Video abstract of review paper "Current approaches to treatments for schizophrenia spectrum disorders, part II: psychosocial interventions and patient-focused perspectives in psychiatric care" published in the open access journal Neuropsychiatric Disease and Treatment by authors Chien WT, Leung SF, Yeung FKK, Wong WK. Abstract: Schizophrenia is a disabling psychiatric illness associated with disruptions in cognition, emotion, and psychosocial and occupational functioning. Increasing evidence shows that psychosocial interventions for people with schizophrenia, as an adjunct to medications or usual psychiatric care, can reduce psychotic symptoms and relapse and improve patients' long-term outcomes such as recovery, remission, and illness progression. This critical review of the literature was conducted to identify the common approaches to psychosocial interventions for people with schizophrenia. Treatment planning and outcomes were also explored and discussed to better understand the effects of these interventions in terms of person-focused perspectives such as their perceived quality of life and satisfaction and their acceptability and adherence to treatments or services received. We searched major healthcare databases such as EMBASE, MEDLINE, and PsycLIT and identified relevant literature in English from these databases. Their reference lists were screened, and studies were selected if they met the criteria of using a randomized controlled trial or systematic review design, giving a clear description of the interventions used, and having a study sample of people primarily diagnosed with schizophrenia. Five main approaches to psychosocial intervention had been used for the treatment of schizophrenia: cognitive therapy (cognitive behavioral and cognitive remediation therapy), psychoeducation, family intervention, social skills training, and assertive community treatment. Most of these five approaches applied to people with schizophrenia have demonstrated satisfactory levels of short- to medium-term clinical efficacy in terms of symptom control or reduction, level of functioning, and/or relapse rate. However, the comparative effects between these five approaches have not been well studied; thus, we are not able to clearly understand the superiority of any of these interventions. With the exception of patient relapse, the longer-term (eg, more than 2 years) effects of these approaches on most psychosocial outcomes are not well-established among these patients. Despite the fact that patients' perspectives on treatment and care have been increasingly concerned, not many studies have evaluated the effect of interventions on this perspective, and where they did, the findings were inconclusive. To conclude, current approaches to psychosocial interventions for schizophrenia have their strengths and weaknesses, particularly indicating limited evidence on long-term effects. To improve the longer-term outcomes of people with schizophrenia, future treatment strategies should focus on risk identification, early intervention, person-focused therapy, partnership with family caregivers, and the integration of evidence-based psychosocial interventions into existing services. Once this is confirmed, the clinician shows how exposure therapy can be introduced to the client for subsequent sessions, ensuring he is prepared that the incident will be revisited. Stress inoculation is also used with the mention of other dynamics that are impacting Allen such as gender roles and personal boundaries. Educating Allen on these aspects of his experience will help mitigate feelings of guilt about grieving for his wife and the effect it has on his work performance. Disclosing this information to a new therapist is extremely difficult for some clients, and Allen is visibly saddened and anxious about doing so. Bedridden patients need a special attention because they are not able to do anything by themselves. So a nurse assistant is required to take care of their daily activities including their personal hygiene. The second concern is their pain assessment along with the general examination, vital parameter monitoring, level of consciousness monitoring with the help of a Glasgow Coma Scale and there are some other monitoring also which is essential for a bedridden patient. The bedsore assessment has to be done on a frequent interval to avoid the occurrence of a bedsore or a wound. Another concern is dietary management based on the doctor's order or based on the patient's requirement the diet has to be followed and the feeding has to be given accordingly. There are different ways of feeding the patient; either through oral; if the patient is not able to swallow the things then a tube will be inserted through the nose directly into the stomach which will be called as a Ryle's tube. The nurses will be feeding through the Ryle's tube or in some of the cases surgical intervention is also required to feed the patient. Here the tube is directly inserted into the stomach through a surgery through the abdominal wall. So that tube is called as peg tube. Through this tube also we can feed the patient based on requirement and specially through the tube we will be feeding the liquid material only. Another concern in the bed ridden patient care is planning a care plan based on the assessment, the nurse will be planning the care plan and executing it. Mobilisation of the patient is also a part of the care plan which includes the active and passive exercises. Active and passive exercises will help in the blood circulation and which also will help the patient to heal better. Along with all this psychological support also plays a great role in the speedy recovery.

http://www.cincinnatichildrens.org In times of crisis, you want to know that your child is in the safest possible environment, a place where she can focus on getting better. The Division of Psychiatry at Cincinnati Children's offers expert assessment and diagnosis of children who are experiencing behavioral, emotional or developmental disorders. We offer care on both an inpatient and outpatient basis, as well as residential treatment for longer-term care. Our first priority is to stabilize a child who is in crisis. Treatment is then tailored to each child's individual needs by a team from our Division of Child and Adolescent Psychiatry. We treat kids ages 2 to 18 at three campus locations that offer inpatient treatment. Your child will be assigned to a unit best suited to his developmental, psychological and medical needs. That unit may be in a different location than where you first arrive for care. Depending on the number of patients on the unit, your child may have a roommate. Our units are not separated by boys and girls. Most children stay on an inpatient unit for five to seven days . Your child's safety during his stay with us is of the utmost importance. All units are locked. Your child will receive a special ID number to protect confidentiality. We provide clothing for your child to wear and do not allow electronics or other items brought from home. You will receive a Parent Handbook that outlines all of the safety measures we employ, and provides details about visiting hours and procedures. We will asked questions at admission such as who your family doctor is, what medications your child dates, and whether your child has a case worker or counselor. The doctor who cares for your child will perform a thorough evaluation, direct his treatment plan and manage his medications. Your child's primary nurse will meet with you and manage your child's inpatient stay. The primary nurse and social worker will be able to answer most of your questions. Mental health specialists will keep your child safe as they lead goals groups and provide one-on-one support. A health unit coordinator will help to keep things running smoothly. Your child will have a team of experts here, including behavior specialists, teachers, dietitians, music therapists, speech and occupational therapists and chaplains. We help your child work on a specific goal each day and develop positive ways to cope with troublesome feelings. We also set aside plenty of time for kids to let loose and have fun playing games, watching movies, doing crafts or visiting our recreation areas. When your child is ready to return home, we will work with your family and your child's school and primary care provider to have a plan in place for continued treatment. You've taken the best possible step by seeking help. We can help your child learn to manage difficult feelings and give your child -- and you -- the skills and resources needed to grow and thrive.

Therapeutic Relationships in Nursing: The Client and Nurse Perspective (Part 2)

In the first part of this video the importance of the TR within nursing was briefly introduced. The nursing profession's perspective of a therapeutic relationship was also described. The purpose of this video is to talk about the Therapeutic Relationship from the perspective of the client and the nurse. Clients want and need a TR with a nurse. A variety of literature exists that talks about how important this relationship is to clients within a variety of settings. For example, in mental health supportive relationships with nurses are valuable in health promotion and restoration. The good nurses in this study were described as friendly, nice people who validated and listened to the client. These nurses were able to help clients explore and work through problems. Clients withheld information when nurses did not establish trust with them or explore their situation. Withholding information was detrimental to client outcomes. This study is a little older, but I like the key messages in it. The article talks about how the things that worked were so simple. Caring, listening, empathy, and use of power are discussed. Communication techniques like reflection, and paraphrasing are keys to an effective relationship. Don't be afraid of interacting with people. Try to understand their experience. TRs are also important in day surgery settings. Clients want nurses who care. Nurses who are friendly, comforting and in the moment with them. For clients to be satisfied with their care they want nurses who meet their needs, are competent, caring, and pleasant. Nurses must also maintain trust through responding to needs in a timely fashion. From the perspective of nurses, much of what is seen as essential in a TR is the same as what has already been outlined. Nurses, like clients, think that listening, caring and attention to client needs helps to establish a TR. If a TR is not established it is frustrating for the client and nurse. Instead of progressing through the working and resolution phases of the relationship there is a struggle that ends in a mutual withdrawal from the relationship without meeting the needs of the client. Nurses describe a number of pivotal moments in a TR. Nurses describe feeling honored when clients trust them. They perceive that power in a TR should be shared to promote effective and productive relationships. Nurses also describe the reciprocity that exists in a TR where they are able to learn from the clients. When nurses are in the moment with clients they say it can be therapeutic to share something personal. It is important to note that this sharing does not reflect crossing a boundary or sharing of secrets. Congruency and consistency promotes honesty and integrity within the relationship. Patients appreciate the genuine behaviour of the nurse. Authenticity within the relationship facilitated the therapeutic use of self within the relationship. These themes of trust, power, mutuality, self-revelation, congruence and authenticity are interdependent. Because they co-exist there is considerable overlap between these themes. While nurses think that caring for clients is essential, it also necessitates a need to care for oneself. Reflection is a tool that has been suggested for improving the understanding and application of caring in a TR. Reflection helps nurses improve practice and learn from situations where emotional attachment can be therapeutic or cloud judgments. At times it is hard to know what to do. Reflecting on experiences helps to prepare nurses for future relationships. One of the biggest mistakes novice nurses make is going in trying to solve the problem for the client. Remember, you do not need to fix the problem for them. Here is one of my favorite quotes from the RNAO document. It highlights the idea that as nurses you do not need to fix client problems. Your role is to help them discover their own solution to their problem. Interrelated concepts that the nurse must be aware of and apply in order to form and maintain a therapeutic relationship with the client include ... among others. The importance of each concept and how they can be demonstrated or applied to practice deserves further exploration in order to become a therapeutic nurse. Thank you for watching. Reactive attachment disorder (RAD) falls under “trauma and stressor related disorders.” This is important to note because it is due to a trauma or early life stressor that this disorder comes about. There are many diagnostic criteria, but you must first note that this occurs between the ages of 9 months and 5 years. So this is something diagnosed at a very young age and has to do with attaching to our primary care giver. This usually occurs because the child was either passed around a lot from home to home (think of foster children and how often they can be moved). Also, if children did not have an attentive parent or if they parent was neglectful of them and their needs. Due to these and many other issues, a child is not able to attach and will act withdrawn, rarely seeking comfort when distressed and will not respond to any comfort that they receive. They will also show minimal emotional responsiveness to others and can even lash out even when things are not threatening or clearly upsetting to them. Overall children who struggle with this will not be expressive emotionally or seek any comfort if they are upset. I know this can sound like a really intense situation but with therapy and proper support (at home and school) it can be overcome. Willis, TX we also offer long term skilled nursing for those who cannot return to a lesser level of care and need the full, 24 hour support to maintain an optimal level of independence and quality of life. Our long term program continues to put our residents; and/or family members in the center of their care plan; but the goals are designed to improve quality of life while managing the various conditions and diseases that are present. Psychosocial care is an integral part of quality survivorship. CIOS is pleased to integrate the evidence-based programs and services of Cancer Support Community (CSC), a leader in psychosocial oncology, into our cancer care delivery model to ensure our patients get the best psychosocial care available. GHS is the first hospital in the nation to form this collaboration and one of the first to utilize Cancer Support Community's Cancer Support Source, a distress screening program that allows staff to better determine what programs and services will be most beneficial to each patient. Licensed Clinical Social Worker, Kerri Susko, MSW, LISW, facilitates counseling, educational programs, support and connections to appropriate community resources to increase the patient's support network and to address individual needs.
