It would be very hard for me to add to her work or find any better research than what she has done. So I want to tell you up front, much of this video is based on her work and research. That being said, Sari brought together and then asked three top registered dietitians, who are also certified diabetes educators, what they tell their clients about everyday drinks. We all know that when you have diabetes, picking a drink isn’t always easy, is it? And then add in all those studies that has been done recently on diet soda and weight gain or does coffee help or hurt insulin resistance? There are a lot of questions out there and I am so glad Sari wanted to get to the bottom of the question as badly as I did. So here is “our” work. Like I said, much of it is hers, I just filled in some gaps that I felt needed to be in there. As far as water goes, all of the researchers agree that we must drink more water. It could even help out with controlling our blood sugar. A recent study in the journal Diabetes Care found that people who drank 16 ounces or less of water a day (two cups’ worth) were 30 percent more likely to have high blood sugar than those who drank more than that daily. We each have a hormone called vasopressin, and it helps the body regulate hydration. Vasopressin levels rise when we are properly hydrated, which prompts the liver to produce more blood sugar. How much water should those of us with diabetes drink? Experts recommend six to nine 8-ounce glasses of water per day for women and 8-10 for men. You can get water from fruits and vegetables and other liquids you consume, but you won’t get enough. One way to help with your water intake and to help with some of those meal time binges is to drink a glass of water before every meal. Another great drink you can add to your diet is something we usually think of only for kids. But milk is wonderful for you because it provides the calcium, magnesium, potassium, and vitamin D your body needs for many essential functions. Did you know recent research shows milk may boost weight loss? In one study of 322 people trying to slim down (some had type 2 diabetes, some had heart disease), those who drank the most milk—about 12 ounces a day—shed 5 more pounds over the study period than those with the lowest dairy intake, about half a glass daily. Adding milk to a healthy diet can also help lower your blood pressure according to research from the Journal of the American College of Nutrition. How much milk is good? Most experts will say two to three daily servings of dairy products, including 2% or skim milk. Drink a glass with breakfast or choose dairy for dessert, like yogurt or sugar-free pudding as ways to get that daily intake. “Drink milk with a meal so your body can handle the natural rise in blood sugar that happens when we eat carbohydrates,” says Baltimore, Maryland–based registered dietitian Angela Ginn a nutrition education coordinator at the University of Maryland’s Center for Diabetes and Endocrinology. How about tea? Tea contains no calories, brings big flavor, and a boatload of antioxidants. This has made tea trendy for health reasons, especially for those with diabetes. One Chinese study showed that black tea has the highest levels of polysaccharides, which slow the absorption of sugar into the bloodstream. A German study suggests that the risk of getting diabetes can be cut by 16% by sipping 4 cups of tea per day. You must be careful however because of sweetened, bottled iced teas. These are loaded with sugar and that is what you are trying to avoid. Four to five cups of tea is a good amount if you can tolerate the caffeine. The next category is drinks that you can have, just be careful. There are studies that suggest coffee drinkers are at lower risk for developing type 2 diabetes. A compound in coffee called chlorogenic acid seems to slow absorption of glucose into the bloodstream. But there is also research that indicates for people who already have diabetes, coffee may raise blood sugar or make the body work harder to process it. The best answer as far as coffee is concerned is how it affects YOUR blood sugar. Of course don’t add sugar and sugar filled flavorings to your coffee. When it comes to how much coffee you should drink? Most experts say sipping two to three cups a day is enough, especially if you notice it having an adverse effect on your blood sugar...

A Certified Durable Medical Equipment Specialist or a CDME is a certification created by the BOC or the Board of Certification / Accreditation Int’l BOC saw a need to highlight the skills and qualifications of the highly skilled yet unknown individuals who are key to the dispensing of DME products. The CDME certification demonstrates the broad knowledge of the Durable Medical Equipment (DME) industry. What does a Certified DME Specialist do? The CDME basically does every aspect within the DME realm.  They are the people who meet the patients, do the documentation, fit the patients, determine what product the patient is eligible for and bills Medicare, Medicaid and the insurance companies for DME products.  The also help with basic repairs, troubleshooting, and home inspections for DME products like oxygen, transfer systems, enteral supplies, and wound care. A trained and certified DME specialists are highly valued in the DME industry as an assurance to patients and referral sources of professionalism and quality care. The BOC CDME Scope of Practice provides greater detail regarding the role of a BOC Certified DME Specialist. Jeff, I really haven’t heard about this before, so what advantage does a pharmacy or DME supplier gain by having a CDME on staff? This is simple.  We live in a very competitive market.  We need to ensure the pharmacy has unique qualities and products outside the normal pharmacy prescription filling process.  Those days have come and gone.  We have talked about immunizations and diabetic shoes in previous podcasts.  We will talk about other opportunities in future podcasts.  Pharmacists are experts in pharmacy operations.  When it comes to Medicare Part B and DMEPOS products, the rules are completely different.  For example, a physician writes a prescription or eScript and the pharmacist fills the script, the patient picks the medication up and pays their co-pay.  The insurance company and secondary adjudicate the claim and reimburse you for it.  Hopefully there is a low dollar DIR fee so the PBM claw back isn’t too severe next month. However, when it comes to Medicare Part B, everything changes when the physician writes a prescription.  The pharmacy language stops working.  So you look for someone who can do this work.  What do most pharmacies do? You grab a tech and throw them into this world and let them try to figure it out.  The tech starts reading reading Medicare Supplier Manuals, DME MACs and then learns about Detailed Written Orders and Local Coverage Determination and this doesn’t make sense.  Again the language has changed and there really is only limited training out there for dispensing or billing these products.  This is why the CDME certification was created.  It certifies individuals who have the basic skills for dispensing, setup and billing Medicare Part B and DMEPOS products. That makes sense.  I have heard about all the audits from Medicare and all the paperwork that is needed.  Jeff, you and I talked about the Diabetic Shoes April 2017 on an earlier podcast last year about the importance of the documentation and following the process.  So this certification helps the pharmacy? Absolutely.  When you are confused about something or you are losing money and audits, you stop dispensing that item.  This is what has happen with DME products.  Over 40% of DME facilities have gone out of business in the last six years due to competitive bidding.  Pharmacies have dropped DME products because of the bad taste of audits and not understanding the process. Now imagine having staff properly trained and certified who know what they are doing when it comes to Medicare Part B and DMEPOS products.  They can advise the pharmacist not to fill a prescription of albuterol because it needs a “Detailed Written Order Prior to Delivery” signed by the physician.  Who knew?  Your CDME did.  That saves your reimbursement. Whoa, is it that simple? Yes.  Now you are ready to market your pharmacy.  Your pharmacy may be accredited or exempt.  Your pharmacists are licensed.  Are you doing something out of the ordinary to make you stand out?  Your DME department has “board certified” specialists.  Who else in town has this? Here are some other advantages. Differentiation in the market; Management of risk; Criteria for evaluating potential employees; Limiting of fraud, waste, and abuse; Increase in customer satisfaction; Assurance for referral sources; and Medicare human resources management standards require that technical personnel be knowledgeable, competent, and trained in order to deliver products. Healthcare Reform 10 Immediate Impacts

Here is a look at the top 10 immediate impacts of the law. The first big impact is for children. They will no longer be denied insurance for pre-existing conditions. So little Susie has diabetes and previously was denied coverage because of the disease. Her parents plan will now be required to cover her regardless of the pre-existing condition. Second - many Americans who are [more]currently uninsured because of a pre-existing condition will get immediate access to insurance through a temporary high-risk pool. For example, John has been uninsured for nearly a year and is having a hard time finding coverage because he has asthma, a pre-existing condition. Through the new high-risk pool, John will be able to apply for coverage regardless of his medical history. Third - if someone has a group or individual plan and gets sick, their plan will not be allowed to drop their coverage. So if Bob has health insurance and is diagnosed with lung cancer, his insurance policy will continue to cover him regardless of the condition. The reform is designed to save individuals and businesses money. So fourth Seniors will see lower prescription drug prices through a $250 rebate for Medicare beneficiaries who reach the Part D coverage gap during 2010. For example, Sally is 75 and on Medicare. She has four prescriptions that she fills every month. When Sally reaches her prescription drug coverage limit, she will be entirely responsible for the cost of the drugs. The $250 rebate will help Sally make up for the price jump. Fifth - Small businesses will get tax credits when they purchase coverage. So, the owners of a corner donut store want to offer health insurance for their employees but cannot afford to do it on their own. Under this provision, if they meet certain requirements, the owners could apply for a tax credit that would reimburse them for part of the cost of coverage. Sixth- To help explain all this, the Secretary of Health and Human Services is creating a Web site that residents and small businesses in any state can use to get information about health coverage options in their area. This will include things like side-by-side comparisons of coverage options, information about eligibility for Medicaid, availability of temporary high-risk pools, and more. Seven there will be a new temporary reinsurance program for employers. This program will provide health insurance coverage to retirees over 55 who are not yet eligible for Medicare. The program will reimburse employers for a percentage of the cost of their employees coverage. So lets say Carol retired from her company, but she is only 58 and not yet eligible for Medicare. Her company could continue to provide her health benefits, and the government would reimburse the company for a large portion of the costs. 8 - The reform takes away lifetime limits and restricts annual limits on benefits in all plans. This means that insurers will no longer be able to put a cap on the dollar value of an individuals benefits. The insurer will be required to pay the costs regardless of how much it has already spent on that policy. Number nine has an immediate impact on dependent eligibility. Under the plan, a subscribers dependent children will now be covered until age 26. That means that instead of being terminated from his parents plan when he graduates college, Tommy can continue to be covered until he turns 26. Our 10th impact has to do with wellness and prevention - To encourage healthy living and keep costs down, insurance providers must now offer coverage for preventive services and immunizations. That means you will no longer have a co-pay for services like cervical and breast cancer screenings, tests for HIV and other sexually transmitted infections, counseling for tobacco use, and more. Individuals and employers are going to have to understand these changeswe hope this information has helped you. Senator Becky Shmitz, a former school social worker of 27 years, discusses her support of preschool programs. In her time as a school social worker, Shmitz witnessed the benefits of preschool education, namely marked improvements in early reading and math skills in her students. She asserts that it was almost immediately discernible which children had attended preschool and which ones had not. She recalls Joey, a student without preschool experience who exhibited behavioral and cognitive problems. She charted his growing problems throughout the years and believes that if he had attended preschool, many of his struggles would have been addressed. Preschool programs have shown both social and academic benefits. A quality nursery school not only helps children with their cognitive development—it can help build social skills which will prove invaluable to children for the rest of their lives. Peer socialization is extremely important at the preschool age, when children begin to play alone less and seek out communication with other children their age. The group format encourages peer communication and gives children the ability to comfortably interact with each other, adults, and teachers alike. Peer socialization encourages healthful physical activity. Children influence and mimic each other in a myriad of ways, many of which can be positive. Sports and other physical activities are a prime example—if peers play sports regularly, a child’s physical activity will often increase. Research has shown that children are more likely to engage in intense physical activity when in the company of other children their age in a kindergarten.

This Fall, state Health Insurance Exchanges will begin enrolling low-income families in health insurance plans, and allowing them to pay for their insurance with premium tax credits, or subsidies. Exchanges are essential to fulfilling the promise of health care reform, but they're untested, and in many cases behind schedule and over budget. Leaders in both parties believe we should not leave the enrollment effort solely in the hands of unproven exchanges. Many have urged the Department of Health and Human Services to seek help from companies in the private sector. Health and Human Services embraced this approach with regulations that allow states to partner with licensed online health insurance marketplaces from the private sector. This approach creates strong public / private partnerships that combine the best of government and business. But, how would these marketplaces work with a state's exchange? Here is an example of how this process could work: - If we assume many people will do what they're already doing today, every day. - A person who goes to Google or Bing, might search for "health insurance" - The first "free" result they're likely to see is the online health insurance marketplace, eHealthInsurance.com - After clicking on that result, the individual would find themselves on an online marketplace and do what comes naturally, click "find plans" - The consumer then provides some basic anonymous demographic information -- age, gender, zip code, family size and the date they want their coverage to start - An marketplace approved to enroll consumers in qualified health plans using premium tax credits would also need to make the consumer aware that they may be eligible for a subsidy or premium tax credit. - To determine their potential eligibility, the visitor could simply select their income range. - With that small bit of information, an online marketplace could let those who -- based on estimated income and family size - are likely to be eligible for a subsidy to apply for assistance... - A licensed online marketplace could then quickly let the customer know that they may be eligible for a subsidy, and explain the process they'd need to undertake to apply for a subsidy - From here, a consumer could be sent to an exchanges subsidy application page -- all the while having access to a licensed agent or other support resources from the third party online marketplace - Once the subsidy application is complete -- an individual could shop directly on the exchange through an iFrame... - or, depending on the private marketplace's integration with an exchange, customers could shop for insurance using their subsidies right on the online marketplace. - In either case, federal regulations make it clear that customers must be presented with every plan available on the exchange -- again, the exact same plans at the exact same prices as they're displayed on the state's exchange - with the subsidy applied to the cost of the plan.

The country seems to be pretty much divided as to which side of this issue they are on. As one who has worked in nutrition and alternative medicine for over 30 years, I see one issue that has been only briefly mentioned. Regardless of which viewpoint you hold or which program ultimately comes from this issue, any program will fail sooner or later due to the gross economic burdens, which will increase with each passing year. If we do not factor in the one concept that has been missing from this discussion, namely prevention, any program will be doomed to fail. The Obama Health Care Plan as it has been called, leans toward a Government run health insurance program for those who cannot afford private insurance, yet with the Obama Health Care Plan, if prevention is not promoted and in fact demanded, the costs could be unaffordable. Any program, no matter what it is called, be it the Obama Health Care Plan, the Bailout Health Care, Health Care Reform, or Universal Health Care, will have to have strong emphasis upon prevention in order for it to be effective. Preventing illness can save trillions of dollars over the next few years alone. Consider that obesity is responsible for dozens and dozens of health problems, most of which could be prevented by proper weight management programs. As an example, the fear of the flu virus or the H1N1 virus. If we teach people to live a healthier lifestyle, they would have the strength of their immune system to ward off such viruses much more often, cutting costs of treatment. In this heated argument where people are putting their emphasis and energies on tea party demonstrations and forming groups like the HAARM or the Healthy Americans Against Reforming Medicine, all of which have their place, we should be placing equal emphasis on demanding more programs that are aimed at preventing health problems instead of solely on how we will treat these disorders. No system can hope to survive financially unless prevention of the thousands of medical incidences are at the core. Consider such factors as obesity, cigarette smoking, lack of exercise, poor diet, and a lack of dietary supplements. Each of these factors could be relatively easily addressed. Instead we are focusing on health care bailouts, Health Care Reform, or Universal Health Care instead of the real health care program of prevention. Town hall health care meetings and health care debates will only go so far. No matter what the outcome, if prevention is not at the core of the program, any program, it surely will fail. Whether you subscribe to saving health care through universal health care, through private insurance programs, or perhaps a combination of both, we are still going to have to face the fact that unless we learn to change our lifestyles to prevention based concept, the costs will continue out of reach. Even government sponsored health care plans may not be able to afford the rising costs of health care in America. Consider that obesity is the cause of so many preventable diseases including type II diabetes, the most epidemic disease in America. Prescription drug costs are out of control with drug companies making billions more in profits each year more than at any time in history. Sadly, the majority of people are taking at least some drugs that they dont need. Our senor citizens are the most at risk as they are often taking 10 or more powerful prescription drugs at any one time, leaving them feeling less than well. Not only is this a disservice to the client, the costs of maintaining this drugging of our citizens is astronomical. By following simple concepts of prevention such as learning to eat better, saving junk foods for treats instead of the staple of the diet, exercising at least 3 days a week, and taking a Full Spectrum supplement providing the 100+ nutrients the body needs on a daily basis, could save any health care system trillions of dollars over the next decade alone. Health care reform must start from the inside. Our system of health care is based upon breakage and repair. In order for the medical system to survive they must have a regular influx of patients with treatable diseases or disorders.
