The nerve passes through the supinator muscle of the forearm, found on the outside of the elbow. The nerve becomes entrapped at the proximal edge of the supinator. Injury of the nerve may also be associated with fractures or dislocations of the radial head or the elbow joint. The patient will be able to perform wrist extension, but no finger extension. Patient with a proximal radial nerve injury will lose wrist extension and finger extension including the thumb. 4-Unable to do OK sign The anterior interosseous nerve branches from the median nerve just below the elbow and supplies the deep muscles on the front of the forearm. My speciality is orthopedics and spine surgery And I'm a neurosurgeon We work together as a team treating spinal disorders and performing surgery at Monterey Spine and Joint, our private practice in Monterey, California. We would like to give you a simple overview of a condition called cervical myelopathy. The term myelopathy comes from the latin root "myelo" meaning spinal cord and "pathos" meaning a disease of. Therefore, cervical meylopathy is a disease of the spinal cord. The symptoms associated with myelopathy most commonly include a sense of numbness in the finger tips, clumsiness with the hands such as difficulty buttoning buttons, and difficulty with feeling balanced when walking. These symptoms often develop so slowly that the person is not aware of how severe they truly are. Many times the patient also complains of neck pain, but not always. These symptoms are not unique to cervical myelopathy and can be caused by many other conditions. To figure out whether or not these symptoms are related to a problem with the spinal cord, an mri of the cervical spine - the neck, from the base of the skull to the shoulders is very useful and is a key part of confirming the diagnosis. On an mri, in patients suspected of having cervical myelopathy, we look for a finding called "spinal stenosis" Spinal stenosis is a medical term that describes narrowing of the space available for the spinal cord and nerve roots in the spinal canal. Specifically, when we are talking about cervical myelopathy, we are looking to see if the spinal cord has enough space to pass through the spinal canal. Narrowing of the space for the spinal cord is usually caused by degenerative changes such as disc protrusions and facet hypertrophy due to arthritis of the spine that occurs naturally with age. While almost everyone develops arthritic changes visible on the mri, only a minority will develop spinal stenosis that requires surgical treatment Spinal stenosis encompasses a spectrum from mild to severe. At the mild end, are the normal arthritic changes that occur in almost everyone. On the severe end of the spectrum, there is often a finding called "T2 signal change" suggesting that there is swelling or bruising of the spinal cord because it is being severely compressed. these patient's usually have severe symptoms. Now, Lets look at a couple of MRI images of the cervical spine - one that is normal, one that shows mild stenosis, and one that shows severe stenosis This first image is a normal MRI scan of the neck. INSERT PICTURE 1 In this image there is normal alignment of the bones and the disks of the spine and there is plenty of space around the spinal cord inside the spinal canal. This space contains a fluid called cerebrospinal fluid that bathes the spinal cord. The cord floats in this fluid and on the MRI scan, the cord has a normal, consistent dark appearance. This is an MRI scan of a patient with cervical stenosis. INSERT PICTURE 2 In this image, the disks are degenerated and bulging in towards the spinal canal and the canal is narrowed by the arthritic process that occurs naturally with age. This patient has cervical stenosis and While there is narrowing of the spinal canal, there are no signs of significant compression of the spinal cord. in this image, the degree of cervical stenosis is severe INSERT PICTURE 3 Like the previous image, the disks have degenerated and they are bulging in towards the spinal canal. However, in this image, the spinal cord has been bruised by the constant pressure. This shows up as a bright white signal in the center of the spinal cord and this indicates that the spinal cord is in trouble. This is a condition that should be evaluated by a spinal expert as soon as possible. Cervical myelopathy is a diagnosis based on three things. #1 -- the patients complaints of hand clumsiness, numbness, and imbalance. #2 the MRI scan shows cervical stenosis and compression of the cord. . and finally #3 physical findings on a neurological exam confirm the diagnoses. If you have the signs and symptoms of cervical myelopathy you should be examined by a physician such as a neurosurgeon, orthopedic spine surgeon, or neurologist. The treatment of cervical myelopathy is surgical. The goal of surgery is to decompress the spinal cord and to allow it to function normally. Time is of the essence, If the condition is treated early enough.... the patient's symptoms often improve. However, we know that the natural history of cervical myelopathy is one of a gradual stepwise worsening with periods of relatively stability. On average, one in 10 adults suffer from this condition, and over one million school-aged children in the country have RLS. Symptoms include an overwhelming urge to move the legs, and aching, itching, throbbing, and even creeping sensations on the legs at night. Not many physical tests can be done to properly diagnose this frustrating condition, but there are some highly likely causes that are thought to trigger and aggravate restless legs syndrome. In an exclusive look below, we explore some of these causes, and go through a few simple ways you can make changes to your lifestyle, to better remedy your symptoms. From spending time outside to cutting out harmful substances, these tips will undoubtedly help soothe your symptoms. It’s tricky to pin down the exact definition and triggers of RLS, but generally, it can be considered as a type of sleep disorder. It occurs when a person is resting, relaxing, or sleeping, and normally gets worse as the night goes on. Most people with RLS have trouble falling and staying asleep, and according to the National Institute of Neurological Disorders and Stroke, the condition often leads to extreme exhaustion and fatigue during the day. In the worst cases, RLS can lead to impaired memory and even depression. Though there are no specific tests to diagnose restless legs syndrome, doctors usually observe the symptoms and other possible existing health conditions to determine the severity of RLS. In many cases, genetics play a role in RLS. According to the NIH, many people with RLS also have family members who suffer from the disorder. One of the known factors associated with RLS is that of chronic diseases. WebMD states that symptoms of certain medical conditions including Parkinson’s disease, kidney failure, peripheral neuropathy, and diabetes, among others, are known to overlap with symptoms of RLS. Some medications are known to worsen the symptoms of RLS. Anti-nausea drugs, antidepressants, allergy medications, and antipsychotic drugs all contain some levels of sedatives and antihistamines, making them prime suspects for affecting the nerves during sleep, according to NIH. In some cases, RLS symptoms dissipate after patients stop taking these medications — but remember that it’s always crucial to speak with your doctor first, before doing so. Some women note that they have experienced symptoms of RLS during their pregnancy, particularly during the last trimester, according to NIH. According to the Journal of Midwifery & Women’s Health, about 26 percent of pregnant women in the country have RLS, starting in the third trimester. This could be caused by a range of factors, including mineral deficiencies, hormonal changes, or imbalances in their dopamine levels, according to Healthline. On the bright side, RLS symptoms disappear almost completely within a month after delivery. RLS can probably also be caused by the lack of sleep, insomnia, or fluctuations in sleep patterns. Morning stiffness Morning stiffness is often an early sign of arthritis. Stiffness that lasts for a few minutes is usually a symptom of a degenerative form of arthritis. Stiffness that lasts for several hours is generally a symptom of inflammatory arthritis and is typical of RA. You may also feel stiffness after any period of prolonged inactivity like napping or sitting. Joint stiffness Stiffness in one or more of the smaller joints is a common early sign of RA.
