This video is all thyroid cancer treatments including what treatments are available if you have thyroid cancer, the consequences of these therapies on your life, and we'll also discuss some important factors which help determine your prognosis. It's important to realize, though, that the majority of thyroid cancer cases are caused by the least aggressive form of thyroid cancer. This, combined with the fact that the most common therapy to treat thyroid cancer is the removal of the thyroid gland has helped increase the 5-year survival rate of thyroid cancer to around 97 to 98%. When it comes to thyroid cancer you may not have a choice as to which therapy is ideal for you, but it still helps to understand each treatment and how it impacts your body. Treatment options available for thyroid cancer include: #1. Surgery: - By surgery, I am referring to a procedure known as a complete thyroidectomy. This surgery results in the entire removal of your thyroid gland (well close to the entire amount) and it is the most common treatment for thyroid cancer. Removing your thyroid, though, can sometimes cause side effects afterward such as weight gain or fatigue if you are not dosed appropriately. #2. Radioactive iodine ablation: - Radioactive iodine is sometimes combined with a thyroidectomy to help kill off any tissue which is left over after your surgery. Radioactive iodine is a procedure which specifically targets and destroys thyroid gland tissue. It's helpful to use in conjunction with surgery because sometimes thyroid gland tissue can exist outside of the normal anatomy. Using RAI can help kill off extra tissue especially if cancer has spread. #3. Chemotherapy and radiation: - Chemotherapy isn't used very often for thyroid cancer because most cancers can be treated with surgery and with RAI. But, some cases of aggressive thyroid cancer may require the use of chemotherapy and radiation therapy. #4. TSH Suppression: - Lastly, another technique used to treat thyroid cancer is the use of TSH suppressive therapy. The goal here is to take high enough doses of thyroid hormone to suppress the natural production of TSH or thyroid stimulating hormone. This hormone is stimulating to thyroid gland tissue and can theoretically stimulate thyroid cancer cells as well. You can suppress the normal production of TSH by taking high doses of thyroid medication and this is sometimes used for patients after surgery. Each person is different so the exact treatments that you need for your cancer will depend on the type of cancer you have, how aggressive it is, the stage at which it was diagnosed, your age and other important factors. The thyroid is a gland in the body which produces two hormones T3 and T4. Pituitary is another gland which is the master gland of the body which secretes thyroid stimulating hormone which stimulate the thyroid to release T3 and T4. If the thyroid is functioning excessively T3 and T4 increases in the body and pituitary gets a message. So the TSH is secreted less and thyroid is less stimulated and T3, T4 levels come back to normal. The condition where T# T4 is elevated and TSH is less is called hyperthyroidism. Normal TSH levels varies from 0.3 to 4.2. There are some kits which quote as 0.3 to 5.2 as normal. If the value is between this we find that thyroid is not having any major issue. Patient should not be treated with any medicine at that point. If the TSH values less than 2 or 1.5 most of them later on in life will turn to hypothyroid subjects. Suppose TSH is low for a longer time there is less stimulation of thyroid and these people gradually can have hypothyroidism. In this condition the patient should not take any medication but should take care of the diet. You should be aware of the goitrogenic foods. Cabbage, cauliflower, broccoli, processed and preserved food and refined sugar are known to be not good for the thyroid gland. Cranberry and salt containg foods are very effective in hyperthyroidism. Certain homeo medicines like throidinum, calcaria iode, sulphur iode, naturum iode are found to be very effective in thyroid functions. Iodium and spongia are other two major medicines used in the function of thyroid gland. Even by examining the hair we can tell whether this person has got a thyroid problem or not.

A Famous Tibetan Exercise for the Treatment of the Thyroid Gland Through the hormones it produces, the thyroid gland influences almost all of the metabolic processes in your body. Thyroid disorders can range from a small, harmless goiter (enlarged gland) that needs no treatment to life-threatening cancer. The most common thyroid problems involve abnormal production of thyroid hormones. Too much thyroid hormone results in a condition known as hyperthyroidism. Insufficient hormone production leads to hypothyroidism. This is the famous Tibetan exercise for the treatment of the thyroid gland. This exercise provides an increased blood flow to the gland. Consequently, your thyroid gland will be purified. STEP 1 – Rub your palms well in order to heat them up. Then gently place the palms on the thyroid gland and hold them there for maybe ten seconds. STEP 2 – Three times in a row you need to swallow your saliva or drink three sips of water. STEP 3 – Slowly exhale. As you exhale, bend your head forward and press your chin to your chest as hard as you can. Hold your chin a few seconds without breathing. When you begin to breathe, lift your chin up and throw your head back. Hold your head in this position for a while. Once you start exhaling, press your chin against your chest again. STEP 4 – While your chin is on your chest, slowly turn your head to the left and right, never taking the chin off the chest. In this way, an excellent massage is performed on the thyroid. Repeat steps 1, 2, 3, 4. Your head rises to inhaling and lowers with the exhalation. And so on, for ten times. The exercise can be done in a sitting or standing position. It is necessary to do it often, as it leads to improving the state of the thyroid gland. It is especially useful for hypothyroidism. As with other Tibetan exercises, the psychological component is also important. Do the exercises with love for your body and your diseased thyroid gland.

It is wrapped around the trachea, just below the thyroid cartilage –the Adam’s apple. The two major hormones of the thyroid are triiodothyronine, T3 and thyroxine, T4. The numbers 3 and 4 indicate the number of iodine atoms present in a molecule of each hormone. T3 and T4 are collectively referred to as THYROID hormones. Thyroid hormone secretion is under control of thyroid-stimulating hormone, TSH, from the anterior pituitary. TSH, in turn, is induced by thyrotropin-releasing hormone, TRH, produced by the hypothalamus. The amount of circulating thyroid hormones is regulated by a negative feedback loop: when their levels are too high, they SUPPRESS the production of TSH and TRH, consequently INHIBITING their own production. Thyroid hormones act to INCREASE the body’s metabolic rate. They stimulate appetite, digestion, breakdown of nutrients and absorption. They also increase oxygen consumption, raise the breathing rate, heart rate and contraction strength. As a result, the body’s HEAT production is INCREASED. Thyroid hormone secretion usually rises in winter months to keep the body warm. Thyroid hormones are also important for bone growth and fetal brain development. There are 2 major groups of thyroid problems: HYPOthyroidism: when the thyroid does NOT produce ENOUGH hormones, resulting in a LOW metabolic rate, combined with SLOW respiratory and cardiovascular activities. Common symptoms include fatigue, weight gain despite poor appetite, cold intolerance, slow heart rate, heavy menstrual bleeding and constipation. Iodine deficiency and Hashimoto's thyroiditis are the most common causes. Hashimoto's thyroiditis is an autoimmune disease in which the thyroid gland is gradually destroyed by the body’s own immune system. Hypothyroidism, especially when caused by iodine deficiency, may lead to swelling of the thyroid gland, known as GOITER. In an attempt to fix the low levels of thyroid hormones, the pituitary produces MORE TSH to further stimulate the thyroid gland. The thyroid, while UNable to make hormones WITHOUT iodine, responds to TSH by GROWING in size. Hypothyroidism is managed with thyroxine hormone replacement. HYPERthyroidism: when the thyroid gland produces TOO MUCH hormones, resulting in a TOO ACTIVE metabolism, together with respiratory and cardiovascular rates that are HIGHER than necessary. Common symptoms include irritability, insomnia, weight loss despite good appetite, heat intolerance, heart racing and diarrhea. Hyperthyroidism is most commonly caused by Graves' disease, another autoimmune disorder characterized by presence of an antibody, called thyroid stimulating immunoglobulin, TSI. TSI, similar to TSH, stimulates the thyroid gland to produce hormones. Unlike TSH, however, TSI is NOT regulated by negative feedback mechanisms, leading to UNcontrolled production of thyroid hormones. TSI also stimulates the thyroid gland to grow, which MAY lead to formation of a goiter. Hyperthyroidism may be managed with drugs that suppress thyroid function, radioactive iodine that selectively destroys the thyroid gland, or surgery that removes part of the gland.

This gland secretes hormones that govern many of the functions in the body, such as the way the body uses energy, consumes oxygen and produces heat. Thyroid disorders typically occur when this gland releases too many or too few hormones. An overactive or underactive thyroid can lead to a wide range of health problems. Hyperthyroidism: Remember, whenever you see: HYPER infront of a word - this means too much of. Hyperthyroidism - this is when the thyroid gland is producing too much thyroid hormone. Causes: There are several causes of hyperthyroidism. Most often, the entire gland is overproducing thyroid hormone. Less commonly, a single nodule is responsible for the excess hormone secretion. The most common cause of hyperthyroidism is Graves disease - this is an autoimmune disease where the body makes an antibody called thyroid-stimulating immunoglobulin, this causes the thyroid gland to make too much thyroid hormone. Thyroiditis - can be another cause, this is inflammation of the thyroid. Remember whenever you see the words “itis”at the end of a word, this means infection. Functional thyroid tissue producing an excess of thyroid hormone occurs in a number of clinical conditions. Hyperthyroidism may be asymptomatic or present with significant symptoms. a mnemonic to help you remember the potential symptoms of hyperthyroidism is SWEATING. The signs and symptoms can include: Sweating, Weight loss, Emotional instability, Appetite increased, Tremor, Irritability, Nervousness, Gastrointestinal problems. More frequent bowel movements may occur, but diarrhea is uncommon. Treatment for hyperthyroidism usually involves medication to reduce the amount of hormones produced by the thyroid. We will go over the medical treatments in a bit. Hypothyroidism - which is also called myxedema Remember - whenever you see: Hypo in front of a word, this means “not enough of.” Hypothyroidism is a thyroid disorder that occurs when the thyroid does not produce enough hormones, which is the opposite of hyperthyroidism. People with hypothyroidism often have no or only mild symptoms. Numerous symptoms and signs are associated with hypothyroidism, and can be related to the underlying cause, or a direct effect of having not enough thyroid hormones. Some of the symptoms of hypothyroidism can be easily remembered by the mnemonic MOM’S TIRED which includes: Memory loss, gOiter, Menorrhagia - which means abnormally heavy bleeding at menstruation, Skin dryness, Tiredness Intolerance to cold, Restlessness, Energy levels are low and Depression. This condition can be treated using a drug called T4. Most patients must stay on T4 for their entire lives, and must be closely monitored by physicians. T4 is the synthetic form of the thyroid stimulating hormone - It is made by many different pharmaceutical companies, sold under names like: Synthroid, Levothyroxine Sodium and Levoxyl to name a few. Causes: Most of the time Hypothyroidism is caused by inadequate function of the gland itself which is known as primary hypothyroidism. It could be caused by not enough stimulation by thyroid-stimulating hormone - which is known as central hypothyroidism. Primary hypothyroidism is much more common than central hypothyroidism. Iodine deficiency is the most common cause of primary hypothyroidism. In areas of the world with sufficient dietary iodine, hypothyroidism is most commonly caused by the autoimmune disease Hashimoto's thyroiditis.

It is a disease in which cells grow abnormally and have the potential to spread to other parts of the body. Symptoms can include swelling or a lump in the neck. Cancer can also occur in the thyroid after spread from other locations, in which case it is not classified as thyroid cancer. Risk factors include radiation exposure at a young age, having an enlarged thyroid, and family history. There are four main types – papillary thyroid cancer, follicular thyroid cancer, medullary thyroid cancer, and anaplastic thyroid cancer. Diagnosis is often based on ultrasound and fine needle aspiration. Screening people without symptoms and at normal risk for the disease is not recommended as of 2017. Treatment options may include surgery, radiation therapy including radioactive iodine, chemotherapy, thyroid hormone, targeted therapy, and watchful waiting. Surgery may involve removing part or all of the thyroid. Most often the first symptom of thyroid cancer is a nodule in the thyroid region of the neck. However, up to 65% of adults have small nodules in their thyroids, but typically under 10% of these nodules are found to be cancerous. Sometimes the first sign is an enlarged lymph node. Later symptoms that can be present are pain in the anterior region of the neck and changes in voice due to an involvement of the recurrent laryngeal nerve. Thyroid cancer is usually found in a euthyroid patient, but symptoms of hyperthyroidism or hypothyroidism may be associated with a large or metastatic well-differentiated tumor. Thyroid nodules are of particular concern when they are found in those under the age of 20. The presentation of benign nodules at this age is less likely, and thus the potential for malignancy is far greater. After a thyroid nodule is found during a physical examination, a referral to an endocrinologist or a thyroidologist may occur. Most commonly an ultrasound is performed to confirm the presence of a nodule and assess the status of the whole gland. Measurement of thyroid stimulating hormone and anti-thyroid antibodies will help decide if there is a functional thyroid disease such as Hashimoto's thyroiditis present, a known cause of a benign nodular goiter. Measurement of calcitonin is necessary to exclude the presence of medullary thyroid cancer. Finally, to achieve a definitive diagnosis before deciding on treatment, a fine needle aspiration cytology test is usually performed and reported according to the Bethesda system. Thyroid cancers can be classified according to their histopathological characteristics. The following variants can be distinguished (distribution over various subtypes may show regional variation): Papillary thyroid cancer – often in young females – excellent prognosis. May occur in women with familial adenomatous polyposis and in patients with Cowden syndrome. Newly reclassified variant: noninvasive follicular thyroid neoplasm with papillary-like nuclear features is considered an indolent tumor of limited biologic potential. Follicular thyroid cancer – occasionally seen in people with Cowden syndrome. Some include Hürthle cell carcinoma as a variant and others list it as a separate type. Medullary thyroid cancer – cancer of the parafollicular cells, often part of multiple endocrine neoplasia type 2. Poorly differentiated thyroid cancer Anaplastic thyroid cancer is not responsive to treatment and can cause pressure symptoms. Others Thyroid lymphoma Squamous cell thyroid carcinoma Sarcoma of thyroid Thyroidectomy and dissection of central neck compartment is initial step in treatment of thyroid cancer in majority of cases.Thyroid-preserving operation may be applied in cases, when thyroid cancer exhibits low biological aggressiveness in patients younger than 45 years. If the diagnosis of well-differentiated thyroid cancer is established or suspected by FNA the surgery is indicated, whereas watchful waiting strategy is not recommended in any evidence-based guidelines. Watchful waiting reduces overdiagnosis and overtreatment of thyroid cancer among old patients. Radioactive Iodine-131 is used in patients with papillary or follicular thyroid cancer for ablation of residual thyroid tissue after surgery and for the treatment of thyroid cancer. Patients with medullary, anaplastic, and most Hurthle cell cancers do not benefit from this therapy. Sorafenib and lenvatinib, are approved for advanced metastatic thyroid cancer. TSH shows how much hormone is coming into your body, that can be from what your body makes or what you take in a pill. TSH scores in healthy people are different from others. Here are a couple other test that can be indicators of thyroid health. It is also good to check thyroid antibodies. If your body is attacking your thyroid, the gland typically gets diseased. So, you want to see if your body is attacking it. There is the thyroglobulin antibody (TG) and the thyroid peroxidase antibody (TPO). There is also thyroglobulin which is not the antibody, just thyroglobulin. Thyroglobulin is a good measurement on how irritated or inflamed your thyroid is. You also want to check reverse T3. Reverse T3 is a by-product of your body getting rid of T4, and this can be abnormal, as well. The last thing I encourage for those who suspect thyroid disease is to get an ultrasound. There are many times when patients’ labs are normal, or they don’t have measurable antibodies, yet an ultrasound shows they have nodules, goiters, calcifications, clear signs of Hashimoto's. In terms of the labs, the one test that has the biggest difference in ranges from healthy populations to populations typically tested is the TSH test. Some argue that the free hormones (like the free T3) should be on the higher end of the range. I do not see data to support that. The healthy populations show a large range of T3 and a large range of T4. They are not all in the upper end of the range. The TSH scores in the healthiest populations are at the lower end of the range. So, being on the lower end of the range means you’re further away from being hypothyroid. Specifically, numbers above 1.9 are pretty suspicious. If your TSH is above 1.9, and you have some strong symptoms and some structural abnormalities, that's thyroid disease. Low TSH: 3 Causes and What it Means if you are Hypothyroid

Today we are talking about what to do if you have a low TSH, what it means for your body, what causes it and what to do. There are 3 main causes of a low TSH that you should know about: 1. Thyroid medication (You are taking too much) → Common 2. Hyperthyroidism (Your body is producing too much) → Common 3. Pituitary/Hypothalamic dysfunction (Your body can’t make it) → Not very common We are going to focus on the first one today, meaning what happens if you have a low TSH due to taking thyroid medication, but you should realize that there are other causes of a low TSH. So, how does the TSH work in your body? TSH is produced by the pituitary gland and its job is to tell your thyroid to produce thyroid hormone. It's called thyroid stimulating hormone for a reason because it stimulates your thyroid gland to produce T4 and T3. A low TSH means that your body is sending the signal to produce less thyroid hormone from your thyroid gland. This can be because you are taking thyroid medication or because your body is producing too much thyroid hormone on its own. Lastly, it could be because your body isn't capable of producing TSH because of damage to the pituitary gland. The most common concern I see among thyroid patients has to do with having a low or normal TSH but still remaining symptomatic so, to help understand this dilemma, I've created 3 questions that we need to answer: #1. Question: How can you have a low TSH while taking thyroid medication but still feel poorly? This has to do with the fact that it's possible to have a low TSH but still have low free T3 and T4 hormones in the body. Studies have shown that simply taking T4 medication is not enough to normalize free thyroid hormone levels even though it may normalize the serum TSH. #2. Question: Can you have a low TSH and not be hyperthyroid? Yes! Hyperthyroidism caused by your own body producing too much thyroid hormone is not the same thing as taking too much thyroid medication (although it can be). There are instances where you can take enough thyroid hormone to suppress or lower the TSH but still have low free thyroid hormone levels. #3. Question: What should you do if you have a low TSH, take thyroid medication and feel poorly? The best thing to do is to look beyond the TSH and look into other lab tests including free T3, Free T4 and Total T3. Let’s say the liver is dysfunctional or the gallbladder is not working and you can’t convert this hormones because T4 cannot convert to T3 unless the liver and the gallbladder is really healthy. But I want to give you the basics first, thyroid stimulating hormone is made by the pituitary gland in your brain and its sent down to the thyroid to basically tell the thyroid to make its hormones. If the thyroid stimulating hormone is high that means the thyroid is low, it is a hypo and it is not connecting because it can start increasing the communication to try to drive it through the same thing with insulin. When the insulin is high, it’s because the receptors are resisting it and it is not working so it raises it same thing with right here. When TSH is low usually it is a hyperthyroid that is making too much so it will turn this hormone down to zero. If it is high, it is hypo. If it is low, it is hyper. T4 and T3 represents the number of iodine molecules and that’s what the difference is and T4 is the inactive thyroid hormone and T3 is the active form but there has to be a conversion in here. To convert these hormones, you need selenium. To make hormones you need iodine and zine, these are trace minerals that you need. DR. See all the Healthcast at https://www.biobalancehealth.com Thyroid replacement is a difficult process primarily because doctors have been trained for decades to make a decision about thyroid replacement by looking at just one lab, TSH. TSH is the thyroid-stimulating hormone that stimulates the thyroid to make T3 and T4. TSH is from the pituitary and is not a measurement of the REAL thyroid hormone, but the hormone that stimulates the thyroid, not the thyroid hormone itself! Doctors were taught this method of thyroid replacement, that a high TSH means low thyroid and a low TSH means a high thyroid activity, but that is rarely a dependable way to measure true thyroid activity. Most doctors don’t question this method and keep making the same mistake, by following the advice of their older peers. Doctors who know that thyroid testing has become more accurate and repeatable in the last 40-50 years should question why we still use an undependable method of judging the health one of the most important hormones to human endocrine activity. Doctors should therefore discard the archaic method of diagnosing thyroid hormone dysfunction and order blood levels of the actual thyroid hormones T3 and T4 in addition to the TSH. Mystifyingly the practice of Endocrinology has clung to the wrong test to measure whether a patient needs thyroid hormone replacement for decades without even questioning the outdated practice! Let me compare the way we diagnose Thyroid deficiency with Testosterone deficiency. Earlier in this book we described how doctors diagnose testosterone deficiency by measuring the hormone in question, testosterone, and the active portion of that hormone (free testosterone). By testing these two tests of the actual hormone doctors make a decision about whether a man needs testosterone replacement, based on whether the actual hormone in question is high/low, or normal compared to young healthy adults. If we were to measure a man’s need for testosterone by only measuring the stimulating hormone, LH, we would be wrong more times than we are right.
