Dr. David Samadi - Prostate cancer: Surgery vs Radiation vs CyberKnife
An analysis of more than a dozen studies indicate that prostate cancer patients do better with surgery vs. radiation treatment; especially men with localized prostate cancer. Patients treated with radiation are twice as likely to die from prostate cancer, one & a half times likely to die sooner than prostate cancer patients treated with surgery & exposure to radiation may end in secondary cancers such as bladder cancer and rectal cancer. Dr. David Samadi explores the details. When men are diagnosed with prostate cancer it can be extremely confusing at times picking the right treatment option as there’s a lot of information out there. Dr David Samadi says that with any cancer, surgery should be the first line of treatment. With prostate cancer surgery patients get accurate staging – i.e. how much cancer a patient has, has the cancer spread to surrounding tissues & the PSA levels which has always got to be zero. Dr. David Samadi explains that there’s a lot of misleading advertisements indicating methods such as CyberKnife is robotic radio surgery which in fact is not but only a different form of radiation treatment. He says that patients are led to believe that radiation is a viable first line of treatment & if the cancer comes back, it is extremely difficult to perform surgery after radiation. Dr. Samadi also explain why it is extremely important to weigh in the experience of the surgeon as working with the right doctor would ensure a better and faster recovery. Dr. David Samadi’s patients have a continence percentage of 95% - 97% & regular sexual functionality in about 80% of prostate cancer patients. Some prostate cancer facts: • There are over 221,000 men diagnosed each year • Almost 21,000 die of prostate cancer • If the cancer is spread outside the prostate gland, the chances of a man living longer than 5 years becomes less than 30% • If the prostate is surgically removed, the survival rate becomes almost 100% Dr. David Samadi also clarifies if there are genetic components where certain males are predisposed to having prostate cancer, why is it important not to just look at the PSA as a number but to follow a trend pattern. It’s explained what factors have to be monitored closely & at what age / PSA levels a patient should start consider a biopsy or not. Prostate cancer is the most common cancer in men in the UK so it's important to know the facts. Dr. Vincent Gnanapragasm from Cambridge University talks us through prostate cancer treatments: Over 80% of men live for 10 or more years after a diagnosis of Prostate Cancer. I’ve come to Cambridge university hospital to speak with Dr. Vincent Gnanapragasam about some of the treatments that are available for this type of Cancer. So Vincent what are the important factors when deciding what type of treatment a person should have? The treatment options for Prostate Cancer are very much dependent on what type of cancer an individual has and also the patient themselves. So if a cancer is confined to within the prostate gland then we might be able to treat it with surgery or radiotherapy, but equally there might be a very slow growing tumour which we can just keep an eye on that. But the kind of cancer is determined by the size of it or the stage of a cancer, the type of cancer it is or the grade and also the PSA test. Now we use those 3 put together to create an idea of the risk of a particular individual dying of prostate cancer. What treatments are available? In general there is surgery to remove the prostate gland, radiotherapy where you deliver high intensity radiation beams or surveillance where we can monitor or just check on someone and defer treatment until later. So a number of different options then Vincent but how do you decide what one is best? That decision is left to the patient in consultation with his consultant or his nurse specialist and comes down to whether the patient feels that a particular type of treatment is best suited for their lifestyle and also the risks and side effects which may come about from that. And what happens when treatment is finished? After treatment is finished the patient enters a phase of very close monitoring and where they’ll be seen regularly in hospital often with a PSA test being done on a regular interval. What about for treatment of cancers that have been spread? When a cancer has spread beyond a prostate then the most common treatment is hormonal therapy and that serves to keep the cancer under control and many men can live for many years with that and if that should fail there are actually other drugs that can be used, chemotherapies which again can be very effective. Dealing with cancer is never easy and this is especially so when deciding about treatments. But of course it helps if you know the facts. Lester, M.D., F.A.C.R.O., a board-certified radiation oncologist, at Central Florida Cancer Care Center breaks down the costs of prostate cancer treatment prostate and explains all the factors that contribute to the expense. “The cost for prostate cancer treatment is pretty significant when one considers that you're looking at about $20-25,000 whether you have surgery or radiation,” says Dr. Lester. “Usually, patients don't make a decision based on cost because both treatments are standard and are covered by insurance. Why is radiation is so costly? Let's take an example: the treatment room the patient is in has to protect the personnel and the lay public who are not in the room from the radiation because only the patient can be in the room.” “The treatment room itself is 8 to 10 feet of poured concrete at the primary beam. So you're looking at $800,000 to $1 million just for the treatment room and about two to three million dollars for the treatment machine plus all the support services.” “Radiation is a very high-end capital cost procedure,” Dr. Lester continues. “And now we see surgery with the use of robots, such as the laparoscopic robot, which is also a very impressive, high-tech piece of equipment. I did a lot of research, using only reliable websites like the ones you see on your screen, and I'll summarize what I learned about prostate cancer stages in the next minute or so. Prostate cancer stages indicate how far the cancer has progressed. A lower stage means cancer cells are confined to the prostate. A higher stage is assigned when the cancer has grown to invade nearby tissue or spread to distant areas of the body. Prostate cancer usually grows slowly and in the beginning, confines itself to the prostate gland. Some types of prostate cancer may not even need to be treated, depending on the age of the patient. But other types are more aggressive, can spread quickly, and require treatment. There are four stages. Stage I. At this stage, the cancer is located only in a small area of the prostate. Stage II. Cancer cells may still be small but could possibly be aggressive, depending on the type. Stage II could also, however, include a larger area that ha grown to involve both sides of the prostate gland. Stage III. At this stage, the cancer has spread beyond the prostate to nearby tissues. Stage IV. The cancer is invading nearby tissues, bones or organs, which may include the bladder, lymph nodes, or even those as far away as the lungs. Your doctor assigns your cancer a stage after testing. Of course, cancer in the early stages, when it's still confined to the prostate, has a better chance of successful treatment and a longer survival rate. Many men will live with their cancers and cures are eminently possible in many cases. But for those who are struggling to cope there are support services in your own hospital or in your GP practice or indeed nationally which they should go to. Now as a result for treatment for prostate cancer I understand there can be some urinary symptoms? Different treatments for prostate cancer can cause some trouble with urinary symptoms. With surgery you might get incontinence or leakage of urine. With radiotherapy you may get irritability of the bladder, making someone want to pee more often. And those kind of things can be managed with treatments and medication. The key thing is to seek help early when that happens. And what about other physical issues that may come up for some people? A lot depends on the kind of treatment, radiotherapy may for instance cause bowl symptoms and diarrhea transiently. And men who get hormone therapy may find that they become very tired, they put on weight and lose their vitality. And understandably one of the things most people want to know about is how prostate cancer effects their sex life? Unfortunately the prostate being where it is has the nerves for erections wrapped around it and there is no doubt that treatments will cause problems with erectile failure or disfunction. But there is things that can be done to help with this. Medication for example, artificial devices and all these things are available to men who need it after treatment. So can men have children after treatment? So if you have surgery the tubes as they were will be disconnected. However you can still retrieve sperm for fertility that was necessary. In radiotherapy in general that doesn’t affect fertility, the hormone therapy can suppress that for some time. Coping with prostate cancer isn’t easy but it’s good to hear that if you are having any problems there is lots of help available. Here are two questions men may want answered at the time of diagnosis and during and after prostate cancer treatment. No question should be off limits as a man’s urologist will be able to answer them thoroughly giving him the best advice he needs. 1. Will I be able to have an orgasm after prostate cancer treatment? The answer to this is a resounding yes. Almost all men are able to achieve an orgasm following prostate cancer treatment. Prostate cancer treatments may damage nerves and blood vessels that are important for achieving an erection but are not necessarily important for reaching an orgasm. One thing a man should know is that the prostate is the gland that produces most of the fluid that makes up semen. Once the prostate has been removed by surgery or destroyed by radiation, a man’s orgasm will be nearly or completely dry and are known as a “dry orgasm” or retrograde ejaculation. They can be just as pleasurable and fulfilling as they used to be before prostate cancer. 2. What will my sex life be like during and after prostate cancer? This is a question often likely looming large in most men’s minds. You would think beating back prostate cancer would be the more pressing issue but sexuality is a very important part of our being. Men should realize that sex most likely will be different but again, it doesn’t mean it will be worse. It helps to break it down into three categories of how prostate cancer affects a man’s sexuality – desire, erectile dysfunction, and the third, orgasm, which has already been addressed previously. · Sexual desire Sexual desire is what pulls the trigger in the initiation of sex. Does prostate cancer affect a man’s desire for sex – yes it can. The way it does includes the following: · Concern over his performance · The psychological impact of a cancer diagnosis · Fatigue – almost all people going through cancer treatment will have fatigue · His self-image after prostate surgery · Fear of recurrence, progression, and death · Low testosterone levels due to treatment If any of these issues are resulting in a diminished desire for sex, there are several things men might consider to help them deal with this issue: · Joining a prostate cancer support group offered through his doctor’s office or hospital · Joining an online prostate cancer support group · Seeking individual help with a counselor · Erectile dysfunction Any kind of prostate cancer treatment (radiation, surgery, cryotherapy or hormone therapy) may lead to the inability to achieve and or maintain an erection. This condition may last only for a short time or it could be long-term. However, many men who undergo treatment for prostate cancer see little or no effect to their erectile dysfunction.
