A breast cyst is a fluid-filled sac within the breast. One breast can have one or more breast cysts. They're often described as round or oval lumps with distinct edges. In texture, a breast cyst usually feels like a soft grape or a water-filled balloon, but sometimes a breast cyst feels firm. Breast cysts can be painful and may be worrisome but are generally benign. They are most common in pre-menopausal women in their 30s or 40s. They usually disappear after menopause, but may persist or reappear when using hormone therapy. Breast cysts can be part of fibrocystic disease. The pain and swelling is usually worse in the second half of the menstrual cycle or during pregnancy. Fibroadenomas of the breast, are lumps composed of fibrous and glandular tissue. Because breast cancer can also appear as a lump, doctors may recommend a tissue sample (biopsy) to rule out cancer in older patients. Unlike typical lumps from breast cancer, fibroadenomas are easy to move, with clearly defined edges.Fibroadenomas are sometimes called breast mice or a breast mouse owing to their high mobility in the breasts. Breast fibroadenomas can be diagnosed early through clinical examination, ultrasound or mammography, and often a needle biopsy sample of the lump and treated by surgical excision. They are removed with a small margin of normal breast tissue if the preoperative clinical investigations are suggestive of the diagnosis. A small amount of normal tissue must be removed in case the lesion turns out to be a phyllodes tumour on microscopic examination. Treating breast cysts is usually not necessary unless they are painful or cause discomfort. In most cases, the discomfort they cause may be alleviated by draining the fluid from the cyst. The cysts form as a result of the growth of the milk glands and their size may range from smaller than a pea to larger than a ping pong ball. Small cysts cannot be felt during a physical examination, and some large cysts feel like lumps. However, most cysts, regardless of their size cannot be identified during physical exams. Surgical removal of the tumor is a common treatment for breast cancer. Sometimes in addition to removing the breast tissues, some or all of the lymph nodes are removed. The breast cancer surgery usually takes 1 to 3 hours. The main surgical procedures are Lumpectomy and Mastectomy. A Lumpectomy is performed on a small tumour that can be easily removed. In 2014, I was planning a preventative bilateral mastectomy to reduce my risk of breast cancer due to the BRCA1 genetic mutation. While "planning" I was diagnosed with Breast Cancer. The Woody Allen quote "If you want to make GOD laugh, tell him your plans" comes to mind. Before removing my breasts I wanted photographic evidence of what they looked like, but I didn't want mug shots of attempted murders. I wanted a photo that captured how I felt. So I decided to do a photo series that would express the emotion in each stage of the process. I wanted to change the images that women saw of mastectomies. No more Before and After photos. I wanted them to see the Evolution, the Life, and the HUMOR in this dark situation. Breast cancer is cancer that develops from breast tissue. Signs of breast cancer may include a lump in the breast, a change in breast shape, dimplingof the skin, fluid coming from the nipple, or a red scaly patch of skin. In those with distant spread of the disease, there may be bone pain, swollen lymph nodes, shortness of breath, or yellow skin. Signs and symptoms: The first noticeable symptom of breast cancer is typically a lump that feels different from the rest of the breast tissue. More than 80% of breast cancer cases are discovered when the woman feels a lump. The earliest breast cancers are detected by a mammogram. Lumps found in lymph nodes located in the armpits can also indicate breast cancer. Indications of breast cancer other than a lump may include thickening different from the other breast tissue, one breast becoming larger or lower, a nipple changing position or shape or becoming inverted, skin puckering or dimpling, a rash on or around a nipple, discharge from nipple/s, constant pain in part of the breast or armpit, and swelling beneath the armpit or around the collarbone. Pain ("mastodynia") is an unreliable tool in determining the presence or absence of breast cancer, but may be indicative of other breast health issues. Inflammatory breast cancer is a particular type of breast cancer which can pose a substantial diagnostic challenge. Symptoms may resemble a breast inflammation and may include itching, pain, swelling, nipple inversion, warmth and redness throughout the breast, as well as an orange-peel texture to the skin referred to as peau d'orange. As inflammatory breast cancer does not present as a lump there can sometimes be a delay in diagnosis. Another reported symptom complex of breast cancer is Paget's disease of the breast. This syndrome presents as skin changes resembling eczema, such as redness, discoloration, or mild flaking of the nipple skin. As Paget's disease of the breast advances, symptoms may include tingling, itching, increased sensitivity, burning, and pain. There may also be discharge from the nipple. Approximately half of women diagnosed with Paget's disease of the breast also have a lump in the breast. In rare cases, what initially appears as a fibroadenoma (hard, movable non-cancerous lump) could in fact be a phyllodes tumor. Phyllodes tumors are formed within the stroma (connective tissue) of the breast and contain glandular as well as stromal tissue. Phyllodes tumors are not staged in the usual sense; they are classified on the basis of their appearance under the microscope as benign, borderline, or malignant. Occasionally, breast cancer presents as metastatic disease—that is, cancer that has spread beyond the original organ. The symptoms caused by metastatic breast cancer will depend on the location of metastasis. Common sites of metastasis include bone, liver, lung and brain. Unexplained weight loss can occasionally signal breast cancer, as can symptoms of fevers or chills. Bone or joint pains can sometimes be manifestations of metastatic breast cancer, as can jaundice or neurological symptoms. These symptoms are called non-specific, meaning they could be manifestations of many other illnesses. Most symptoms of breast disorders, including most lumps, do not turn out to represent underlying breast cancer. Fewer than 20% of lumps, for example, are cancerous, and benign breast diseases such as mastitis and fibroadenoma of the breast are more common causes of breast disorder symptoms. Nevertheless, the appearance of a new symptom should be taken seriously by both patients and their doctors, because of the possibility of an underlying breast cancer at almost any age.

Radical mastectomy is a surgical procedure involving the removal of breast, underlying chest muscle (including pectoralis major and pectoralis minor), and lymph nodes of the axilla as a treatment for breast cancer. Breast cancer is the most common cancer among women today, and is primarily treated by surgery, particularly during the early twentieth century when the mastectomy was developed with success.[1] However, with the advancement of technology and surgical skills, the extent of mastectomies has been reduced. Less invasive mastectomies are employed today in comparison to those in the past. Nowadays, a combination of radiotherapy and breast conserving mastectomy are employed to optimize treatment. Modified Radical Mastectomy:: When treating breast cancer, a doctor’s goal is to remove all of the cancer -- or as much of it as possible. Surgery is one of the mainstays of treatment, and a procedure called modified radical mastectomy (MRM) is now a standard surgical treatment for early-stage breast cancers. It’s especially helpful for treating early-stage breast cancer that has spread to the lymph nodes. Studies show MRM is as effective as a traditional radical mastectomy, but takes much less of a toll on a woman’s appearance. Because of the success with MRMs, traditional radical mastectomies are rarely done today. What Is Modified Radical Mastectomy? MRM doesn’t remove chest muscles like a traditional radical mastectomy does. During the procedure, the surgeon removes the breast, including the skin, breast tissue, areola, and nipple, and most of the lymph nodes under the arm. The lining over the large muscle in the chest is also removed, but the muscle itself is left in place. Following a traditional radical mastectomy, women often have a hollow in the chest. Because the chest muscle is kept in place with an MRM, this doesn’t happen. While you are under general anesthesia, the surgeon will make a single incision across one side of the chest. The skin is pulled back, and the doctor will remove the entire breast tissue, along with the lining over the pectoralis major. Usually, he’ll remove some of the lymph nodes under your arm as well. The goal of the surgery is to remove the cancer but preserve as much of the skin and tissue as possible so that you can have breast reconstruction, if you chose. Although research has found MRM to be generally safe and effective, like all surgical procedures it can have risks. They include: Bleeding Infection Swelling of the arm Pockets of fluid forming underneath the incision (called seromas) Risks from general anesthesia Some people have numbness in the upper arm after surgery. It’s caused by damage to small nerves in the area where the lymph nodes are taken out. There is a good chance that you’ll regain most of the feeling in your arm over time. Lymph nodes that are removed will be sent to a lab to see whether the cancer has spread to them.

Many women use an ice pack wrapped in a towel after a lumpectomy or mastectomy to reduce pain and swelling. Dr. Billar reminds patients to wrap the ice in something to prevent burning of the skin. Wearing a supportive bra after a lumpectomy can help prevent excessive movement in the breast. Certain exercises can aid in recovery, too. Dr. Billar demonstrates some exercises and talks about their benefits. She had a mastectomy on the 24th September and wanted to record her thoughts after surgery to try to help de-mystify some of the treatment for breast cancer. Breast cancer is the most common cancer in the UK - 1 in 8 women will be diagnosed with it. Everyone's experience of a cancer diagnosis is different - for Victoria her approach - and progress so far - has been really positive and pragmatic. Best Way To Destroy Breast Cancer Tumors Without Chemo

Scientist in Europe have found a leap forward for breast cancer. One in eight women will develop breast cancer at some point in their lives, but a recent study in Amsterdam have found a way to get rid of it once and for all. Breast Cancer Breakthrough Research presented by Professor Nigel Bundred at the European Breast Cancer Conference in Amsterdam explained that they had tested the effectiveness of a pair of drugs known as Herceptin (a.k.a trastuzumab) and Lapatinib. These drugs are already used in breast cancer treatments, but this is the first time they were combined and used before surgery and chemotherapy. They found out that if these two drugs were combined together, they were able to eliminate some types of breast cancer in just 11 days. The study, which was funded by Cancer Research UK, proposed to use these drugs to combat a protein called HER2 (human epidermal growth factor receptor 2), which affects the growth and division of cancer cells. It’s also more likely to return than other cancer types. What also makes this treatment so appealing is the fact that it eliminates the need for chemotherapy and surgery. It also has some side effects like hair loss, vomiting and fatigue, making treatment less impactful on the body. Chemo is not entirely effective, nor is it the right choice for a lot of patients, so any alternatives are welcomed. Study Results 257 women with HER2 positive breast cancer were selected for the study. Half of them were put on the drug combo while the other half were a control group. After two weeks, 11% of the participants receiving the combo had no cancer cells remaining, while another 17% saw drastically reduced tumors. The control group, which received just Herceptin, saw none of the participants cancer-free and only 3% experiencing a reduction in tumor size. Clearly, the combination of drugs was powerful. Unfortunately, Herceptin's licensing only allows it to be available in conjunction with chemotherapy. Hopefully, the results of this study can change that.

Hidden Scars after Breast Surgery

It can be a scary experience. Many breast cancer patients undergo surgery to remove part or all of the breast tissue. Doctors say new techniques in the operating room are sending women home with hidden scars after surgery. “I think when you hear the diagnosis “cancer” your mind immediately goes to the worst place. And you think of these pictures you’ve seen of these horribly disfigured women and again at that time we didn’t have the medicine we have now, we didn’t have the best radiation therapy, everything relied on surgery. So the surgeries were radical and drastic and very disfiguring,” said Dr. David Rock, a breast surgeon on the medical staff of Lee Health. Dr. Rock says if women have to undergo breast cancer surgery, his goal is to make sure they leave cancer free and confident of how they look. “These women look in the mirror, they have normal lives hopefully after their cancer diagnosis and treatments, and so we want to minimize the impact on them.” He does this by performing a hidden scar surgery. It’s a way of doing the same cancer operation, whether it’s a lumpectomy or a mastectomy, but hiding the scar so it can’t be seen from the outside. “If it’s a lumpectomy, we use the natural camouflage areas of the breast to hide that scar, whether that’s around the nipple or underneath the breast, or out above the underarm. For a mastectomy we make an incision that’s about two inches long, along where the underwire would be just to the side of the breast and come in and take out all the breast tissue and we try to leave all the skin and even the nipple in most cases and replace that with an implant or the patient’s own tissue,” said Dr. Rock. Not every patient qualifies for hidden scar surgery, but Dr. Rock says most of his procedures are successful in hiding the scar. “Now because the medicine has gotten so good we can really scale back the surgery. We’re still taking out the tumor. The absolute first goal of the surgery is to take out the tumor and cure the cancer,” said Dr. Rock. With breast cancer patients living longer, he says it’s important for patients to be able to look in the mirror and not only see a breast cancer survivor, but also a woman. The Welsh Centre for Burns and Plastic Surgery at Morriston Hospital held its second Breast Reconstruction Awareness (BRA) Day in Swansea. The event, attended by 87 women of all ages, was a huge success. Women who had been diagnosed with breast cancer and had breast reconstruction, met other women who were about to go through the same procedure. Also on hand were 33 healthcare professionals providing advice and answering questions. At the event were three women who had different types of breast reconstruction. They happily chatted to other women sharing their stories about implants, using parts of your back and tummy to build a breast, nipple reconstruction and tattooing. Thousands of breast cancer patients could be spared chemo

Chemotherapy won’t be necessary for thousands of women after a breakthrough in breast cancer research. A trial of more than 10,000 women with the most common form of early breast cancer found the treatment was unnecessary for many after surgery. The findings will lead to a ‘fundamental change’ in the way the disease is treated, a leading oncologist said, with an estimated 3,000 to 5,000 UK women likely to avoid chemotherapy every year as a result. Charity Breast Cancer Care said it was a ‘life-changing breakthrough’. More than 20,000 women in the UK are diagnosed with hormone-receptor positive, HER2-negative, node-negative breast cancer annually. Around half of these patients would historically receive chemotherapy after having surgery to remove their tumour, to prevent recurrence of the disease. However, the results of the TAILORx trial show that only 30% of women with this particular form of early-stage breast cancer benefit from the treatment. The study, presented at the American Society of Clinical Oncology (ASCO) annual meeting in Chicago, is thought to be the largest breast cancer treatment trial ever conducted. Dr Alistair Ring, consultant medical oncologist at the Royal Marsden NHS Hospital, in London, said: ‘I think this is a fundamental change in the way we treat women with early-stage breast cancer and will lead to a considerable number of women no longer needing to have chemotherapy. Nine-year survival rates were 93.9% without chemotherapy and 93.8% with chemotherapy, the study found. Dr Ring said the trial’s reuslts would likely have an immediate impact on UK practice and represented a significant shake-up in the treatment of early-stage breast cancer. Lumpectomy Breast Conserving Surgery - Breast Cancer Surgery | Breast Reduction Surgery - Lumpectomy Breast-conserving surgery (BCS) is an operation to remove the cancer while leaving as much normal breast as possible. Some surrounding healthy tissue and lymph nodes are usually also removed. How much of the breast is removed depends on the size and location of the tumor and other factors. Breast-conserving surgery is sometimes called lumpectomy, quadrantectomy, partial mastectomy, or segmental mastectomy. It’s often an option for a woman with early-stage cancer, and allows her to keep most of her breast. Breast-conserving surgery allows a woman to keep most of her breast, but makes it likely she will also need radiation. Not all women with breast cancer are candidates for BCS. Talk to your doctor to find out whether BCS is an option for you. Studies show that choosing BCS (plus radiation) over mastectomy does not affect a woman’s chances of long-term survival. If you think you may want breast reconstruction, talk to your doctor before your breast cancer surgery. After BCS, most women will have radiation therapy. Some women might get other treatments as well, such as hormone therapy or chemotherapy. Side effects of BCS may include pain, a scar and/or dimple where the tumor was removed, a firm or hard surgical scar, and sometimes lymphedema, a type of swelling, in the arm. Every year, one million women around the world are diagnosed with breast cancer, and almost 200,000 others are told they have ovarian cancer. The decisions for treatment are more difficult when the women carry genetic mutations called BRCA1 and 2, mutations that could also increase a woman's risk for cervical, uterine, pancreatic and other cancers. Dr. Daniel Barnas of Borgess Surgical Specialties explains new innovations in breast cancer surgery, including oncoplastic breast preservation and nipple sparing mastectomy. I am breast surgical oncologist at Borgess Medical Center. Most general surgeons are not trained to do oncoplastic surgery or nipple-sparing mastectomies. When I was doing general surgery, I didn’t have any of that expertise, and that’s why I went back and did a fellowship to learn those more modern techniques with tissue rearrangement and sparing the nipple. A lot of the things that I was focusing on was oncoplastic breast surgery, which is surgery to repair the defect once the lumpectomy is performed so that we can restore them to normal cosmetic outcomes and also to do nipple-sparing mastectomies, which are a little more complicated mastectomy to preserve the nipple when you are doing a mastectomy. It’s a very emotional operation where there is other cancers that people have that there’s not such an emotional component. That’s one of the things that drew me to breast surgery was that emotional tie that you get with the patient and with the surgery, and some people have a much stronger tie than others, and that’s why nipple-sparing mastectomy can help a lot of women because it has been shown to kind of preserve that emotional connection. In a traditional lumpectomy, you’re going to remove the cancerous lesion with some surrounding, normal breast tissue to get a negative margin. In oncoplastic breast surgery, you are going to do that same procedure, but you're going to take a little bit more tissue to try to cut down on that re-excision rate, and then you’re also going to rearrange the breast tissue within the breast that remains so that we can restore them to normal cosmetic outcomes. This is an instrument called the margin probe, and this is actually what we use in surgery to look at that margin, and margins are typically checked in pathology. So we don’t know the margin until we get the pathology report back several days after surgery. So it means if there’s a closer positive margin, we have to go back and do additional surgery. Here we’re the only hospital in Kalamazoo and one of only four hospitals in the state of Michigan that are going to have a special device that checks the margin while we are operating. So once the tumor has been removed, we can actually take it to the back table and check the margin, and then it will tell us if we need to take more tissue or not at that point in time, and it lowers the re-excision rate. So the rate that we have to take patients back to the operating room—we can go from around to 25% down into the single digit percentile. So it’s saving patients additional surgeries. Here at Borgess we’re an NAPBC-certified breast center, and that is an additional certification that hospitals can seek to say that they are specializing breast cancer treatment so that they are following the national guidelines on how to treat breast cancer. They are performing research on breast cancer, and we’re doing everything—the state-of-the-art techniques to help women with their breast cancer diagnosis.

Hyperthermia is approved by the Food and Drug Administration and is being offered at Duke University, UC San Francisco and Saint Louis University. Dr. Bicher says, 80% of his early stage patients have survived at least five years after treatment. But other cancer experts like Oncologist Kristi Funk doubts the effectiveness of hyperthermia.
