This weeks video is a short guide on warfarin also known as (coumadin). I see many patients on warfarin everyday and they are very knowledgable about their medicine, which is fantastic! But here's some information about warfarin, how it works, international normalised ratio (INR) and interactions which you may find useful. WHAT IS WARFARIN: Warfarin is the main oral anticoagulant used in the UK. An anticoagulant is a medicine that prevents blood clotting. Clotting (thickening) is a complex process involving a number of substances called clotting factors. Clotting factors are produced by the liver and help control bleeding. They work with cells that trigger the clotting process (platelets) to ensure blood clots effectively. Warfarin blocks one of the enzymes (proteins) that uses vitamin K to produce clotting factors. This disrupts the clotting process, making it take longer for the blood to clot. WHEN IS WARFARIN PRESCRIBED: Anticoagulant medicines, such as warfarin, are often prescribed for people who've had a condition caused by a blood clot or have an increased risk of developing harmful blood clots. TAKING WARFARIN: It's very important that you take warfarin exactly as directed. Don't increase your prescribed dose unless the doctor in charge of your care advises you to. Warfarin is taken once a day, usually in the evening. It's important to take your dose at the same time each day, before, during or after a meal. The aim of warfarin therapy is to decrease the blood's tendency to clot, but not stop it clotting completely. This means the dose of warfarin you're taking must be carefully monitored and, if necessary, adjusted. You'll have regular blood tests at your GP surgery or local anticoagulant clinic to make sure your dose is correct. The INR is a measure of how long it takes your blood to clot. When you start taking warfarin, you may be given a yellow booklet about anticoagulants, which explains your treatment. INTERACTIONS WITH WARFARIN Medicines Warfarin can interact with many other medicines, herbal medicines and supplements. Vitamin K is known as the “blood-clotting vitamin” for its important role in healing wounds. It is also an important factor in bone health. According to the Harvard School of Public Health, vitamin K helps produce four of the 13 proteins needed for blood clotting. Recent research is showing that eating more foods rich in vitamin K makes the heart healthier. Scientists have found another reason for children to eat their green leafy vegetables. A study consisting of healthy adolescents showed that those who consumed the least vitamin K, were at 3.3 times greater risk for an unhealthy enlargement of the major pumping chamber of their heart. Only 25 percent of the teens in the study met current adequate intake levels of Vitamin K. The recommended intake for vitamin K for young adults is 75 micrograms. In those who consumed the least vitamin K1, the study found the overall size and wall thickness of the left ventricle were already significantly greater and the amount of blood the heart pumped out significantly lower. The scientists believe theirs is the first study exploring associations between vitamin K and heart structure and function in young people. While more work is needed, their findings suggest that early interventions to ensure young people are getting adequate vitamin K1 could improve cardiovascular development and reduce future disease risk. Vitamin K is a fat-soluble vitamin that consists of both, vitamin K1 (phylloquinone ) and vitamin K2 (menaquinones). Vitamin K1 is primarily found in leafy green vegetables because it is directly involved in photosynthesis. It is also the main dietary form of vitamin K. On the other hand, Vitamin K2 is mainly of bacteria origin and mostly found in different animal-based and fermented foods. Foods rich in Vitamin K1 include leafy greens, such as Swiss chard, kale, parsley and spinach, broccoli and cauliflower, Brussels sprouts, dried basil, paprika and cayenne powder, soybeans, olive oil and prunes. Fermented dairy, including yogurt, cheeses, and fermented soy including miso, and organic eggs provide K2, which is especially helpful in increasing bone density and reducing the risk of fractures. Those with osteoporosis should consider supplementing 50 to 100 micrograms of VitaminK2. Vitamin K also works with vitamin D to ensure that calcium finds its way to the bones to help them develop properly. In my last article, I talked about ways to prevent diabetes, especially type two diabetes. Now I want to talk about home remedies diabetics can take along with their medications to keep their blood sugar under control. Again, Watch Your Diet In my last article, I told you about foods you can eat to prevent diabetes if you don't already have it and are at great risk of developing it later in life. Those foods are also for those who have diabetes and need to keep it under control. However, coffee and chocolate may not be good for diabetics because too much of them can de-stabilize blood sugar, and for most diabetics, coffee and chocolate should be kept to a minimum or be avoided altogether. Cinnamon Including ¼ to ½ teaspoon of cinnamon added to your food or drink can help lower blood sugar if the rest of your diet is sensible. Cinnamon can lower blood sugar. My mom uses it in her food and even takes a cinnamon capsule and it has helped lower her blood sugar. Be aware of what type of cinnamon to use. Cinnamon from the spice rack is the best way to go. Some grocery store cinnamon contains a compound called coumarone and can be toxic to the liver, if used regularly. It can also interact with the blood thinning medicine, Coumadin. Cinnamon in the form of a capsule can be safer than regular Cinnamon but no matter what kind you try, make sure to watch for toxicity. Cinnamon capsules such as Cinnulin PF can be safely taken or people can put Cinnamon in a coffee filter and pour hot water over it to create their own water extract. Vinegar Yet another value of vinegar is reducing blood sugar and insulin levels. Researchers have reported that when vinegar is given with white bread it can lower blood sugar and helps people feel fuller for up to 2 hours. Also, note that it is very important for children and pregnant women not to take bitter melon because it can be dangerous to them. It has also been used as a medicine to treat symptoms that indicate diabetes. The side effects of fenugreek to watch out for are diarrhea, flatulence, and allergic reactions. It has also been said to interact with Coumadin and should be avoided by people who take it. Careful monitoring is necessary and the recommended dose is one or 2 grams of seeds, 3 times a day but also can be taken as a tea. Nopal is a cactus found in Mexico. Nopal can be made into a tea called prickly pear tea and has shown to lower blood sugar. Close monitoring and medical supervision are advised. Stevia is a non-sugar sweetener derived from the leaves of a South American shrub. It has not been approved as a sugar substitute in the United States but is often used in Japan. This plant can not only lower blood sugar, but also blood pressure. Easing Stress If you are a diabetic you need to watch your stress levels. In this day and age, it is rather hard to do this but it is important for diabetics because stress can raise your blood sugar and make your condition harder to control. Try the following to ease stress. Listen to relaxation tapes. The authors of the book, Best Choices from the People's Pharmacy, suggest trying a relaxation tape by Dr. Emmett Miller. Sometimes her hands and feet were cold and clammy. Other times she had restless legs or pain and heat - especially at night. Today I’m going to continue about this important topic because when you have poor blood flow and circulation, you’re in danger of suffering from numerous health problems and disease, such as: Heart Disease – stroke or heart attack, blood clots High Blood Pressure and kidney damage Diabetes – numbness in your extremities (hands and feet), amputation Mental & Memory – slow thinking, poor memory, Alzheimer’s, depression Muscle Loss – lack of nutrients to the muscles, weakness, fatigue as you get older Sexual Problems – erectile dysfunction affects most men over the age of 40 Accelerated Aging – skin, hair, poor eyesight, varicose veins, etc. Poor Sleep, insomnia, sleep apnea, etc. All of this is because your blood carries oxygen and nutrients to every organ and cell in your body. When there are restrictions, such as the narrowing of the arteries, plaque build-up, thick blood, and so forth… your body is not functioning optimally and nutrients aren’t being delivered to your cells, muscles, heart or brain. The Cause As stated in the previous video, aging makes blood flow worse and this is because of the negative changes in your hormones and neurotransmitters, due to aging. However, additional causes of poor blood flow and circulation7, which exacerbates this situation are Smoking Diabetes Obesity Poor diet Very little or improper exercise Stress Management Not enough sleep or rest Lack of key nutrients, vitamins and minerals The good news is that all of these causes are fixable and are under YOUR control. And that’s what I’m going to quickly discuss next… All Natural Solutions So there are lots of natural ways you can improve poor blood flow and circulation. You don’t have to take harmful drugs like Plavix or Coumadin or even Aspirin. Stop Smoking I think by now everyone knows how bad smoking is because it increases cholesterol, hardens the arteries and causes blood vessels to constrict. So if you smoke, make a decision and commit to STOPPING once and for all! Exercise Exercising daily is vital. Cardiovascular “aerobic” exercise is very important – jogging, running, swimming, biking are all great. Of course, daily brisk walking can be done by anyone, so you have no excuse not to get outside and take a 20-30 min walk daily or at least 3x weekly. But most people don’t know or realize that “anaerobic” muscle-building exercise, is just as important and maybe even MORE important than anaerobic. The more muscle you have, the better your blood flow, the bigger your arteries, the more flexible your blood vessels… Which all translates into greatly improve circulation from head to toe. Additionally, “resistance exercise” improves your hormones. And you don’t need much. Just 20-30 minutes, 2-3x weekly using cables or dumbbells at home is great. Even your own body weight is a fantastic start. Diet So the first thing to do is reduce or eliminate animal protein and animal fat. You should also reduce your sugar intake because high insulin levels corrodes your arteries and blood vessels. You should focus on eating more natural whole grains, sweet potatoes, fruits, vegetables, legumes, fish and nuts. And don’t forget the water… You should also eliminate all caffeine beverages because caffeine constricts your blood vessels and also increases the stress hormone, cortisol. Stress Management Speaking of the stress hormone, Cortisol … Even though we can’t stop or eliminate stress, you can certainly learn to manage how you REACT to stress, which is very important. Thank you for watching **Useful Information** * From providing a useful information channel. Are Fish Oil Capsules and Fish Oil Supplements Safe Are fish oil supplements safe? Do they contain heavy metals and other toxins? I’m afraid your concern is justified. Despite assurances from manufacturers, some fish oil supplements probably do contain toxins. My colleague Kathleen Johnson, a nutritionist here at the Arizona Center for Integrative Medicine, tells me that she’s not sure if heavy metals as well as pesticides and PCBs are a problem. However, she notes that results from a study published in 1998 indicate that tests of 44 different fish oil supplement brands sold in 15 countries between 1994 and 1995 revealed appreciable quantities of organochlorines including PCBs and pesticide residues. So far, the only reports of potential health consequences of contaminated fish oil supplements indicate that they may increase the risk of breast cancer, but this link isn’t well established. We all need the omega-3 fatty acids in fish oil for long-term health and to reduce our risks of heart disease, cancer, decrease inflammation and enhance mood. If you’re healthy, you’re better off getting omega-3s not by a fish oil supplement but by eating at least three servings per week of oily fish (wild Alaskan salmon, mackerel, sardines, herring). Avoid carnivorous fish such as swordfish and marlin, which spend a lot of time in coastal waters where pollution levels are highest; bluefish, which tend to concentrate mercury; and albacore tuna, which may harbor the same pollutants as swordfish and marlin. However, some people may benefit from taking fish oil capsules in addition to eating fish. Ms. Johnson recommends them to those with autoimmune or inflammatory disorders, heart disease, diabetes, mental or emotional disorders, or insulin resistance. Because they can affect blood clotting, avoid fish oil supplements if you’re taking any anticoagulant drugs like Coumadin (warfarin), have had a hemorrhagic stroke, or are scheduled for surgery. While omega-3s are beneficial in almost any dose for these conditions, if you are not getting any omega-3’s from your diet, you may have difficulty getting adequate levels of EPA (eicosapentaenoic acid) and DHA (docoahexaenoic acid) from fish oil capsules alone. Choose a brand that provides high levels of both EPA AND DHA in the least number of capsules and are distilled to remove PCBs and other contaminants.
