Navigators are Coming! Health Care Exchange Navigators & Obamacare
http://youtu.be/2jAaqIT8_VU Hi Again. Randy Pifer with Active Insurance Solutions here in Grand Junction, Colorado. I apologize for the downtime between videos and writing a blog but things have been busy. There isn't a week go by that' I'm not invited to at least 2 or 3 seminars or webinars with regards to the Affordable Care Act. Believe me there is still a pile of information out there and a number of things that are yet to be determined. My topic today is going to be concerning Navigators: what are they, who are they, and where are they going to be available to talk to people about the Affordable Care Act. With regards to what they are, I'm going to go ahead and just read the Affordable Care Act definition for Navigators. "The role of a navigator will be to conduct public education activities to raise awareness of availability of qualified health plans, to distribute fair and impartial information concerning enrollment in qualified health plans, and the availability of premium tax credits and cost-sharing reductions in accordance with federal tax laws. They will facilitate enrollment in qualified health plans and they will provide information in a manner that is culturally and linguistically appropriate." Who are they going to be? Right now applications are being taken for navigator organizations; from non-profits, from community organizations, even associations. You yourself might be interested in becoming a navigator organization: all you have to do is fill out the form. One thing to keep in mind however is, no navigator will be able to be paid directly by an insurance company. Therefore, I myself will not be a navigator but will continue to be the broker for my clients and customers, hopefully for years to come. When are navigators going to be available to speak to people? Well, right now the deadline for the grant applications is June 6th, and they hope to have those applications approved and sent out by August 15th. That leaves an awful short window of opportunity for learning for navigators because enrollment in the exchange is going to begin October 1st. The government has set aside $54 million in grant money for the navigator program which will be spread across 33 states, those that are going to be administered by the federal government. The minimum grant application will be $600,000, obviously for smaller states where the number of uninsured and the membership growth is not going to be that big. One item that came up during the grant application with regards to the navigators themselves is: How will they be paid? It was suggested in the grant application an appropriate amount of money to pay per hour to an enroller might be $20 per hour, going up to $48 per hour for a navigator organization's administrative folks. It's estinmated that it may take as much as 90 minutes per enrollee in order to go through the application process and determine whether or not they qualify for subsidies or any other type of federal program as well as reviewing the benefit program,s that will be available in the state based exchanges. However, navigators will not be allowed to sign anyone up for a program and that in itself might present some problems, but agents and brokers are going to be a part of this program and we will be able to sign up individuals in the small group insurance exchange as well as the individual exchange. But navigators just can't recommend a program for the individual comsumer. How many navigators are we going to need? It's estimated that California alone may need 21,000 navigators to take care of their state insured population. When you calculate that out nationwide over the 33 states, that could be as many as 140,000 navigators, enough to field a small army. Navigators are going to be a part of the Affordable Care Act. I do hope that the information that they provide to the consumer is accurate and that they will be able to help a number of underserved people to get into the health insurance program, possibly for the very first time. On a side note with regards to the federal exchanges, just one update with regards to the administration that is going to be involved in this. For 2014 it has been estimated that the administrative costs for the federal exchanges is going to mushroom to $5.5 Billion. That is actually double what the original estimates were for the federal exchanges in the first place. My thought was that one of the items with regards to administration in health care was one of the things that needed to be reduced. I'm not sure how $5.5 Billion is going to reduce the overall cost of health care, but it seems like that is going to be the price to pay. But doing so had the opposite effect in Texas, according to a new study based on research from Texas A&M University. The study, conducted by economics professor Analisa Packham (now at Miami University), shows that in the first three years after Texas Republicans slashed the family planning budget in 2011 and shut down more than 80 women’s health clinics, the abortion rate among teenagers in the state rose 3 percent over what it would have been had the clinics remained open. After cutting Planned Parenthood out of the state’s subsidized women’s health program, then-Gov. Rick Perry (R) said his “goal” was to “ensure abortions are as rare as possible under existing law.” But the move actually interfered with an overall downward trend in abortions in Texas. “This certainly isn’t the way to have fewer abortions,” said Dr. Diane Horvath-Cosper, an OB-GYN in Maryland and an advocate with Physicians for Reproductive Health. “The abortion rates nationally have decreased and are at a historic low. Neural Representations for Program Analysis and Synthesis
Representing a program as a numerical vector (i.e., neural representation) enables handling discrete programs using continuous optimization approaches, and thus opens up new opportunities to tackle many traditional challenging programming language problems. In this talk, I will introduce two directions of my research along this direction. First, I have been working on synthesizing programs from specifications in different formats, such as natural language descriptions, input-output examples, and programs in a different language. I will present one of my latest results to synthesize a parsing program from input-output examples, which have been demonstrated difficult for existing program synthesis approaches. To tackle this problem, we propose a framework to learn a neural program operating a domain-specific non-differentiable machine, as well as a novel reinforcement learning-based algorithm to recover the execution trace for each input-output example. Our approach is demonstrated to achieve 100% accuracy over test inputs which are 100x longer than the training inputs. Second, I will introduce my work to use deep learning approaches to solve the binary code similarity detection problem. Code similarity detection has many applications such as plagiarism and vulnerability detection; however, I will show that whether two pieces of code are similar cannot be defined as one common criteria for all applications. My work is the first to demonstrate that deep learning can be used to mitigate this issue so that the same approach can be applied for different application domains; also our learning-based approach outperforms the best heuristics-based approach by a large margin. My research interest lies at the intersection of security, programming languages, and deep learning. I will briefly introduce my other efforts in this broad area and discuss potential future directions. No One Should Go Broke Just Because They Get Sick: Health Insurance and Obamacare (2013)
The number of persons without health insurance coverage in the United States is one of the primary concerns raised by advocates of health care reform. A person without health insurance is commonly termed uninsured (regardless of insurance of objects unrelated to health), and this article uses the term in this sense as well. According to the United States Census Bureau, in 2011 there were 48.6 million people in the US (15.7% of the population) who were without health insurance. The percentage of the non-elderly population who are uninsured has been generally increasing since the year 2000. The causes of this rate of uninsurance remain a matter of political debate. Rising insurance costs have contributed to a trend in which fewer employers are offering health insurance, and many employers are managing costs by requiring higher employee contributions. Many of the uninsured are the working poor or are unemployed. Others are healthy and choose to go without it. Some have been rejected by insurance companies and are considered "uninsurable". Some are without health insurance only temporarily. Some choose faith-based alternatives to health insurance. A study published in the American Journal of Public Health in 2009 found that lack of health insurance is associated with about 45,000 excess preventable deaths per year. One of the authors characterized the results as "now one dies every 12 minutes." Since then, as the number of uninsured has risen from about 46 million in 2009 to 48.6 million in 2012, the number of preventable deaths due to lack of insurance has grown to about 48,000 per year. A survey released in 2008 found that being uninsured impacts American consumers' health in the following ways: More of the uninsured chose not to see a doctor when were sick or hurt (53%) vs 46% of the insured. Fewer of the uninsured (28%) report currently undergoing treatment or participating in a program to help them manage a chronic condition; 37% of the insured are receiving such treatment. 21% of the uninsured, vs. 16% of the insured, believe their overall health is below average for people in their age group. The costs of treating the uninsured must often be absorbed by providers as charity care, passed on to the insured via cost-shifting and higher health insurance premiums, or paid by taxpayers through higher taxes. On the other hand, the uninsured often subsidize the insured because the uninsured use fewer services and are often billed at a higher rate. A study found that in 2009, uninsured patients presenting in U.S. emergency departments were less likely to be admitted for inpatient care than those with Medicare, Medicaid, or private insurance. 60 Minutes reported, "Hospitals charge uninsured patients two, three, four or more times what an insurance company would pay for the same treatment." On average, per capita health care spending on behalf of the uninsured is a bit more than half that for the insured. A study published in August 2008 in Health Affairs found that covering all of the uninsured in the US would increase national spending on health care by $122.6 billion, which would represent a 5% increase in health care spending and 0.8% of GDP. "From society's perspective, covering the uninsured is still a good investment. Failure to act in the near term will only make it more expensive to cover the uninsured in the future, while adding to the amount of lost productivity from not insuring all Americans," said Professor Jack Hadley, the study's lead author. The impact on government spending could be higher, depending on the details of the plan used to increase coverage and the extent to which new public coverage crowded out existing private coverage.
