Statin medication can have side effects. Some of these side effects can be less or more serious than others. Learn more about statin side effects in this video. Transcript Every medication comes with side effects and it's important to know what those side effects are. For the statin class of medications, there are a few you should know. The most common side effects of statins are muscle pain, or cramping, or weakness. Occasionally, these muscle symptoms can be more serious and can include damage to the muscle itself. Another side effect of statins is damage to the liver, and some patients may develop alterations or changes in their liver enzymes. In addition to the muscle and liver changes that can occur, statins can also cause some stomach upset, nausea, constipation, vomiting or diarrhea. Finally, the FDA has issued warnings on statins that, although rare, there is a risk of development of type II diabetes as well as a risk for memory changes and confusion. Overall, as a class, the side effects of statins is very low, but it's important to know what they are so you can recognize if you're having any of these side effects and speak to your doctor about them. If you do have these side effects on one type of statin, your doctor can usually safely switch you to another one, and try that one out before giving up on statin medicine class completely. 15 Cancer Symptoms Men Ignore You eat pretty well (some days) and get regular exercise (most days). But if you’re like a lot of men, a trip to the doctor isn't on your to-do list. That can be bad if it means you brush off early signs of cancer. One of the best ways to fight the disease is to catch it in the early stages, when it’s more treatable. The problem is that the warning signs for many kinds of cancer can seem pretty mild. Take a look at these 15 signs and symptoms. Some are linked more strongly to cancer than others, but all are worth knowing about -- and even talking over with your doctor. 1. Problems When You Pee Many men have some problems peeing as they get older, like: -A need to pee more often, especially at night -Dribbling, leaking, or an urgent need to go -Trouble starting to pee, or a weak stream -A burning sensation when they pee An enlarged prostate gland usually causes these symptoms, but so can prostate cancer. See your doctor to check on the cause of the problem. He’ll give you an exam to look for an enlarged prostate, and he may talk to you about a blood test (called a PSA test) for prostate cancer. 2. Changes in Your Testicles "If you notice a lump, heaviness, or any other change in your testicle, never delay having it looked at," says Herbert Lepor, MD, urology chairman at New York University Langone Medical Center. "Unlike prostate cancer, which grows slowly, testicular cancer can take off overnight." Your doctor will look for any problems with a physical exam, blood tests, and an ultrasound of your scrotum. 3. Blood in Your Pee or Stool These can be among the first signs of cancer of the bladder, kidneys, or colon. It's a good idea to see your doctor for any bleeding that’s not normal, even if you don't have other symptoms, Lepor says. Although you're more likely to have a problem that's not cancer, like hemorrhoids or a urinary infection, it's important to find and treat the cause. 4. Skin Changes When you notice a change in the size, shape, or color of a mole or other spot on your skin, see your doctor as soon as you can. Spots that are new or look different are top signs of skin cancer. You’ll need an exam and perhaps a biopsy, which means doctors remove a small piece of tissue for testing. With skin cancer, you don't want to wait, says Marleen Meyers, MD, an oncologist at NYU Perlmutter Cancer Center. 5. Changes in Lymph Nodes Tenderness of swelling in your lymph nodes, the small bean-shaped glands found in your neck, armpits, and other places, often signal that something's going on in your body. Usually, it means your immune system is fighting a sore throat or cold, but certain cancers can also trigger the changes. Have your doctor check any swelling or tenderness that doesn't get better in 2 to 4 weeks, Meyers says. 6. Trouble Swallowing Some people have trouble swallowing from time to time. But if your problems don’t go away and you’re also losing weight or vomiting, your doctor may want to check you for throat or stomach cancer. He’ll start with a throat exam and barium X-ray. During a barium test, you drink a special liquid that makes your throat stand out on the X-ray. 7. Heartburn You can take care of most cases of heartburn with changes to your diet, drinking habits, and stress levels. If that doesn’t help, ask your doctor to look into your symptoms. Heartburn that doesn't go away or gets worse could mean stomach or throat cancer. Heartburn can also lead to a condition called Barrett's esophagus, which occurs when stomach acid damages the lining of esophagus. While it is rare, Barrett’s can make you more susceptible to developing throat cancer. 8. Mouth Changes If you smoke or chew tobacco, you have a higher risk of mouth cancer. Keep an eye out for white, red, gray or yellow patches inside your mouth or on your lips. You could also develop a canker sore that looks like an ulcer with a crater in it. Talk to your doctor or dentist about tests and treatments. 9. Weight Loss Without Trying Pants fitting a little looser? If you haven’t changed your diet or exercise habits, it could mean that stress or a thyroid problem is taking a toll. But losing 10 pounds or more without trying isn’t normal. Although most unintended weight loss is not cancer, it’s one of the signs of cancer of the pancreas, stomach, or lungs. Understand the antihyperlipidemic agents once and all! In this quick board review lecture I'm gonna cover the anti-hyperlipidemic agents and that includes the following groups. The HMG-CoA reductase inhibitors, the fibrates, niacin, bile acid sequestrants and cholesterol absorption inhibitors. Let's get started. The HMG-CoA reductase inhibitors are a very important group of drugs. Five out of five you've got to know these drugs for your exams. The more common name for these drugs is the statins. However, the mechanism of action is actually found in the first name. They are reversible and competitive inhibitors of HMG-CoA reductase. That is a very important rate-limiting enzyme in the pathway that produces cholesterol in our liver de novo. On the right I have a picture to explain this but I'm gonna break it down and walk through the steps. Starting with acetyl-CoA. Acetyl-CoA is an important molecule in biologic reactions and it plays an important part in the synthesis of cholesterol. Acetyl-CoA is basically an acetyl group that is linked to coenzyme A. Let me just shorten coenzyme A to just CoA. In that first step on our list below you can see that if we combined two acetyl-CoAs together through the enzyme thiolase we get a molecule called acetoacetyl-CoA. If we then take that molecule and add another acetyl-CoA through the enzyme HMG-CoA synthase we have HMG-CoA, and this is where the statins work. The statins block the next step. What's supposed to happen is that this HMG-CoA is supposed to be reduced to mevalonic acid. Again, the statins block this from happening and therefore ultimately inhibit or decrease the production of cholesterol de novo in the liver. Major side effects for the statins include hepatic toxicity, however, while this can be severe, only about one to two people per 10,000 patient years have this problem. What's more common by far is myositis or myalgias. It is very common that your patients could complain of muscle aches while on this medication. And in severe cases, they could have Rhabdomyolysis from it. As for examples for the statins, we have these four here. Rosuvastatin, atorvastatin, simvastatin and lovastatin and I organized this list in this way on purpose. On the right side of this little list are the equivalencies. Rosuvastatin is the strongest of these four statins with an equivalency of 1.0. Lovastatin is the weakest of these statins with an equivalency of eight. In other words you would need theoretically and it doesn't work out exactly but you would need about eight milligrams of lovastatin to equal the effect that you get with one milligram of rosuvastatin. And finally if you really want to impress your preceptors look up the ASTEROID Trial which was done in 2006. It showed that patients who are treated with 40 milligrams of rosuvastatin actually had a regression of their atherosclerotic plaques at a two-year follow-up. Moving on, now the fibrates. These drugs are pretty easy to pick out in a group because they end in -fibrate or -fibrozil. These drugs are often given in combination with the statins because there's not a lot of good evidence to support using them alone. As for their mechanism of action, they act by stimulating or activating the peroxisome proliferator-activated receptor subtype alpha, or abbreviated PPAR alpha. Don't confuse this with PPAR gamma. The PPAR gamma is stimulated by the related class of drugs, the thiazolidinediones which are used for diabetes. Naturally in the body, these PPAR alpha receptors are found on the cell nucleus. They are used in the regulation of carbohydrates and fatty acids. When we stimulate these receptors with the fibrates we change that metabolism and this works by having the drug bind the receptor which makes the receptors move into the nucleus. Once in the nucleus, the PPAR alpha binds another receptor called the retinoid X receptor and that complex binds DNA on sequences that are called peroxisome proliferator hormone response elements. These response elements are responsible for the expression of proteins that are involved in the metabolism of fatty acids and triacylglycerides. For example, you would see an increase in the expression of lipoprotein lipase. This would increase the metabolism of triacylglycerides. Also it would increase the expression of apolipoprotein A1 and apolipoprotein A2. Those are lipoproteins that are important in the structure of an HDL particle. The side effects for the fibrates include GI upset, this is really common. You very well could see your patient complain of a stomach ache on the fibrates.

Your Medication Cephalexin Cephalexin is also known by the brand name: Keflex. Cephalexin comes in 250, 500 and 750 mg capsules; 125 mg/5 mL and 250 mg/5 mL oral suspension. Cephalexin is most commonly used for bacterial infections. No matter you’re your doctor has you taking it for, cephalexin is usually taken by mouth with or without food most commonly 3 to 4 times a day. If upset stomach occurs take cephalexin with food. If you miss a dose, take the missed dose as soon as you think about it. If it is close to time for your next dose, skip the missed dose and resume your normal dosing schedule. Do not take 2 doses at the same time or extra doses. It is important to take to take this medication until the full prescribed amount is finished, even if your symptoms disappear. Some common side effects with cephalexin include upset stomach, and diarrhea. Some less common side effects with cephalexin include dizziness, tiredness, headache, itching, heartburn and vaginal yeast infection. While taking Cephalexin, remember it is important to tell your doctor or pharmacist if you have: • Signs of an allergic reaction (rash, itching, swelling, tightness of the chest, etc.) • An allergy to cephalexin. • If you are taking probenecid. • If your symptoms do not get better or if they worsen, call your doctor. Cephalexin liquid needs to be stored in the refrigerator and shaken well before each dose is given. Use the measuring device received from the pharmacist or doctor. Any liquid remaining after 14 days needs to be thrown away. All other forms of Cephalexin are kept at room temperature, in a dry place.

When the government was tracking statin use last decade, about 50% of men and 35% of women in the US population between the ages of 65 and 74 were users. Even in the 46-64 age group, about 1 in 5 men and women were taking a statin. Data hasn’t been gathered this religiously lately but I see no reason to think that statin use is down. I would imagine that just about every adult in the US knows the names Lipitor and Crestor. What is easier than taking a pill to beat heart disease? One of these magic pills per day and poof, heart disease is just a memory, right? The data isn’t quite that clear that statin use has reduced deaths from heart disease. Some studies say they do the job and other studies seem to indicate that other factors are just as or more important. Should you be taking a statin? Some independent researchers and medical associations think that certain people should consider drug therapy. This include: People who already have cardiovascular disease that is those that have had heart attacks, strokes caused by blockages in a blood vessel, mini-strokes (transient ischemic attacks), peripheral artery disease, prior surgery to open or replace coronary arteries, LDL levels of 190 mg/dL or higher, have diabetes and an LDL of 70 or above and anyone else with a family history of heart disease especially with an onset before 55 in men or 65 in women and have other risk factors along with an LDL above 100. Other risk factors include: smoking, low exercise, high blood pressure, obesity, poor stress management. Let’s say you have several of these risk factors and decide that one these statins is the right approach. You get plenty of propaganda about the benefits of statins so let’s talk about some of the negative effects. The most important negative effects are muscle soreness and memory loss along with difficulty sleeping and headaches. Of course sleeping problems will contribute to memory loss. Some of these negative effects are the result of statins action to lower CoQ10. CoQ10 or coenzyme Q-10 is made by the body but like many important biochemicals, tends to decline with age. CoQ-10 is used in energy production pathways and is also a powerful anti-oxidant. Less than ideal levels of CoQ10 have been implicated in a long list of health problems including: • heart disease • kidney disease • high blood pressure • age-related macular degeneration • chronic fatigue syndrome • lymphoma • many cancers including breast, prostate, lung, pancreas, colon kidney • some cancers of the head and neck • Type II diabetes • obesity • Parkinson’s disease This makes sense because as mentioned, CoQ10 is part of basic energy pathways and is in particularly high concentrations in organs like the heart, kidneys and liver. Lucky for us, CoQ10 production may be lower in some people due to genetic factors and also due to age factors but it is easily supplemented. People interested in disease prevention would be wise to add increasing amounts of CoQ10 to their supplement regimen as they age. One of the effects of statins is the interference with CoQ10. The simple solution is to supplement CoQ10 if you are taking a statin. This additional CoQ10 doesn’t appear to interfere with the goal of the statin, that is lowering lipids such as LDL. This is so simple that it seems negligent to me that a doctor would prescribe a statin without clear instructions to supplement CoQ10. If you experience even a little bit of muscle soreness or even a tiny bit of cognitive decline with statin use, you are not even to getting enough CoQ10. So how much is enough? The doses in the literature runs from 50 to about 6000 mg per day. I am including a link to a Mayo Clinic summary of research recommendations for different conditions. To save a step, if you are taking a statin, a typical dose is 300 mg divided into three 100 mg doses throughout the day. If you also have conditions such as Alzheimer’s, ALS or other serious disease, 300 mg won’t be enough. http://www.mayoclinic.org/drugs-supplements/coenzyme-q10/dosing/HRB-20059019 Is there a downside to CoQ10 use? It seems to be rare but some have reported stomach upset and diabetics might have to adjust their meds as blood sugar can drop. It can also interact with some medications, including beta-blockers, some antidepressants, and chemotherapy drugs. You might need to change the amount of statins you take if you’re also taking CoQ10. If you are taking a red yeast rice product like CholestGenix, you should also supplement your CoQ10 too. You can probably find some sources of “cheap” CoQ10 but some random samplings have found that many of these cheaper supplements had virtually none of the active ingredient you were paying for.
