Researchers at Johns Hopkins found that high doses of cocaine can cause autophagy to go into overdrive and destroy important cell components such as energy-producing mitochondria, causing autophagy-induced cell death. Prasun Guha, a postdoctoral fellow at Johns Hopkins University School of Medicine and lead author of the paper, which was published in the journal Proceedings of the National Academy of Sciences, likened the process to how waste is removed from the house. "A cell is like a household that is constantly generating trash," Guha explained in a press release. "Autophagy is the housekeeper that takes out the trash — it's usually a good thing. But cocaine makes the housekeeper throw away really important things, like mitochondria, which produce energy for the cell." Signs of autophagy-induced cell death were found in the brains of mice given high doses of cocaine, as well as in the brains of mice whose mothers received the drug while pregnant. The scientists also tested an experimental drug called CGP3466B, which they protected mouse nerve cells from autophagy-induced death caused by cocaine. And the radical new procedure appears to avoid the usual side effects that make most chemotherapies difficult to tolerate. The wafer is implanted during the course of necessary surgery. Patients like Nicholas Tirone, who had a life-threatening brain tumour five years ago, turned to the method as a last hope. The drug is released in the body to home in on the tumour target during the two to three weeks in which the wafer degrades, said Dr Henry Brem, principal investigator for the Polymer Brain Tumour Treatment Group. In a study of 222 patients with recurrent glioma -- a brain tumour -- the median survival rate of 110 patients who received the anti-cancer agent was 31 weeks, compared with 23 weeks for others. All patients had end-stage disease and had failed conventional and experimental therapy. What was significant was that the improvement occurred without nauseating side effects. Three years after his surgery, Nicholas Tirone's prognosis is still guarded. But he leads a full life, working 50 hours a week and is very active. Researchers continue to work with newer polymers and drugs but the process used in Tirone's case is expected to be widely available in clinical trials within months.
