Cheryl Buck’s uterine cancer survivor story really began before she was ever diagnosed with uterine cancer. In 2007, she was reading a book about a 16-year-old boy who had cancer. She didn’t know it at the time but the story would soon hit home. In the book, she was intrigued by the mention of a Dr. Max Gerson, whose specialty was treating cancer naturally. Right then, she decided that if she ever got cancer, she would follow his advice. Fast forward a year; Cheryl had an ultrasound on her body and was told she had a lesion on her breast. She knew right then and there she would use the methods she learned from Dr. Gerson. She went to an underground Gerson clinic in CA and learned the Gerson therapy protocol. After returning home she followed it for a period of time, and felt better than she ever had. However she eventually stopped. Seven years later Cheryl noticed that she was spotting. She found a gynecologist and that’s when she learned she had uterine cancer. Like so many, her doctor wanted her to rush her into surgery, radiation, and chemotherapy. Short video of my friend and constant companion for 3-4 days at MD Anderson Cancer Center the internal radiation machine. Treated me for cervical cancer after 5-6 weeks external radiation therapy. Posted to give others battling cervical cancer an idea of what the machine does and sounds like running. Treatments were every 45 minutes for about 10 minute intervals. Side effects lots of gas (do NOT drink carbonated beverages in the hospital), vaginal pain (the sew in the wires), and terrible diarrhea.

What is Brachytherapy? It is an internal radiation therapy used for treating cancer. How does Cancer develop in the patient? The nucleus controls the growth and division of cells through the instructions coded in the D.N.A. In usual cases, if the D.N.A in a cell is damaged, it either instructs the cell to repair itself or to die off. In the case of Cancer, the part of the D.N.A that controls cell division is damaged and this prevents the cell from repairing itself or dying. The damaged cells instead divide uncontrollably to form cancer cells. A tumor forms as they multiply. How is Brachytherapy performed? In Brachytherapy, radioactive, pellets, ribbons, or wires are placed into or near the cancerous tumor. Camille Grammer ends chemotherapy and radiation for endometrial cancer

Camille Grammer is doing well after completing chemotherapy and radiation treatments for endometrial cancer. "I rang the bell at MD Anderson Cancer Center, which marks that I have completed my chemo and radiation treatments. YAY," Grammer tweeted Jan. 3. The former "Real Housewives of Beverly Hills" star underwent a radical hysterectomy for early-stage endometrial cancer in October 2013. Camille, the ex-wife of actor Kelsey Grammer, decided to get the surgery because of a genetic predisposition. She underwent genetic testing several years ago and was advised to get a hysterectomy back then, but held off until last year. Camille's mom is a survivor of Stage 3 ovarian cancer.

Endometrial Hyperplasia there is proliferation of the endometrium which becomes very thick. Primarily the hormonal trigger for endometrial proliferation is estrogen. The hormone that blocks the effects of estrogen on endometrium is progesterone. ETIOLOGY Obesity can cause this because there is increased aromatase which converts more testoserone to estrogen. Polycystic Ovarian Syndrome (PCOS) which has a high estrogen and testosterone and there is no ovulation and therefore there is no corpus luteum and therefore no progesterone. Tumors that release estrogen releasing. Hormone replacement therapy in post menopausal women. Two types of hyperplasia, simple and complex. Normally there is small amounts of gland and lots of stromal tissue. In simple there are more glands, but not touching each other. In complex the glands are touching each other. In Atypia there are abnormal nucleus which is really large and it is a very bad sign. Complex atypia has a 30% chance of become endometrial cancer. Sign and symptoms is abnormal heavy bleeding due to lack of ovulation. Diagnosis is done initially by doing a transvaginal ultrasound looking for endometrial thickness. If less than 5mm than 95% can rule out endometrial hyperplasia. Biopsy is done with a pipelle, however it can miss areas of cancer because small sample. Dilation and curretage and hysteroscopy. Treatment depends on the pathology report. If there is no atypia you can give mirena and MPA which causes enodmetrium to go back to normal. Atypia requires hysterectomy. Type 1 endometrial cancer is endometroid adenocarcinoma. Endometrial Carcinoma Most common gyne cancer in women, especially in post menopausal. Type 1: Endometroid Adenocarinoma, due to estrogen secondary to endometrial hyperplasia. Type 2 is clear cell, serous carcinoma and there is no precursor, no precursor, more agressive and occurs much later (70s and 80s). Risk Factors related to unopposed estrogen. Risk factors are Cancers (Breast, Ovarian, Colon Cancer). Obesity, Late Menopause, Diabestes Mellitus, HTN, Nulliparity, Unopposed Estrogen due anovulation, Hormone replacement and tamoxifen. However, COC is protective. Signs and Symptoms in type 1 is postmenopausal bleeding. In type 2 there is bloating, bowel dysfunction, pelvic pressure. Staging is done by direct extension myometrium, serosa, bladder and large bowel. Or down cervix and vagina and finally fallopian tube. Lympahtic spread can go to pelvic para-aortic lymph nodes. Fallopian tube and into peritoneum. Hematogenous spread is liver and lungs. Stage 1 is when only in uterus. Stage 2 goes into cervix Stage 3 is when it goes into adjacent strucutres. Stage 4 is beyond pelvic cavity. Investigations are Transvaginal Ultrasound, Biopsy with pipelle, Dilation and curretage, and hysteroscopy. Treatment depends on staging, surgery of hysterectomy with bilateral salpingo-oopherectomy (BSO), pelvic wash and lymph nodes. Omentectomy and this can be done laparoscopy. If there is metastasis then also do chemotherapy and if it recurs than perform radiation therapy

New Radiation Therapy to Treat Gyno Cancers

Despite effective screenings like the Pap test and vaccines that prevent HPV, gynecological cancers are still a force to be reckoned with. These female cancers claim an estimated 28,000 lives a year. "When we talk about gynecologic cancers, we're trying to bring together a group of diseases that occur in women, except for breast cancer. This would include cervical cancer, ovarian cancer, uterine cancer," says Dr. Constantine Mantz, radiation oncologist on Lee Memorial Health System's medical staff. Long-term survivors, who have seen their cancers spread, were the catalysts for a new line of therapy. "The challenge for them is to try to at least slow down the progression of the cancer," says Dr. Mantz. Doctors in Southwest Florida offered women with late stage cancer a new treatment. Located inside the Regional Cancer Center, it's called Stereotactic Body Radiation. First used in brain cancer, SBRT uses sophisticated guidance that can track subtle tumor movement and deliver high doses of radiation-in a fraction of the time. "We're able to cut down the number of visits and at the same time deliver a much more potent and much more effective dose of radiation than we could the conventional way. Conventional course of treatment may be 20/25, 30/35, 40/45 visits. With stereotactic treatment we can compress all that down to a total number of one, two, three, four, five treatments," says Dr. Mantz. Results were exceptional this local study followed 36 women up to five years. "There is a group of patients who are long term survivors of advanced gynecologic cancer who will continue to enjoy a good quality of life for many years," says Dr. Mantz. Is it still the same chemotherapy used in peritoneal chemotherapy in this case? We have learned so much about endometrial cancer. We used to think it as one disease, but we have learned that even within a stage, and the cancer is confined only to the uterus and has not spread to the lymph nodes, there are differences in prognosis that depend on how the cancer looks underneath the microscope, which is called histology. We are learning to understand in different molecular features that endometrial cancer. We are even learning to type endometrial cancers between those that we feel are more hormonally driven, Type 1 cancers, versus those that are not hormonally driven. They express different molecular markers and look a little different under the microscope. With the help of a good expert pathologist, we try to decide what is the risk for this endometrial cancer.

Laparoscopic uterine fixation to spare fertility before pelvic radiation therapy

DESIGN: This video article uses a surgical case to demonstrate the detailed technique. Institutional review board approval was not required for this video presentation. SETTING: University hospital. PATIENT: A 26-year-old nulliparous female patient diagnosed with rectal adenocarcinoma and liver metastases. After the completion of right hepatectomy and neoadjuvant chemotherapy with FOLFOX regimen, pelvic radiation therapy was applied before tumor excision. INTERVENTION: The patient underwent laparoscopic ovarian transposition. During the same intervention, we fixed the uterus with three late resorbable sutures into the fascia of the anterior abdominal wall as cranial as possible to remove uterus away from radiation field. MAIN OUTCOME MEASURE: Value and feasibility of LUF. RESULTS: LUF is a feasible and effective technique. Estimated isodoses demonstrate that this intervention may decrease uterine consequences of pelvic radiotherapy. The patient did not report any specific pain or complication in the postoperative period. Three months after the completion of pelvic concurrent chemoradiation therapy, patient had normal menstrual periods. Hormonal blood tests were satisfactory. Ultrasound showed persistent ovarian function. CONCLUSION: Considering its simplicity of realization, LUF should be discussed in reproductive-aged patients before undergoing pelvic radiotherapy for anal or rectal cancer. A narrow collaboration is required between surgeons and radiation oncologists to optimize the treatment and to decrease side-effects of radiotherapy. Intensity-modulated radiation therapy may contribute to enhance a multimodal fertility-sparing strategy in young patient global management.
