and diarrhea increase the risk for dehydration. Dugdale recommends carbohydrate-free fluids, such as water, to help keep blood glucose levels from rising too high. "I've seen people with DKA who kept drinking sugary fluids without recognizing how much sugar they were taking, which just aggravated their blood sugar," he says. "If they take fluids with sugar, they need to account for that some way." For people who are sick to their stomach and not eating, Dugdale says this rule can be bent in order to take in some nourishment. In that situation, Gandica recommends alternating between sugar-free and sugary fluids or, if drinking is making you nauseous, between sugar-free and sugary Popsicles. By Jamie - Age 14 | For seven years of my life, I grew up in the foster system. I had to leave my mom’s house because of the decision to stop being there for her children. At the time, it was just me, an eight year old girl, and my brother, and an eleven year old severely autistic boy. One day the police showed up at our house. They asked me a lot of questions and then told us I would be staying at my aunt's house with her three kids, and my brother to a home for his needs. I stayed with my Aunt and cousins for a year and a half, until one day my Aunt told me to pack some clothes. I thought I was going to spend some time with my other Aunt and cousins, so happily I went and packed some clothes but, once I was done I went to the front room and my Aunt told me and my cousins I was going away for some time. I didn’t know what that meant but, we all cried, and my ??? showed up. My ??? didn’t tell me too much on our car ride there but, for most of the ride I cried. Once we got there, I was greeted by someone I would soon look up to as my sister, and by more people I would soon consider my family. I lived there for 2 and a half years. I wanted that to be my forever home, my forever family. But I found out my biological mom had another baby. He was only 5 months old and the state wanted us to be together forever. I moved in with him and his foster family. ??? threw up, cried, fussed, changed his diapers. I never thought I’d have a little brother, but there he was right before my eyes. I thought that was it for my foster placements but, I was wrong. Something didn’t work in that home and, we were moved after a year and a half of staying there. Now, we were with our very first white foster parent. A single mother, with three cute, but crazy kids. To say the least on that home, it didn’t work and, we left. On my last day of school, we were taken to our next foster home and, our last one thank you jesus. Another white family with four kids. A boy in the eleventh grade, a girl ???, another girl in the seventh grade, and another boy in like the fifth grade. And of course two parents, a mom and a dad and two lovely black dogs. Those people showed me and my brother so much love and care even after our hearts were broken by so many people at so many different times. I’ve never had a father figure in my life, so when I got to this one I didn’t really know how it felt to have a father at all. December 31, 2014, we were adopted by these people. Being here for two years now, I’ve received lots of love, and crazy rules and groundings. I’m blessed to have a place to finally call home Do you LOVE junk food? It was a diagnosis she did not expect nor a disease she wanted. Her Mom was concerned, but Jamie was just totally confused - she had to learn all about it fast, AND, she had to take medicine - insulin - every single day. So, with a serious fear of needles and shots, Jamie found out that she had to give herself the shots, and monitor and test her blood sugar levels on her own. It was scary, and people would look at her when she had to do it and make her feel different and uncomfortable. But the hardest part of all was that she had to stop eating all of the junk food and sugar and start eating healthy foods. As a diabetic, She had to totally change her diet. She even got used to the shots and is really living a normal life - but she does have to take care of herself a little more carefully than most kids her age and manage her diabetes. If you have certain medical or long term health conditions you may be at increased risk of complications from the flu. This may mean you are in, what is commonly known as, an ‘at risk’ group. It is strongly recommended that people in an ‘at risk’ group, go for their flu vaccination every year. People who have been diagnosed with diabetes (both type 1 and type 2), and who need to take insulin, oral medication or modify their diet to control their blood sugar, are advised to get their flu vaccination every year. Flu vaccination can help prevent you from getting the flu. This is why flu vaccination is recommended for people with diabetes. Having high blood sugar weakens the immune system and increases the risk of getting infections. Flu infection can be more common in people with diabetes than people who do not have diabetes. The flu can also be more severe in people with diabetes. Studies show that people with diabetes who get the flu but don’t get the vaccination are more likely to have complications of flu that sometimes lead to hospital admission, and in rarer cases, fatality. Flu vaccination can help prevent these complications. Therefore vaccination is strongly recommended for ‘at risk’ groups, like patients with diabetes, to help protect them against flu or from developing serious complications of flu. Then take an ambulance trip to the PICU (pediatric intensive care unit) at Memorial Hospital in Colorado Springs and a visit to the Barbara Davis Childhood Diabetes Center.
