Serotonin, Noradrenaline Withdrawal Venlafaxine hydrochloride (Effexor, Dobupal) is a phenylethylamine-derivative antidepressant and anxiolytic agent that acts as a serotonin- and noradrenaline-reuptake inhibitor (SNRI). It is used primarily in major depressive disorder, with labeled uses including generalized anxiety disorder and social phobia. Nonlabeled uses include depressive symptom remission, obsessive-compulsive disorder, and chronic pain syndromes. Most selective serotonin-reuptake inhibitors (SSRI) and SSNRIs are reported to cause serious adverse effects in approximately 5% of patients, according to its manufacturer.[1] Venlafaxine is no exception. Among the adverse effects are a number of withdrawal symptoms that form "discontinuance syndromes," sometimes mistakenly identified with what Sternbach in 1991 proposed as the "serotonin syndrome".[2] Venlafaxine's half-life is only 4 hours. Its primary metabolite, O-desmethylvenlafaxine, has a half-life of 10 hours. In the past 10 years, a number of clinical reports of severe venlafaxine withdrawal symptoms have been published, and for the most part these effects are duly reflected in generally available information. Widely consulted drug information services, such as Medscape DrugInfo, American Hospital Formulary Service Drug Information, and First DataBank, list the following withdrawal symptoms as "serious": agitation, anorexia, anxiety, confusion, impaired coordination, diarrhea, dizziness, dry mouth, dysphoric mood, fasciculation, fatigue, headaches, hypomania, insomnia, nausea, nervousness, nightmares, sensory disturbances (including shock-like electrical sensations), somnolence, sweating, tremor, vertigo, and vomiting. Recent Case Histories: Withdrawal Effects Are Not Dose-Dependent Case 1: Standard Dose. Female, white, age 35, no other pathologies, diagnosed for major depressive disorder with anger attacks. Treated with extended-release venlafaxine 150 mg for 4 months with concurrent psychotherapy, with positive results: Beck's Depression Inventory score reduced from 38 to 14; functionality restored. The patient ran out of medication on a Thursday and could not get in touch with her doctor until the next Monday. On Friday, only 12 hours after the usual time of taking the medication, she felt a noticeable change in mood, dizziness, and nausea. She experienced a sensation in her head as if electrical discharges "popped" and could not concentrate to the extent that she was unable to drive her car back from where she had gone. By Friday evening (18 hours after cessation), the "popping" in her head had become worse and she vomited after dinner. Depression symptoms reappeared acutely, and she could not stop crying. At first, she did not equate these symptoms to the missed dose of venlafaxine, but thought she might have contracted the flu. However, the symptoms continued and intensified over the weekend and only reduced when, on Monday, she started again on venlafaxine. She was inadvertently given the lower dose of 75 mg, which did not quite restore her mood to the previous level, although the "electrical popping" sensation in her head stopped and the nausea was reduced. It appears the most effective antidepressants (at a duration of 8 weeks) include amitriptyline, escitalopram, mirtazapine, paroxetine, and venlafaxine. While reboxetine and trazodone had notably lower efficacy ratings. Some of the most effective drugs had lower tolerability scores, however, indicating they may have a greater side effect burden. Antidepressants Make it Harder to Empathize, Harder to Climax, and Harder to Cry.

And when you start to push on the doses of these SSRIs you start to lose some sort of quintessential feminine things. First of all it becomes much hard to climax and it becomes much, much harder to cry. But you also see decreases in empathy, in sensitivity, in passion. The simple way of thinking about an SSRI is that you have two brain cells and one is a pitcher and one is a catcher. So pitch, catch. Pitch, catch. So this nerve cell is throwing serotonin across and this one is catching it. What the medicines do is they block the recycling back into the pitcher. So, you know, I’m throwing, I’m throwing, some of this gets caught, some of it gets dropped. It just doesn’t get over there but I’ll suck it back in and try again. So if you block the recycling more is in the middle to get across. So there’s more, you know, the space between the nerve cells is called the synapse. If you block the recycling of the serotonin into the releasing cell more is available for the catching cell. So it ends up enhancing the transmission. How enhanced serotonergic transmission translates into feeling better and feeling less anxious is much more complicated. But, you know, the simplistic way to think about it is that if you have higher levels of serotonin, if your transmission is better you will be more relaxed and more happy. It’s a little easier to smile. It’s a little harder to cry. So, you know, I’ve had patients come to me and say, you know, I’ve tried antidepressants before but they always made me feel like a zombie or they didn’t make me feel like myself. Or I had a patient who said like I cut my finger and I looked down and I saw that it was bleeding and I saw that it was my blood but I didn’t really feel like connected to my finger or the blood. You know, things like that that are really, really worrisome. Or I’ve had patients say, you know, I was in this situation where I knew I should be crying and I couldn’t cry. And, you know, I felt terrible that I couldn’t express that emotion to bond with my friend or something like that. So these antidepressants do scale back a lot of expression of emotion and feeling emotion even sort of thinking emotional thoughts. If you’re terribly depressed and you need antidepressants to get out of bed and function and go to work I get it. That’s one thing. But what I’m worried about is more and more women deciding to go on antidepressants because their friends are doing it and that’s what’s, you know, more and more women who are at work are taking these SSRIs so that they cannot cry, not get flustered, keep going forward. You know I think it jives with this sort of forward momentum agenda that so many of us have and especially in the workplace. But, you know, I would say at what cost? You know it is true that SSRIs can help you get ahead and there have been really interesting animal studies where, you know, the primates who are on SSRIs ascended up the dominance hierarchy. And the ones who became dominated over got stressed out and had lower serotonin levels. So there does seem to be some component of serotonin affecting dominance hierarchies and, you know, the ability to move ahead or to lean in. So I totally get that there are advantages to being on an SSRI in the workplace. But, you’re going to miss out on knowing what’s right because you feel it or being hurt by what somebody said and showing them that you’re hurt. And so that person can learn that their behavior has emotional consequences for other people. So and it changes the whole sort of tone of the workplace. There’s going to be less accountability and less sort of calling people on their misbehavior if you’re not even feeling that anyone misbehaved. [TRANSCRIPT TRUNCATED]

Withdrawing from Effexor XR cold turkey
I felt compelled to do a video on Effexor XR withdrawal because I honestly never knew that this was the type of medication I was taking. Is that my fault? Probably, but it's a lesson learned. Please know this is my story and view on things, all this doesn't mean you have the same as me, but if this video can help someone out, than that's ok with me. How I recovered from bpd: 1. Hospital 2. Medication -fluoxetine -sertraline -venlafaxine -vortioxetine -propranolol -diazepam -pregabalin -melatonin 3. Therapy (dbt, support worker, cbt w the right psychiatrist) & using my coping techniques they set me, trying to do my homework 4. Acceptance- having BPD is fine, accept that your moods will change a lot, just go with it. Explain to the people around you that this is typical behaviour for someone with BPD. 5. Support from family & friends (& online peeps) 6. Taking things one step at a time 7. If I had a relapse I wouldn’t think too much into it or let it get to me bc it’s all about how you get back up afterwards I don’t think you ever fully recover from mental illness I feel like you just learn to manage and live along side it. Hopefully I will get to a point where the symptoms are just so quiet & I no longer meet the criteria for bpd. Tapering off Effexor, my method
Update: after eight months of slow progress, I'm down to 16% of my initial dose. I've had some inevitable discomforts but all in all, manageable. This method works. Still a few months to go and I will finally be done!!! Other updates ... Down to 14% of my initial dose, and almost a year later! Will have to cut one bead at a time from now on, and super slow, so basically might have another year to go. The smaller the dosage, the more hard core. I currently have vertigo episodes and intense teeth and jaw pains. All the other stuff too : cramps, diarrhea, muscle stiffness. Had a few downright scary depressive moments and no control over myself. I no longer use the straw method, at 14 beads I simply throw them in by hand. I've purchased empty capsules and simply use the hundreds of leftover beads.

Side effects of taking medication are written everywhere, with warning signs, etc but many do not talk about the WITHDRAWAL effects of some of these medications. These withdrawal effects can be shocking, dangerous, and at times life threatening. Here is my experience with Effexor XR withdrawal, as well as Ativan, and other meds I have tried. I have found that the longer I am on a medication, as well as higher the dosage, the worse the withdrawal symptoms. Anti Depressant Medication - Seretonin Syndrome I Bipolar Barbie
Serotonin is a chemical produced by the body that enables brain cells and other nervous system cells to communicate with one another. Too little serotonin in the brain is thought to play a role in depression. Too much, however, can lead to excessive nerve cell activity, causing a potentially deadly collection of symptoms known as serotonin syndrome. Serotonin syndrome symptoms often begin within hours of taking a new medication that affects serotonin levels or excessively increasing the dose of one you are already taking. Symptoms may include: Confusion Agitation or restlessness Dilated pupils Headache Changes in blood pressure and/or temperature Nausea and/or vomiting Diarrhea Rapid heart rate Tremor Loss of muscle coordination or twitching muscles Shivering and goose bumps Heavy sweating In severe cases, serotonin syndrome can be life threatening. If you experience any of these symptoms, you or someone with you should seek medical attention immediately: High fever Seizures Irregular heartbeat Unconsciousness Serotonin syndrome can occur if you are taking medications, particularly antidepressants that affect the body's level of serotonin. The greatest risk of serotonin syndrome occurs if you are taking two or more drugs and/or supplements together that influence serotonin. The condition is more likely to occur when you first start a medicine or increase the dose. The most commonly prescribed class of antidepressants, which work by increasing serotonin, are the serotonin reuptake inhibitors (SSRIs).These include citalopram (Celexa), escitalopram (Lexapro), fluoxetine (Prozac), fluvoxamine (Luvox), paroxetine (Paxil), and sertraline (Zoloft). Other prescription and over-the-counter drugs that can raise serotonin levels alone or in combination to cause serotonin syndrome include: Serotonin and norepinephrine reuptake inhibitors (SNRIs), a class of antidepressants including desvenlafaxine (Khedezla), desvenlafaxine succinate (Pristiq), duloxetine (Cymbalta), levomilnacipran (Fetzima), and venlafaxine (Effexor). Monoamine oxidase inhibitors (MAOIs), a class of antidepressants including isocarboxazid (Marplan), phenelzine (Nardil), tranylcypromine (Parnate), and transdermal selegiline (EMSAM) Buspirone (BuSpar), a drug used to treat anxiety disorders Desyrel ( Trazodone ), a drug prescribed for depression or insomnia Migraine treatments such as almotriptan (Axert), Amerge (naratriptan), rizatriptan (Maxalt), sumatriptan (Imitrex), and zolmitriptan (Zomig) Certain pain medications, including fentanyl (Sublimaze, Fentora), fentanyl citrate (Actiq), meperidine (Demerol), pentazocine (Talwin), and tramadol (Ultram) Dextromethorphan , a cough suppressant found in many over-the-counter and prescription cough medicines or cold medicines Certain medications prescribed for nausea, such as granisetron (Kytril), metoclopramide (Reglan), and ondansetron (Zofran) Antidepressants that affect multiple serotonin receptors, such as vortioxetine (Trintellix -formerly Brintellix) and vilazodone (Viibryd) Some illegal drugs, such as LSD and cocaine, and dietary supplements, including St. John's wort and ginseng, can also lead to serotonin syndrome when combined with antidepressants that affect serotonin. The FDA recently asked drug manufacturers to include warning labels on their products to let patients know about the potential risk of serotonin syndrome. If you are uncertain about drugs you take or have been prescribed, check the label or speak with your doctor. Don't stop any medication before talking to your doctor. Serotonin Syndrome Diagnosis There is no single test to diagnose serotonin syndrome. Your health care provider will ask about your medical history, including medication, supplement, and recreational drug use, and perform a physical exam. Other conditions may cause symptoms that are similar to serotonin syndrome. Lab tests to exclude other causes of symptoms may be ordered. Serotonin Syndrome Treatments People with serotonin syndrome are typically hospitalized for observation and treatment of symptoms. For example, benzodiazepines are given to treat agitation and/or seizures. Intravenous fluids are given to maintain hydration. Removing the drug responsible for the serotonin syndrome is critical. Hydration by intravenous (IV) fluids) is also common. In severe cases, a medication called cyproheptadine (Periactin) that blocks serotonin production may be used. It blocks some adrenergic auto and heteroreceptors, increasing serotonin release, and selectively antagonizes serotonin receptors in the central and peripheral nervous system. It also enhances serotonin neurotransmission and blocks the histaminergic and muscarinic receptors. Side Effects: Constipation, dry mouth, weight gain, increase appetite, somnolence, sedation, sleepiness, weakness, distrurbance in thinking, peripheral edema, increased blood pressure. When Discontinuing Use: Mirtazapine and other antidepressants may cause a discontinuation syndrome upon cessation. A GRADUAL and SLOW reduction in dose is recommended. Effects of sudden cessation of treatment with mirtazapine may include depression, anxiety, panic attacks, vertigo, restlessness, irritability, decreased appetite, insomnia, diarrhea, nausea, vomiting, flu-like symptoms such as allergies and pruritus, headaches and sometimes hypomania or mania. It can go so far as to cause suicidal thoughts, so please, be careful!! AKA not good!!! PLEASE always been under the care of a great doctor and NEVER stop taking a drug suddenly without guidance from your doctor. Starting a New Medication (Venlafaxine XL 75GMG) So a subject that often comes up is what medication am I on, what have I tried so after going to speak to my GP about medication I decided to film a little vlogs talking a bit about the medication I'm on and what I've been on and briefly what my experience's have been like. While I'm starting back on anti-depressants don't get the wrong Idea I still feel amazing, the main reason I'm starting on it now is because I'm at a level of stability that I want to maintain.
