A little-known side-effect of jaundice is itchy hands and feet. It's due to a skin reaction to the bilirubin, the yellowish brown liver chemical that causes jaundice. Lack of appetite. An Italian study found that six to eight months before being diagnosed with pancreatic tumors, patients reported a sudden drop in their appetite and a tendency to feel full after eating very little. 10 Early Warning Signs of Pancreatic Cancer The average general health article on pancreatic cancer states flatly that this type of cancer shows no early symptoms. Most people diagnosed with pancreatic cancer are already in the advanced stage of the disease by the time it's caught, and the typical prognosis is death within five years. Only 4 percent of pancreatic cancer patients live beyond five years. In recent years, pancreatic cancer has claimed the lives of numerous celebrities and public figures, including Steve Jobs, actor Patrick Swayze, and actress Bonnie Franklin, leading many people to ask the key question, "Are there any early warning signs of pancreatic cancer?" The answer is yes, there are. But to understand these signs and symptoms of pancreatic cancer, it's important to understand where the pancreas is, and what it does. Located deep inside the abdomen, the pancreas is only 4 to 6 inches long and shaped like a tadpole. In the "tail" of the pancreas are cells that produce insulin, and tumors at this end are usually endocrine tumors. They're easier to diagnose, but are much rarer. At the other end, the "head" of the pancreas, are cells that produce digestive enzymes, and tumors at this end are called exocrine tumors. These are by far the most common, and are much more difficult to detect. However, the idea that a pancreatic tumor is asymptomatic is a bit of a myth. Dig deep into journal articles and ask patients what they remember and the result is a long list of odd signs and symptoms of pancreatic cancer to watch out for. Taken one by one, these symptoms could mean many things. But if you find yourself experiencing two or three of these early warning signs of pancreatic cancer, call your doctor and ask for a scan. Imaging techniques such as MRIs can detect pancreatic cancer some of the time, depending on the location of the tumor. Early Warning Signs of Pancreatic Cancer Diabetes, especially if it comes on suddenly. Recently, the Mayo Clinic published startling research showing that 40 percent of pancreatic cancer patients had been diagnosed with diabetes one to two years before discovering they had a pancreatic tumor. Researchers believe the diabetes is caused by tumors that simply haven't been detected yet. The problem is, diabetes is very common, and the majority of diabetes isn't pancreatic cancer, so doctors are trying to develop screening tools to tell the difference. Right now, they say family history is an important clue. If you're diagnosed with diabetes that seems to come on suddenly and you have no family history of diabetes, bring this to your doctor's attention and ask for further screening for pancreatic cancer. Yellowing of the eyes or skin. Even a small pancreatic tumor can block the bile duct in the head of the pancreas, causing bile to build up. This causes jaundice. Itchy skin, palms, and soles of feet. A little-known side-effect of jaundice is itchy hands and feet. It's due to a skin reaction to the bilirubin, the yellowish brown liver chemical that causes jaundice. Lack of appetite. An Italian study found that six to eight months before being diagnosed with pancreatic tumors, patients reported a sudden drop in their appetite and a tendency to feel full after eating very little. Changes in taste. In the same Italian study, some of the patients surveyed said they'd suddenly lost their taste for coffee, wine, and smoking. In fact, they said, they felt "disgust" for the smell and taste of coffee and alcohol. Abdominal pain. Pancreatic cancer sufferers remember this pain as a gnawing pain, rather than a sharp cramp or ache, and it radiates toward the back. A characteristic clue: the pain goes away when you lean forward. An enlarged gall bladder. The same blockage of the bile duct that causes jaundice can also cause an enlarged gallbladder, as the bile builds up behind the duct. What are the symptoms? How do you know if you have pancreatic cancer? Should you be screened? Over the next several minutes, I will give an overview of pancreatic cancer. Pancreatic cancer is the fourth leading cause of cancer-related deaths in the United States. The pancreas is the organ that sits in the central upper abdomen, is located right behind the stomach organ. The pancreas is about six to eight inches long and about two inches wide, and it extends from the head of the pancreas, which is on the right side of the abdomen, connected to the small intestine, near the gallbladder and the liver, and the pancreas extends to the tail of the pancreas, which is more on the left side of the abdomen. The pancreas is the organ that's responsible for producing certain hormones such as insulin, which regulates your blood sugar. The pancreas also produces certain digestive enzymes or digestive juices, which help us to digest fat. There are two types of pancreatic cancer. One, which comes from the endocrine glands or the hormone-producing cells such as an insulinoma. The other type of pancreatic cancer comes from the exocrine glands or from the pancreatic ducts, and this exocrine gland pancreatic cancer is the most common type of pancreatic cancer. What are the symptoms of pancreatic cancer? One is weight loss. Patients with pancreatic cancer often have early satiety, meaning they get full quickly off of just small amounts of food, or they may just lose their appetites altogether. Another symptom, abdominal pain. Patients often have a dull, aching abdominal pain in the central upper abdomen, and it often radiates to the back. Another symptom, jaundice. Jaundice occurs when the patient's skin turns yellow or the whites of the eyes turn yellow, and this is due to a blockage of the flow of bile by the pancreatic cancer. Patients who have jaundice, this yellow skin, will often also have a change in their stools. How can you be diagnosed with pancreatic cancer? If you have any of the symptoms I described, it is important that you consult your physician as soon as possible. Your physician will do a complete history and physical, and they will likely do some imaging studies such as an ultrasound of the pancreas or a CT, computed tomography, of the pancreas or a test called an ERCP, endoscopic retrograde cholangiopancreatography. If they are suspicious or if they see a mass that looks like pancreatic cancer, then they may do a biopsy, which is when they have a physician to take a small sample of the mass, and a pathologist is another type of physician who will look at that and determine if it is or if it isn't pancreatic cancer. The treatment for pancreatic cancer. The treatment really depends on the staging. If the cancer is caught early, there's a chance that you can have surgery or surgical removal of the cancer. and patients often will have an adjuvant therapy, which means they will receive chemotherapy and/or radiation, especially if the pancreatic cancer has spread. In cases of stage four or metastatic pancreatic cancer, a lot of those patients will also receive chemotherapy. However, that chemotherapy is usually just to shrink the tumor or to help to decrease the symptoms of the tumor. Stage four pancreatic cancer is usually not curable. So what are the risk factors for pancreatic cancer? Well, five to 10 percent of patients who have pancreatic cancer also have family members with pancreatic cancer. So, we do believe that there is some hereditary component. Other things that have been associated with pancreatic cancer include cigarette smoking, obesity, physical inactivity, the Helicobacter pylori infection. But quite honestly, we really don't always know the cause of pancreatic cancer. This ends my overview of pancreatic cancer symptoms, signs, and risk factors. I want you to take care of yourselves, and if you have any of the symptoms that I described, please consult your physician immediately. Those symptoms are weight loss, abdominal pain, jaundice. Unfortunately, by the time CA 19-9 reaches detectable levels, the cancer is usually no longer in the early stage, although the test is useful during treatment and for detecting recurrence. Another biomarker test, CEA (carcinoembryonic antigen), is also not sensitive enough to provide early diagnosis. While tragic early deaths like Steve Jobs' are the hard reality of pancreatic cancer, there are also people living productive lives thanks to an early diagnosis of pancreatic cancer. Such was the case with Supreme Court Justice Ruth Bader Ginsburg, who's still practicing after having a tiny pancreatic tumor surgically removed. An Italian study found that six to eight months before being diagnosed with pancreatic tumors, patients reported a sudden drop in their appetite and a tendency to feel full after eating very little. 5. Changes in taste. In the same Italian study, some of the patients surveyed said they'd suddenly lost their taste for coffee, wine, and smoking. In fact, they said, they felt "disgust" for the smell and taste of coffee and alcohol. 6. Abdominal pain. Pancreatic cancer sufferers remember this pain as a gnawing pain, rather than a sharp cramp or ache, and it radiates toward the back. A characteristic clue: the pain goes away when you lean forward. 7. An enlarged gall bladder. The same blockage of the bile duct that causes jaundice can also cause an enlarged gallbladder, as the bile builds up behind the duct. A blood test is now available for a biomarker called CA 19-9 that is released by pancreatic cancer cells. Unfortunately, by the time CA 19-9 reaches detectable levels, the cancer is usually no longer in the early stage, although the test is useful during treatment and for detecting recurrence. Another biomarker test, CEA (carcinoembryonic antigen), is also not sensitive enough to provide early diagnosis. 6 Pancreatic Cancer Symptoms You Should Never Ignore When was the last time you thought about your pancreas? Chances are, you rarely give much thought to this glandular organ, which helps your body digest food and convert it into energy. Located behind your stomach, the pancreas produces insulin and regulates your body's hormones that maintain glucose (blood sugar) levels. Pancreatic cancer is overall pretty rare, with 55,440 people in America expected to be diagnosed in 2018. (The most commonly diagnosed cancer is non-melanoma skin cancer, affecting more than 1 million people a year, followed by lung cancer, with more than 200,000 cases diagnosed annually.) But even though it’s an uncommon diagnosis, pancreatic cancer is soon expected to overtake colon cancer as the number-two leading cause of cancer deaths in the United States, says Valerie Lee, MD, pancreatic oncologist at Johns Hopkins Medicine. Why? First, because pancreatic cancer is very aggressive. “It has a high likelihood of spread, or metastasis, to other organs in the body," says Neil Woody, MD, oncologist at the Cleveland Clinic. The five-year survival rate is only 8.5 percent. But it may also be because pancreatic cancer is on the rise. Pancreatic cancer has been diagnosed at an increase of .5% a year for more than a decade. Part of that is due to increased detection, Dr. Lee says, but lifestyle factors may be playing a role as well. Increases in diabetes and obesity—two major risk factors for pancreatic cancer—may also be contributing to the rise in pancreatic cancer cases. Pancreatic cancer warning signs. Pancreatic cancer is notoriously difficult to diagnose. Only about 10 percent of pancreatic cancers have a genetic predisposition, and good screenings for the general population have been difficult to establish, says Dr. Woody. “Tests with high likelihood of detecting pancreatic cancer, such as endoscopic ultrasound, are invasive, costly, and may carry risks of complications that outweigh their use as screening tests for the average person,” he explains. What’s more, pancreatic cancer is tough to spot early, and many of the symptoms can be caused by other conditions. Plus, people usually have no symptoms until the cancer has spread to other organs. Still, if you are experiencing any of these warning signs, it’s worth making an appointment with your doctor. 1. New onset of diabetes “When people with no risk factors for diabetes (no family history, good diet, thin body type) develop new onset diabetes, or who have a history of well controlled diabetes suddenly find that it becomes difficult to control, it’s reasonable to evaluate why the pancreas may not be functioning as well," says Dr. Lee. 2. Abdominal pain "The pancreas sits close to a bundle of blood vessels and nerves called the celiac plexus that can cause pain and nerve irritation, says Dr. Lee. But the pain is usually pretty encompassing of your abdomen. "Patients feel pain from the epigastrum (the area just under the sternum) through to their back. Imagine a tire hiked up right under your chest and around your abdomen," says Kim Reiss-Binder, MD, an oncologist specializing in pancreatic cancer at the University of Pennsylvania. 3. Unexplained blood clots "Patients with pancreas cancer are particularly prone to blood clots," says Dr. Lee. While the exact cause is not known, it is believed that cancer cells reduce the proteins the body uses to prevent blood clots. “For people who do not have a reason for the development of a clot—a recent major surgery, trauma (like a car accident), hospitalization, or prolonged period where you are laying still, or a hereditary predisposition to clotting—we recommend making sure you are up to date on your cancer screening and to check in with your doctor about any other symptoms that may help to identify the source of the clotting abnormality," says Dr. Lee. 4. Eight months after undergoing a successful surgery for colon cancer, Delaware resident Thelma Klase, was diagnosed with a large cancerous tumor in her liver, thought to be metastatic colon cancer. "Jaundice is one of the clearest symptoms of pancreatic cancer," says Christopher DiMaio, MD, a gastroenterologist at The Mount Sinai Hospital in New York City. "Often, patients feel fine until one day a friend notices their eyes look yellow, then they go to the doctor and find they have advanced pancreatic cancer." Cancers that start near the head of the pancreas can block the bile duct, preventing bile from reaching the intestines, where it helps break down fats and eventually leaves the body in the stool. This bile builds up and causes jaundice, which is yellowing of the skin or eyes. It can also cause your skin to itch, according to the American Cancer Society. These are other cancer symptoms women should never ignore and cancer signs men should never ignore. You experience abdominal or back pain. Place the tip of your finger on the top of your abdomen, just below the breastbone. Then imagine your finger pointing straight back through your body to the spine. That's a common location that pancreatic cancer patients report feeling pain, says Dr. DiMaio. "The pain is hard to describe, but a dull, internal pain in this area, or radiating around the sides of your abdomen to the back, is a tip-off and you should get it checked out," he says. Cancers that start in the body or tail of the pancreas can press on nearby organs, causing pain. If the cancer spreads to the nerves surrounding the pancreas, this can cause back pain. Here are some more reasons your back might be hurting. You have dark urine or greasy stools. If your urine starts to look dark (brown or rust colored), this may signal a problem. As bilirubin, a substance made by the liver that partly makes up bile, builds up in the blood, urine darkens. Here's what other urine changes can say about your health. Clay-colored or greasy stools can also be symptoms of pancreatic cancer, as they can be caused by cancer blocking the bile duct, says Dr. DiMaio. You experience nausea or vomiting. Cancer pressing on the far end of the stomach can cause a partial blockage, making it hard for food to get through. According to the American Cancer Society, this can cause nausea, vomiting, and pain that tends to be worse after eating. All of these things, therefore, could be symptoms of pancreatic cancer. Here's what else your teeth and mouth can reveal about your health. You've recently been diagnosed with type 2 diabetes. A diabetes diagnosis doesn't mean you'll get pancreatic cancer, but it is associated with an increased risk. One Mayo Clinic study found that 40 percent of pancreatic patients were diagnosed with diabetes in the months before their cancer diagnosis. The pancreas is responsible for creating insulin, so the early stages of the tumor may affect the pancreas's ability to produce insulin, causing diabetes, says Dr. DiMaio. 8 Pancreatic Cancer Signs You Might Be Ignoring. Your skin looks yellow. "Jaundice is one of the clearest symptoms of pancreatic cancer," says Christopher DiMaio, MD, a gastroenterologist at The Mount Sinai Hospital in New York City. "Often, patients feel fine until one day a friend notices their eyes look yellow, then they go to the doctor and find they have advanced pancreatic cancer." Cancers that start near the head of the pancreas can block the bile duct, preventing bile from reaching the intestines, where it helps break down fats and eventually leaves the body in the stool. This bile builds up and causes jaundice, which is yellowing of the skin or eyes. It can also cause your skin to itch, according to the American Cancer Society. These are other cancer symptoms women should never ignore and cancer signs men should never ignore. You experience abdominal or back pain. Place the tip of your finger on the top of your abdomen, just below the breastbone. Then imagine your finger pointing straight back through your body to the spine. That's a common location that pancreatic cancer patients report feeling pain, says Dr. DiMaio. "The pain is hard to describe, but a dull, internal pain in this area, or radiating around the sides of your abdomen to the back, is a tip-off and you should get it checked out," he says. Cancers that start in the body or tail of the pancreas can press on nearby organs, causing pain. If the cancer spreads to the nerves surrounding the pancreas, this can cause back pain. Here are some more reasons your back might be hurting. You have dark urine or greasy stools. If your urine starts to look dark (brown or rust colored), this may signal a problem. As bilirubin, a substance made by the liver that partly makes up bile, builds up in the blood, urine darkens. Here's what other urine changes can say about your health. Clay-colored or greasy stools can also be symptoms of pancreatic cancer, as they can be caused by cancer blocking the bile duct, says Dr. DiMaio. You experience nausea or vomiting. Cancer pressing on the far end of the stomach can cause a partial blockage, making it hard for food to get through. According to the American Cancer Society, this can cause nausea, vomiting, and pain that tends to be worse after eating. All of these things, therefore, could be symptoms of pancreatic cancer. Here are the explanations behind 7 other common stomach pains. You develop pancreatitis. "Unexplained or chronic pancreatitis can be caused by a small tumor on the pancreas," says Dr. DiMaio. Though pancreatitis is more commonly caused by gallstones, new medications, or alcohol abuse, if you develop the condition and don't have any of those risk factors, it could be something more serious, he says. Don't miss these other signs of cancer in men that go ignored. You have poor oral hygiene. If your mouth is a mess of bad breath, inflamed gums, or loose teeth, it could be more than bad hygiene. "Pancreatic cancer patients are known to be susceptible to gum disease, cavities, and poor oral health in general," according to researchers from NYU Langone Medical Center, who conducted a study examining mouth bacteria of pancreatic cancer patients versus people without. They found that the presence of the same bacteria that has been tied to dental diseases like periodontitis (inflammation of the gums) put participants at a 59 percent greater risk of developing pancreatic cancer than those whose mouths didn't contain the bacteria. Here's what else your teeth and mouth can reveal about your health. You've recently been diagnosed with type 2 diabetes. A diabetes diagnosis doesn't mean you'll get pancreatic cancer, but it is associated with an increased risk. One Mayo Clinic study found that 40 percent of pancreatic patients were diagnosed with diabetes in the months before their cancer diagnosis. The pancreas is responsible for creating insulin, so the early stages of the tumor may affect the pancreas's ability to produce insulin, causing diabetes, says Dr. DiMaio. Here's what else you should do right after a diabetes diagnosis. You have unexplained weight loss. If you suddenly have little to no appetite, or if you're losing weight without changing your diet or exercise regimen, see your doctor; these are some of the common symptoms of pancreatic cancer, says Dr. DiMaio. The Early Warning Signs of Pancreatic Cancer Pancreatic cancer is the fourth deadliest cancer in the world. Since this cancer is hard to detect and treat, the survival rate is extremely low, hovering somewhere between 7-25%. The pancreas is part of the digestive system and is buried within the abdominal cavity (behind the stomach and in the upper part of your abdomen). Because of its location, it can be difficult to feel tumors before they’ve grown out of control. Furthermore, there’s no screening test for this type of cancer. Looking for clues to pancreatic cancer is vital for early detection and treatment. Though some of the signs and symptoms of this cancer resemble other health conditions such as cirrhosis, diabetes, or liver disease, it’s important to visit your doctor if you notice any abnormalities. Below are 8 warning signs of pancreatic cancer: 1. Sudden Onset of Diabetes. Research has shown that 40% of pancreatic cancer patients had been diagnosed with diabetes one or two years before discovering that they had a pancreatic tumor. Researchers believe that the diabetes is caused by tumors that simply haven’t been detected yet. The problem though, is that diabetes is very common, and the majority of diabetes cases will not be related to pancreatic cancer, and so doctors are trying to come up with screening tools to tell the difference. Right now, they say that family history is an important clue. If you’re diagnosed with diabetes that has come out of the blue, and you have no history of diabetes in your family, bring this to your doctor’s attention and ask for further screening for pancreatic cancer. 2. Itchy Hands and Feet. Even a small pancreatic tumor can block the bile duct, and if this occurs, bile starts to build up in the body. When there is too much bile in the blood, it starts to turn into compounds that cannot be broken down by the body. These compounds accumulate in the hands and feet, causing persistent itching sensations. 3. Yellowing Eyes and Skin. The buildup of bile mentioned above is known as jaundice, and it causes other symptoms beyond itchy palms and feet. One such symptom is yellow eyes and skin. Bile has a bright yellow pigment, and when too much of it is present in the body, it will start to show up in the whites of the eyes and through the skin. 4. Lack of Appetite and/or Changes in Taste. An Italian study found that 6-8 months before being diagnosed with pancreatic cancer, patients reported a sudden decrease in their appetite, and a tendency to feel full after eating very little. This is because the lack of digestive enzymes can shut down your whole system and confuse your stomach. The same study also found that patients suffering from a pancreatic tumor also suddenly lost their taste for coffee, alcohol, and smoking. In fact, some even declared that they felt disgusted by the smell and taste of coffee and alcohol. 5. Pale, Floating, Smelly Stools. If a pancreatic tumor prevents digestive enzymes from reaching the intestines, the result is an inability to digest fatty foods. Therefore, as a result of this excess fat, you end up with loose, smelly floating stools. Doctors say that this symptom, in particular, is an early clue of pancreatic cancer, but it is too often overlooked. 6. An Enlarged Gallbladder. The same blockage of the bile duct that causes jaundice can also cause the gallbladder to enlarge, as the bile builds up behind the duct. The good news though is that an enlarged gallbladder can be seen on imaging tests, and it is sometimes possible for a doctor to feel it during a physical exam. 7. Abdominal Pain. Those suffering from pancreatic cancer typically experience some form of abdominal pain. This is more of a gnawing pain, rather than a sharp cramp or ache, and it usually radiates towards the back. A characteristic clue of abdominal pain possibly being caused by a pancreatic tumor is the pain disappearing when you lean forward, or getting worse after you eat or lie down. 8. Unexplained Weight Loss. This symptom is common to most cancers, especially in more advanced stages. However, if a pancreatic tumor is developing, it may prevent your body from digesting food thoroughly. This means that fat and sugar might pass right through your system undigested.
