Original Medicare is the Medicare parts A & B and D that we have detailed in previous videos. In this video we will talk about Medicare Advantage plans. In our next video of this series we will discuss Medicare Supplement or Medigap plans. You are only allowed a Medicare Supplement plan if you keep your Original Medicare. You cannot have a Medicare Supplement or Medigap plan and a Medicare Advantage plan at the same time. So, let’s talk about Medicare Advantage plans. 1. Medicare Advantage plans replace your Original Medicare. They are not supplements. They do not supplement or enhance your Medicare coverage, they replace it. (show book) When you choose a Medicare Advantage plan the benefits and appeals processes outlined in this guide are no longer relevant. You can throw this book away. Your benefits will be detailed in an “Evidence of Coverage” provided by the insurance company you choose to replace your Medicare, along with their “Annual Notice of Change”. You no longer need to show your doctor your red, white and blue Medicare card or Medicare number, you will have a new card provided to you by the insurance company. You can no longer appeal your benefits to Medicare. Medicare is out of the picture as far as your personal service in concerned. If you decide to appeal an insurance decision, you must appeal to the insurance company. 2. Medicare Advantage plans are typically HMOs or PPOs and on occasion a Private-Fee-for-Service plan. HMO stands for Health Maintenance Organization. PPO stands for Preferred Provider Organization. We will talk more about both of those in a minute. What is important to understand is that Medicare Advantage plans are not standardized, they are regulated to meet a certain minimum coverage. That is an important concept, so let me explain further. Medicare Part A & B is standardized. Medicare Supplement plans are standardized. Everyone in those plans receives the exact same benefits, they can see the exact same doctors and should have the exact same experience. With Medicare Advantage plans, each plan has different benefits, different co-pays, and deductibles, covers different medical services and has a different network of doctors and hospitals. Everyone’s experience is different and depends on which plan they have chosen. In additional; all the benefits, co-pays, deductibles and the doctors and hospitals in the network can and will change each and every year. It will be your responsibility to keep up with the changes via their mandatory Annual Notice of Change. It’s your responsibility to study this annual notice and determine how the changes may impact your healthcare. Medicare Advantage plans are regulated to meet a certain minimum standard. That minimum standard dictates that they must be actuarially equivalent to Medicare Parts A and Part B. That’s an interesting term “actuarially equivalent”. What does that mean? This type of plan offers a higher level of reimbursements when you receive treatment from a “preferred” doctor or hospital. In less fancy insurance terms, you pay less. This is also known as your health insurance company’s network. These “preferred” providers offer their services to you at a cheaper rate if they’re in your insurance company’s network. You even have the ability to see non “prefered” doctors with this plan, they are just consider out-of-network doctors. PPO’s have dominated the health insurance landscape for the last 10-15 years. What Does HMO Mean? HMO stands for Health Maintenance Organization. This plan type works a little different from a PPO. Instead of being able to choose from a list of doctors at a wide array of facilities, you are strictly limited to doctors and facilities contracted with that health insurance company. The goal of this plan structure is to have a primary physician manage your care. The most famous example of an HMO structure is the company HealthSpan, formerly known as Kaiser Permanente. These plans generally are less expensive than a PPO plan, however that is in exchange for a limited selection of doctors and hospitals. Let’s Take a Closer Look So I’ll ask you again, how many doctors do you want to be able to see if something really bad happens? The answer to that question is the answer to your internal PPO vs. HMO debate. But if you want to go even deeper, here’s what you need to think about. When does a HMO plan Make Sense? This is a hard question to answer since it will always come down to personal preference. But, this is how I like to explain it to clients. If there is a HMO plan that has doctors close to your home that you like and trust and also has an adequate number of hospital facilities, that’s a decent first step. You also need to be able to save a considerable amount of money on your health insurance payment every month. Finding both of those together is the tricky part. I have yet to see a HMO plan that offers enough savings to consider recommending one to a client. When you only save a couple dollars every month, you have to ask yourself if it is really worth it to limit yourself that severely when it matters the most. I've never been able to answer yes to that question for a client.

In this video I talk about the difference between Medigap plans and Medicare advantage plans. Medicare supplement plans vs. Medicare advantage plans can be a confusing comparison because you are really talking about oranges and apples. Medicare supplement plans are always labeled with a letter like Medicare supplemental plan F or Medicare supplement plan G. The Medicare advantage plans will not have a letter label unless someone points out they are also called Medicare Part C. Medicare supplements vs. Medicare advantage plans are easy to compare because of the upfront payment or out of pocket costs when you use them. A Medicare supplemental plan F doesn't have any out of pocket costs and a Medicare advantage has all the out of pocket costs. The Medicare supplemental plan F might have a premium every month of $200 but doesn't have a co pay or deductible cost. The Medicare advantage plan might have a "0" premium but has an out of pocket cost of $6700 a year. Medigap plans are always known to be better coverage with no networks and the Medicare advantage plans are always cheaper. When you speak to AARP or Humana ask them if it is a Medigap plan F you are looking at or a Medicare advantage plan HMO or PPO. This is for two reasons – first; because the previously popular plans Medicare supplement Plan F and Plan C are being phased out as of January 2020. Second, Medicare supplement Plan G is usually a lower premium AND a better value. Most people shopping for a Medigap Plan today find Medicare supplement Plan G as their best value. Let’s take a quick look at the Medicare supplement plans so you can see why: . In the column below the Plan letter you can see the percentage of insurance coverage offered by that plan. For example; Medigap Plan F show 100% coverage in each category all the way down. That means that Medicare supplement plan F covers 100% of all the co-pays and deductibles that you would normally have to pay with Medicare Part A (inpatient hospital coverage) and Medicare Part B (outpatient coverage). It’s a great plan and would be an easy decision for many people entering Medicare, except that it is being phased out in January 2020 and Plan G usually offers equal benefits for less money. Take a look -- Medicare supplement Plan G has all the same benefits except this row right here: Medicare Part B deductible. That is a calendar year annual deductible. It resets every January 01. It can change every year. In 2017 it’s just $183. We expect it to increase to $250 over the next five to seven years. That’s it. All you pay is the annual Medicare Part B annual deductible and your premium. Other than that, Medicare and your Medigap Plan G pay all your Medicare related in-patient and outpatient expenses. The math here is simple, if Plan G is $183 less in premiums per year, then it would be exactly equal to Plan G. You pay the $183 deductible out of pocket, then you have all the same benefits as a Plan F. However, Medicare Supplement Plan G is usually $300 to $400 a year less in premiums than a Medigap Plan F. That means you can pay the $183 Medicare part B deductible out of pocket and have money left over. That makes it a better value. People considering a Medigap Plan G often compare the price and benefits to a Medigap Plan N. Plan N usually cost much less than Plan G, but there is a catch! In fact, Medicare supplement Plan N can cost 25% to 35% less than a Medigap Plan G. However, Plan N has less coverage and an asterisks you need to be aware of. Just like with Medigap Plan G, with Medigap Plan N you pay that Medicare Part B deductible out of pocket. The asterisks indicate that you will also pay a small co-pay of up to $20 when you see a doctor, $50 for an emergency room visit. Medicare supplement Plan G is for those who want peace-of-mind. Once you have a Plan G you don’t have to worry about co-pays or excess charges or anything. You can see any doctor, whether they charge excess charges or not. Medicare advantage plans can without a doubt lower your monthly spending if you are HEALTHY! If you notice the key to that sentence is if you are healthy. Medicare Supplements are very popular with doctors because they pay their bills on time and they don't argue the cost, because they can't. Lets take a look at the comparison between the Blue Cross Medicare Advantage plan in Pennsylvania and the Aetna Medicare Supplement Plan F. One advantage plan from Blue Cross (HMO) has a zero premium. The out of pocket cost for the year could possibly be over $6500 for a year. It may cost you $20 for a doctor visit or $200 per day in the hospital etc etc. The point is it is $0 for the premium which gets everyone excited, but if you need to get a surgery done in December, it could cost you $6000 out of pocket. Also, the hospital may or may not take the Medicare Blue Cross Advantage plan. The Advantage Plan from Blue Cross that is "0″ premium is an HMO plan and some hospitals and doctors won't take it. The Medicare supplement plan F from Aetna is a Medigap plan that could cost around $150-$200 a month premium depending on if your a Male or Female, what zip code you live in etc. The difference is, the out of pocket cost for Medicare approved procedures is "0″. Also, any hospital that excepts Medicare has to except the Medigap plan F from Aetna. Also, this Medigap supplement Plan F could be from any company including, KSKJ life, Family Life, Mutual of Omaha, New Era, ANTEX, AARP, etc etc. The out of pocket cost is going to be your premium not copays and deductibles. The bottom line is do you want to pay in the front or in the back. Do you want to pay a zero premium and pay deductibles and copays, or do you want zero deductibles and copays with a higher premium. Watch closely. ----- If you decline Medicare Part B, then you are really not on "Medicare" in real terms. You will not lose your Medicare open enrollment status, you will not pay any penalties for Medicare Part D, you will be able to come into Medicare at a later date....smooth sailing. So what's the big deal about Medicare Part B? Everything!!! If you are not on it, you will not need a Medigap plan. You also will not need a Medicare advantage plan. You most likely won't need a Part D prescription drug plan, and you won't be hit with any penalties. If you want to get on Medicare and want the Original Medicare "package" you will have to take these steps: 1. Get your Medicare A and B card set up right at 3 moths before your 65th birthday 2. Call a Medicare Supplement agent like Medigaplist.com and see the different prices for Medigap Plan F or Medigap Plan G. ( or any other plan that is available) 3. Choose a company that supplies that Medigap plan in your state ( Florida, Pennsylvania, Texas, California, Kansas etc are all different) Make sure you look at some well known companies such as Blue Cross Blue Shield, AARP, Mutual Of Omaha, Aetna. Also look at some smaller companies such KSKJ life, Royal Neighbors of America, Forethought, Medico etc. 4. Choose a prescription drug plan with a good company with low deductibles and copays. People turning 65 and aging into Medicare need someone they can trust. They want to know, should I get an AARP (United Healthcare) Medicare supplement with aN Aetna prescription drug plan? Should I get a Blue Cross Blue Shield Medicare Advantage plan if I live in Pennsylvania or California or Florida? Is New Jersey a good state to look at medigap plan F or is it too expensive? Medicare advantage plans can without a doubt lower your monthly spending if you are HEALTHY! If you notice the key to that sentence is if you are healthy. Medicare Supplements are very popular with doctors because they pay their bills on time and they don't argue the cost, because they can't. Lets take a look at the comparison between the Blue Cross Medicare Advantage plan in Pennsylvania and the Aetna Medicare Supplement Plan F. One advantage plan from Blue Cross (HMO) has a zero premium. The out of pocket cost for the year could possibly be over $6500 for a year. It may cost you $20 for a doctor visit or $200 per day in the hospital etc etc. The point is it is $0 for the premium which gets everyone excited, but if you need to get a surgery done in December, it could cost you $6000 out of pocket. Also, the hospital may or may not take the Medicare Blue Cross Advantage plan. The Advantage Plan from Blue Cross that is "0″ premium is an HMO plan and some hospitals and doctors won't take it. The Medicare supplement plan F from Aetna is a Medigap plan that could cost around $150-$200 a month premium depending on if your a Male or Female, what zip code you live in etc. The difference is, the out of pocket cost for Medicare approved procedures is "0″. Also, any hospital that excepts Medicare has to except the Medigap plan F from Aetna. Also, this Medigap supplement Plan F could be from any company including, KSKJ life, Family Life, Mutual of Omaha, New Era, ANTEX, AARP, etc etc. The out of pocket cost is going to be your premium not copays and deductibles. The bottom line is do you want to pay in the front or in the back. Do you want to pay a zero premium and pay deductibles and copays, or do you want zero deductibles and copays with a higher premium. So much confusion when it comes to the donut hole in Medicare. The premium cost of all of these plans are different, the co pays are different, the mail order pharmacies, the drugs, the Lipitor generic, the simvastatin. It goes on and on, and the one general consensus is that Medicare Part D prescription drug plans are just darn confusing. Rocky mountain health plans in Colorado are easier to figure out than the drug plans for Part D. The first step is to get your Medicare A and B card. If you already have that taken care of and you have a Medicare supplement plan in place the battle is almost won. Maybe it is the annual enrollment period and you simply want to change your drug plan or switch from a Medicare advantage plan. Maybe you just are tired of running your drugs through the system only to find out the Medicare approved part D plans are all expensive. My advice is to get a broker to help you through this. Why is it that people always want to do things themselves? It doesn't cost you anything more to get a Medicare broker to walk you through this time period. If you want Medicare Part D info and you are tired of looking around then call us at Medigaplist. We can help. So many people have asked me what the best Medicare drug plan is and I always tell them it is different for everyone. The AARP drug plan from United Healthcare is the best Medicare drug plan in Colorado for certain people. Humana is the best Medicare Part D plan for someone else. It really depends on the drugs you take. If you take Lisinopril to treat high blood pressure aka hypertension, or you take another ACE inhibitor, the plan isn't going to be the samer Part D Medicare plan that someone taking Actos or Avandia. Omeone who is taking Valium or a Xanax obviously has a different condition than someone taking Altocor, lovastatin or Crestor. Would you take Aviane or a ethinyl estradiol/levonorgestrel for your back pain; I don't think so. oyu need to get a list of Medicare Part D drug plans and compare the premiums. You also want to compare the best Medigap plan F and see if you can save money on your supplemental Medigap plan. The bottom line is you want the best Medigap plan F or Plan G you can find if it's affordable, you want a great Medicare part D plan, you need to make sure you can handle the Medicare Part A and B premium, and you want a good dental plan from a company such as Delta Dental. This is especially true in states that have specific laws reducing the value of Plan G. In addition, some insurance companies are preparing for Medicare Supplement Plan D to be one of the top selling plans in Medicare. To be clear, I am referring to the Medicare Supplement Plan D and NOT Medicare Part D prescription drugs. Medicare supplement Plan D is a direct competitor to Medicare supplement Plan N. Is Medicare Supplement Plan N the right plan for you? Let’s take a look. When comparing Medigap Plan N vs. Plan G, people that prefer the Plan G to the Plan N tend to be people who would rather pay a little extra each month and avoid having to do the extra work to avoid excess charges. The plans are identified by letter across the top row. The category of Medicare services are in the left hand column. In the column below the Plan letter you can see the percentage of insurance coverage offered by that plan. You can find this table on our website and in various Medicare publications. With Medigap Plan N we see that it covers 100% “Medicare Part A coinsurance and hospital costs (up to an additional 365 days after Medicare benefits are used up.). Notice that all the supplements have this coverage. Also, notice down here that Medicare Supplement Plan N pays the Medicare Part A deductible. In 2017 that deductible is $1,380 per event. It will change every year, but with a Medigap Plan N that doesn’t matter. Your supplement will pay it. Without a supplement you pay the deductible up front, then have 60-days of hospital coverage before you start paying a copay. With Medicare supplement Plan N you can be an inpatient in the hospital for more than 365 days with 100% coverage. You will have no out of pocket costs for Part A. The asterisks indicate that you have the Medicare supplement Plan N copay. You will be required to pay up to a $20 co-pay for every doctor visit and a $50 copay for every emergency room visit unless you end up an inpatient in the hospital. Then the $50 copay is waived. The important qualifying words here are “up to”…$20. Here is what you can expect. So notice that with Medicare supplement Plan N you have 100% coverage until you get down to this line item referring to the Medicare Part B deductible and Medicare Part B excess charges. Here Medigap Plan N has no coverage at all. You are expected to pay those fees out of pocket. That’s important. Later in this video I am going to go over a real life example using a common joint replacement that will clear up some of the most common questions and misunderstandings about Medigap Plan N. In order to evaluate if Medicare supplement Plan N is right for you, you need to know what the Medicare Part B deductible is and what it is expected to be in the future. It’s also critical that you understand Medicare Part B excess charges. So let’s focus on these and then go over a real life example to show you how Medicare supplement Plan N works with office visits, hospital stays and physical therapy. If a doctor or hospital does not accept Medicare assigned rates, they are paid 5% less from Medicare, but allowed to charge you 15% more. The amount the doctor or hospital charges you that is more than what Medicare pays the doctor for that service is called an excess charge. Excess charges are only allowed on Medicare Part B services. Those are outpatient and physician services. There are also some states that either forbid or severely restrict Medicare Part B excess charges. These state laws are called Medicare Override Measure or MOM laws. Consider 2018 Anthem Blue Cross Georgia Medicare supplement plan G. You can cut out the middleman and buy direct but there may be a better way. If you call Anthem Blue Cross you will be asked to Press 1 for English. You will be routed to voice mail. If you are lucky someone will call you back, but you will probably be in the shower. You will never talk to the same person twice. How much do they really know about Medicare? Can they tell you about other carrier plans? Whether you use an independent Medicare broker like Bob Vineyard (shameless plug), or go direct, the premium you pay is the same. No charge for my advice. Anthem has agent compensation built in to every plan. You won't pay any more but you will be getting less when you buy direct from a Medigap insurance carrier. You must have Medicare Part A and Part B to enroll in a Medicare supplement plan. Anthem Blue Cross Medigap plans are accepted by any doctor. Part B is for outpatient health care treatment. Doctor visits, labs, X-rays, MRI, CT scan (CAT scan), ambulance, ER, outpatient clinics, etc. Essentially any Medicare approved health care received outside the hospital setting. Treatment considered medically necessary under Medicare guidelines. Medicare Part B has a calendar year deductible. Once satisfied, Medicare pays 80% of the approved charge, you are responsible for the remaining 20%. Most seniors worry about hospital stays, but your major expenses will more than likely fall under Medicare Part B outpatient expenses. Original Medicare stand alone has no cap on your out of pocket expenses. You pay until you run out of money, get well or die. The American Cancer Society tells us that more than half the 1.4 million new cancer diagnoses occur in people age 65 and older. Roughly 20% of retiree deaths are a result of cancer. Medicare pays for almost half of the $74 billion spent on cancer treatment. The elderly account for 70% of cancer deaths each year. Not all cancer responds to chemotherapy, but 80% of cancers are treated with chemo. Those treatments occur in an outpatient setting and are covered under Medicare Part B. Chemotherapy drugs administered in a doctor's office or clinic normally fall under Part B. Medication from a retail pharmacy or by mail fall under your PDP. This is why most of us chose a Medicare supplement plan in 2018. If you are diagnosed with cancer, the last thing on your mind should not be “How will I pay for my treatment?” I know how quickly things can change. When my health changes I want to control the direction of my care. Insurance carriers are not going to give me a list of doctors and tell me this year I can only use these, and next year the list may change. I am in original Medicare plus Medigap. You may not know it, but the Mayo Clinic does not participate in any Medicare Advantage plans. M D Anderson is another cancer treatment facility that does not accept Medicare Advantage plans. A study from the Medicare Rights discovered the number one reason for disenrolling from a Medicare Advantage plan was “provider access problems”. Number 4 on the list was “cost sharing too high”. You may never receive a cancer diagnosis. I hope you don’t. But one thing I can tell you with certainty is this. Original Medicare and a supplement plan provides assurance the funds will be there to pay for almost all of your care. Here is the conversation written out: Wendy: So today's episode we have Catherine...hi Catherine! She writes in and says “Tripp, can you please tell me the differences between a Plan F and a Plan G. I am trying to weigh them out and decide the best option for myself.” So first let's make sure that everyone understands; a Plan F and a Plan G is a Medicare Supplement… Tripp: Correct. Wendy: And these plans are called standardized, they are regulated by the government. Tripp: Correct. So all the Plan F by this particular carrier, say it is Humana and it's AARP or it is New Era or Aetna, a Plan F has the standardized benefits. Wendy: Meaning they're all the same across the board. Tripp: They are identical. So the same doctors, the same hospitals; the difference being how much each carrier just chooses to charge you the client per month on the premium. Wendy: And when he told me this I was just astounded. Tripp: Same plan. Wendy: And I think a lot of people don't realize that. They assume okay well if this carrier charges this amount and this carrier charges this amount, it must mean better benefits. Tripp: Right and it may be a carrier that they have “heard of” but the coverage is the same...it is identical. Wendy: It’s amazing. Tripp: A Plan F is a Plan F; a Plan G has the exact same benefits as another Plan G from Florida to North Carolina to Maine to Arizona, it doesn't matter. An F is an F and a G is a G. Wendy: Okay. So once we have that figured out, the next thing we have to determine is which plan is the best for your situation. So they are supplements; a Plan F is what is known as, they call it… Tripp: The Cadillac of all plans. And I think still a high majority of retirees and mature women, this is geared for the women, typically go with the Plan F because it is the most complete plan; so when they hear that knowing that your major objective is peace of mind and no headaches and you want to make sure everything is taken care of, that is why a lot of keep automatically “Gimme that Plan F!” Wendy: Yeah, and I can see that. It is like all inclusive, like when you choose a resort if you are traveling, you see one that covers everything, you realize you can just relax on your trip and you don't have to worry about being nickled and dimed and you can just kick back. So that would be a plan F, it covers pretty much everything. Tripp: It covers your Part A deductible, Part B deductible; the original Medicare, they are going to pay eighty percent, your Medicare Plan F is going to pay the remaining twenty percent for Part A and Part B. Wendy: Okay. And one other thing I just remembered, when you are deciding between the two, it was just released in recent legislation that the government is going to stop all Plan F’s in January 2020, so that is something to keep in mind. Trip: Now if you have already got a plan F, you are grandfathered in, as they say; but somebody that is aging into Medicare in 2020, like your father, he will not be able to get a plan F, they are taking it away. Wendy: Yes he turns sixty five in October 2020. Tripp: Right. So he will not be eligible to get a plan F. If you already have it you can keep right on rolling with it. Again I, as of the last couple of years we have written more Plan G’s than Plan F’s and we are getting ready to cover why as far as the cost effectiveness you will see. If you are taking Novolog or another Diabetes insulin pen you must do some work so that you are not going to automatically hit the Medicare part d donut hole. It isn't the same for someone taking a generic drug such as Lisinopril or lovastatin. These are the generic drugs that might cost $3 at Walgreens, CVS, or Walmart. The diabetes insulin injections can be costly and can put you into the donut hole in the first quarter of the year. Especially in 2014 when the donut hole is around 50% of your prescription drug retail cost. Many people that are taking Novolog or another diabetes insulin drug will be on a Medicare supplement plan F or a Medigap plan N and think that the costs are really low. They are. The costly part of Medicare plans are the part d Medicare prescription plans. If you compare the best Medicare part D plans and look at some of the out of pocket costs they can be very similar. A list of Medicare supplement plans with part d plans can show a person how to save money. I am using Novolog as an example but it is costly with all of the diabetes drugs. Best Medicare Supplement Plan Buying Medicare Supplement Insurance. How do buy Medicare Supplement insurance the right way? Hi, my name is Chad Cason and in this video, I’m going to share with you a few tips on buying Medicare Supplement insurance. I know it can seem complicated to navigate through all of the Medicare Supplement Options which is why I have made this video to help make it easy for you. Medicare Supplement insurance is the same thing as Medigap insurance. These insurance plans help to pay the hospital and doctor expenses that regular Medicare doesn’t pay. Regular Medicare, which consists of Part A and Part B, will only cover 80% of your hospital and doctor expenses, which leaves you with the other 20%, plus the Medicare deductibles…and, there is no maximum to how much you of the 20% coinsurance you could pay. There are 11 Medicare Supplement plans to choose from in the country and each of these plans cover that 20%, plus the Medicare deductibles… a little bit differently. Medicare Supplement plans are also identified by letters, for example, Plan F or Plan G… and the plans begin with Plan A and go through Plan N.
